Stability of polysomnography for one year and longer in children with sickle cell disease.

STUDY OBJECTIVES: Serious morbidity may be linked to sleep disordered breathing (SDB) among children with sickle cell disease (SCD). We investigated the stability of polysomnography (PSG) results among children not having acute complications of SCD. METHODS: Two PSGs were performed on a subsample of 63 children 4 to 18 years of age from the Sleep and Asthma Cohort Study. All had Hb SS or HbSβ(0) disease. Two PSGs were compared for 45 subjects. Excluded from comparison were 18 children who had begun transfusions or hydroxyurea, had an adenotonsillectomy between the PSGs, or had a pain crisis o…

PLM detection by actigraphy compared to polysomnography: a validation and comparison of two actigraphs.

OBJECTIVE: To compare periodic leg movement (PLM) counts obtained with polysomnography (PSG) to those obtained from actigraphy with two devices (Actiwatch and PAM-RL). METHODS: Twenty-four patients underwent full night actigraphy with Actiwatch from both legs and simultaneous PSG. Out of these patients, 10 had additional actigraphy with PAM-RL. Bilateral and unilateral PLM indices (PLMI) for both actigraphs were calculated for time in bed and compared to polysomnographic PLMI. Additionally, a comparison between the two different actigraphs was performed. RESULTS: PLMI obtained with Actiwatch…

Omission of polysomnography in treatment of snoring: common reasons and medico-legal implications.

Although polysomnography (PSG) is an important investigation in the treatment of snorers, it was observed that a large number of patients did not have pre-operative PSG assessment in a tertiary hospital in Singapore. Of the 118 Asian patients who underwent surgery for snoring from January 1997 to December 1998, 36 (30.5 per cent) of patients did not have pre-operative PSG and only 21 (17.8 per cent) of patients had post-operative PSG. In this cohort, 43 (36.4 per cent) patients presented with snoring as their only complaint and not associated with symptoms indicative of obstructive sleep apnoe…

Comparison of polysomnography with ResCare Autoset in the diagnosis of the sleep apnoea/hypopnoea syndrome.

BACKGROUND: Increasing referral numbers make the development of simplified accurate methods of diagnosing the sleep apnoea/hypopnoea syndrome highly desirable. The accuracy of one such system--the ResCare Autoset--has been examined. METHODS: Thirty one consecutive patients assessed by polysomnography had simultaneous monitoring of their respiratory pattern using the Autoset system. The Autoset detects episodes of flattening of the flow/time profile using nasal cannulae. RESULTS: There was a good correlation (r = 0.85) between the number of apnoeas+hypopnoeas/hour in bed recorded using polyso…

Age-dependent association between parent-reported snoring and polysomnography findings in children.

PURPOSE: The initial identification of children at risk for sleep disordered breathing (SDB) commonly relies on parental reports of symptoms, particularly snoring. This study aims to assess how age affects the association between parent-reported snoring and SDB-related findings on polysomnography (PSG). We hypothesized that parent-reported snoring correlates more strongly with SDB-related findings on PSG in younger children. METHODS: This study was part of the Child Sleep project, a longitudinal birth cohort study on children born at Tampere University Hospital, which was carried out between …

Predicting Polysomnography Parameters from Anthropometric Features and Breathing Sounds Recorded during Wakefulness.

BACKGROUND: The apnea/hypopnea index (AHI) is the primary outcome of a polysomnography assessment (PSG) for determining obstructive sleep apnea (OSA) severity. However, other OSA severity parameters (i.e., total arousal index, mean oxygen saturation (SpO2%), etc.) are crucial for a full diagnosis of OSA and deciding on a treatment option. PSG assessments and home sleep tests measure these parameters, but there is no screening tool to estimate or predict the OSA severity parameters other than the AHI. In this study, we investigated whether a combination of breathing sounds recorded during wakef…

Polysomnography using abbreviated signal montages: impact on sleep and cortical arousal scoring.

OBJECTIVE: This study examined the impact of using two abbreviated signal montages on the accuracy, precision and inter-scorer reliability of polysomnography (PSG) sleep and arousal scoring, compared to a standard reference montage, in a cohort of patients investigated for obstructive sleep apnoea (OSA). One abbreviated montage incorporated two signals dedicated to sleep and arousal scoring, and the other incorporated a single signal. METHODS: Four scorers from two laboratories each scored 15 PSGS four times in random order: once using each abbreviated montage and twice using the reference mo…

Unattended home-based polysomnography for sleep disordered breathing: current concepts and perspectives.

Recently, portable sleep recording devices became an accepted alternative to polysomnography (PSG) for obstructive sleep apnea (OSA) diagnosis in patients with a high pre-test probability of moderate to severe OSA but home polysomnography (H-PSG) was not recommended because there were insufficient data. The present review has analysed six prospective randomized crossover studies comparing H-PSG to in-lab PSG. These studies convincingly showed that H-PSG allows complete sleep evaluation. The quality of patients' sleep tends to be better at home. H-PSG is accurate for OSA diagnosis and the failu…

Polysomnography underestimates altered cardiac autonomic control in patients with obstructive sleep apnea.

BACKGROUND: Repetitive nocturnal sympathetic activation during episodes of apnea and postapneic hyperventilation increases cardiovascular risk. The effects of hypopnea and non-apneic, non-hypopneic intervals before and after hypopnea/apnea on sympathico-vagal balance have not been assessed yet. HYPOTHESIS: Hypopnea and non-apneic, non-hypopneic intervals before and after hypopnea/apnea cause increased sympathetic activity when compared to normal respiration in nonREM stages 2–4. METHODS: A total of 34 patients were studied using in-laboratory polysomnography including continuous ECG recordin…

Sleep-stage- and event-specific interpretation during concurrent natural sleep endoscopy and polysomnography.

PURPOSE: Polysomnography (PSG) defines sleep stage, respiratory event type, and respiratory effort, but does not directly identify the visible anatomic correlate of individual events. We examined whether concurrent natural sleep endoscopy (NSE) with PSG could support event-level interpretation of selected sleep-disordered breathing events. METHODS: Four adults were selected from archived synchronized NSE-PSG recordings acquired between 2007 and 2009. Cases were chosen to illustrate interpretive scenarios in which PSG context changed, refined, or constrained visible airway interpretation. RES…

Clinical utility of limited channel sleep studies versus polysomnography for obstructive sleep apnoea.

BACKGROUND: Obstructive sleep apnoea (OSA) is a common cause of sleep disturbance, characterised by the presence of repetitive upper airway obstruction during sleep. OSA is associated with sleepiness during the day, reduced quality of life and an increased risk of cardiovascular disease. OSA can be diagnosed using several different strategies. The current reference test is fully supervised polysomnography, which is expensive and time-consuming. Other diagnostic tests, referred to as limited channel sleep studies because they include fewer parameters than polysomnography, are less resource-inte…

Validation of manually scored multichannel frontal electroencephalography against polysomnography in a paediatric cohort.

Polysomnography is the only internationally recognized method to diagnose paediatric obstructive sleep apnea, thus, simpler and more cost-effective diagnostic tools are urgently needed. This study aimed to validate the manual scoring of frontal self-applicable electroencephalography against polysomnography in a paediatric cohort. The polysomnography and the frontal electroencephalography were simultaneously recorded for 1 night (n = 102) in 10-13-year-old children. Scoring was performed according to the American Academy of Sleep Medicine rules, with minor adjustments to the frontal electroence…
