Climate change drives a decline in global grazing systems.

Grazing systems represent the most extensive production systems in the world and are highly sensitive to climate change. However, their global-scale sensitivity and vulnerability to climate impacts remain poorly understood. Here, we apply the safe climatic space framework to assess how changes in core climatic drivers of grazing suitability, including temperature, precipitation, humidity, and wind speed, will reshape global grassland-based grazing systems. Our analysis projects a net decline of 36 to 50% of areas in climate suitability for grazing by 2100, accompanied by inter- and intracontin

Implementation of integrated disease surveillance and response systems in West Africa: lessons learned and future directions.

The Integrated Disease Surveillance and Response (IDSR) framework, introduced by the WHO in 1998, aimed to unify disease surveillance across West Africa, replacing fragmented systems. However, challenges such as limited real-time reporting, inadequate data collection and workforce shortages continue to impede disease control and outbreak response. The resurgence of infectious diseases like Ebola, cholera, COVID-19 and monkeypox highlights the need to strengthen IDSR systems for effective public health management. This article reviews IDSR implementation in West Africa, identifying persistent g

Alabama's LePage decision, the future of ex vivo embryos, and more.

In February 2024 the Alabama Supreme Court held that the destruction of frozen embryos stored at an in vitro fertilization (IVF) center could be the basis of a wrongful death suit by the prospective parents under Alabama law. The opinion caused a bipartisan uproar that led to its effective overturning by the Alabama legislature and governor 19 days after it was issued. The Alabama Supreme Court's unexpected intervention in IVF shows the surprising ways a mishap in a clinic can trigger a collision of originalist jurisprudence, judicial rhetoric, political mobilization, and media amplification i

Strengthening health systems to reduce adolescent fertility: an analysis of Chile's experience, 2010-2023.

Adolescent pregnancy remains a persistent and inequitable in Latin America. Chile has achieved one of the fastest declines in adolescent fertility in the region, offering insights into how policy and health system responses may accelerate progress.MethodsWe conducted a national descriptive-analytical case study using a mixed-methods approach to examine policies, programs, and health system transformations implemented in Chile between 2010 and 2023. Data sources included administrative data, repeated cross-sectional surveys, policy documents, and scientific literature. The analysis was guided b

Firearm violence and racial capitalism: A conceptual framework for understanding the persistence and prevalence of gun violence in the United States.

Firearm violence is a leading cause of death in the United States and is marked by profound racial and socioeconomic disparities. Existing research has identified many of the conditions that make interpersonal firearm violence more likely, and intervention research shows that firearm regulation, targeted policing, and community-based programs can reduce harm. Yet firearm violence remains high, persistent, and unequally distributed. This article develops a racial capitalism framework to explain that persistence. Building on structural racism approaches, it asks how the conditions associated wit

Chile's National Center for Health Information Systems: A Public-Private Partnership to Foster Health Care Information Interoperability.

Despite the continuous technical advancements around health information standards, a critical component to their widespread adoption involves political agreement between a diverse set of stakeholders. Countries that have addressed this issue have used diverse strategies. In this vision paper we present the path that Chile is taking to establish a national program to implement health information standards and achieve interoperability. The Chilean government established an inter-agency program to define the current interoperability situation, existing gaps, barriers, and facilitators for interop

Electoral politics influence expansion and titling of informal urban settlements.

Understanding what and who shapes urban growth and how it unfolds in the Global South is critical to ensure theories and predictive models of urban expansion and, consequently, global environmental change, are salient to contemporary contexts. Much of the urban expansion in the Global South occurs in informal settlements where urbanization is not officially permitted and residents often lack access to basic services and infrastructure. As a result, formalizing settlements becomes an asset for political transactions where politicians provide titles and improve conditions in exchange for residen

Ageing with(out) kin: Migration, longing, and disenchantment in Punjab.

Drawing on an ethnography with older adults in the northern Indian city of Patiala and its peripheral areas, this paper addresses three interconnected questions. How do older adults in Punjab experience ageing in the absence of kin members who migrate transnationally? How does migration reshape their perceptions of care, belonging, family reconfiguration, and other forms of social relatedness? How do these experiences reflect broader socio-political shifts in Punjab, both culturally and politically? First, the paper examines the paradox of transnational migration. Older adults actively facilit

When formal governance persists but resilience does not: health system governance and resilience in Yemen across a decade of conflict and crisis (2014-2025).

BACKGROUND: Yemen's health system has faced prolonged shocks, including war, epidemics, and COVID-19, challenging both governance and resilience. This study examines how formal governance structures and institutional capacities shaped the system's ability to anticipate, absorb, adapt, learn, and transform across a decade of crisis (2014-2025). METHODS: We conducted a longitudinal qualitative analysis based on interviews with health system leaders and technical actors. Using a resilience-capacity framework, we analyzed governance functions across four phases: pre-conflict period (pre-2014), co

[Transfusion risk in sub-Saharan Africa: current situation and challenges specific to the Democratic Republic of Congo].

INTRODUCTION: Ensuring the safety of blood transfusions remains a significant health challenge in sub-Saharan Africa (SSA), especially in the Democratic Republic of Congo (DRC). Political instability, inadequate infrastructure, and the absence of comprehensive hemovigilance systems exacerbate the risks. This situation exposes blood transfusion recipients to post-transfusion complications, especially infectious ones, that could be avoided. Additionally, sociocultural mistrust of blood donation contributes to the shortage of safe blood products. This narrative review aims to address the current

A review of climate change and cardiovascular diseases in the Indian policy context.

There is growing evidence that climate change adversely affects human health. Multiple diseases are sensitive to climate change, including cardiovascular diseases (CVDs), which are also the leading cause of death globally. Countries such as India face a compounded challenge, with a growing burden of CVDs and a high vulnerability to climate change, requiring a co-ordinated, multi-sectoral response. In this framework synthesis, we analysed whether and how CVDs are addressed with respect to climate change in the Indian policy space. We identified 10 relevant national-level policies, which were an

[Psychiatric disorders caused by global social change. Traumatization among the inhabitants of the former GDR].

More than a decennium after the reunion of East and West Germany, the psychological sequelae of traumatic experiences of East Germans are still evident. There were not only about 300 000 people imprisoned for political reasons in the German Democratic Republic between 1945 and 1989, partly exposed to physical and psychological torture, but also much more people subject of subthreshold traumatic experiences by harassment in public and private life. As a result, under a psychiatric perspective it should be taken into account not only specific post-traumatic stress disorders (PTSD), characterized

[Norwegian psychiatry and Nazism. A historical project of current interest].

In 1945 the Ministry of Justice appointed a committee to conduct a psychiatric examination of Norwegian Nazis. This was done after a proposal from the Norwegian Psychiatric Association on the initiative of Professor Langfeldt, who at that time was head of the Department of Psychiatry at the University of Oslo. An investigation based on a crosscheck between membership of the Nazi party and the Norwegian case register for psychoses for the period 1926-1940 showed 30% more psychoses among the Nazis than in the population as a whole. Other studies tended to support the view that there was a high p

Cervical cancer in Bulgaria since EU accession in 2007: a struggle in the face of political instability.

BACKGROUND: Bulgaria has one of the highest cervical cancer incidence rates in the EU, driven by persistently low screening uptake and HPV vaccination coverage. Despite the preventable nature of the disease, efforts to implement effective prevention strategies have been undermined by political instability, fragmented governance, and systemic health system weaknesses. REFORM CONTENT: Since EU accession in 2007, Bulgaria has launched several initiatives, including the "Stop and Get Checked" programme and the National Program for Primary Prevention of Cervical Cancer. These efforts were supporte

Power and Other Commercial Determinants of Health: An Empirical Study of the Australian Food, Alcohol, and Gambling Industries.

BACKGROUND: Commercial determinants of health (CDoH) represent a critical frame for exploring undue corporate and commercial influence over health. Power lenses are integral to understanding CDoH. Impacts of food, alcohol, and gambling industries are observable CDoH outcomes. This study aims to inform understanding of the systems and institutions of commercial and/or corporate forces working within the Australian food, alcohol, and gambling industries that influence health and well-being, including broader discourses materialised via these systems and institutions. METHODS: Twenty semi-struct

Organizational capacities of national pharmacovigilance centres in Africa: assessment of resource elements associated with successful and unsuccessful pharmacovigilance experiences.

BACKGROUND: National pharmacovigilance centres (national centres) are gradually gaining visibility as part of the healthcare delivery system in Africa. As does happen in high-income countries, it is assumed that national centres can play a central coordinating role in their national pharmacovigilance (PV) systems. However, there are no studies that have investigated whether national centres in Africa have sufficient organizational capacity to deliver on this mandate and previous studies have reported challenges such as lack of funding, political will and adequate human resources. We conducted

Financial and Economic Costs of the Elimination and Eradication of Onchocerciasis (River Blindness) in Africa.

BACKGROUND: Onchocerciasis (river blindness) is a parasitic disease transmitted by blackflies. Symptoms include severe itching, skin lesions, and vision impairment including blindness. More than 99% of all cases are concentrated in sub-Saharan Africa. Fortunately, vector control and community-directed treatment with ivermectin have significantly decreased morbidity, and the treatment goal is shifting from control to elimination in Africa. METHODS: We estimated financial resources and societal opportunity costs associated with scaling up community-directed treatment with ivermectin and impleme

Absorptive capacity and disbursements by the Global Fund to Fight AIDS, Tuberculosis and Malaria: analysis of grant implementation.

BACKGROUND: The Global Fund to Fight AIDS, Tuberculosis and Malaria was launched in 2002 to attract and rapidly disburse money to fight these diseases. However, some commentators believe that poor countries cannot effectively use such resources to increase delivery of their health programmes-referred to as a lack of absorptive capacity. We aimed to investigate the major determinants of grant implementation in developing countries. METHODS: With information available publicly on the Global Fund's website, we did random-effects analysis to investigate the effect of grant characteristics, types
