Long-acting formulation strategies for protein and peptide delivery in the treatment of PSED.

The invigoration of protein and peptides in serious eye disease includes age-related macular degeneration, choroidal neovascularization, retinal neovascularization, and diabetic retinopathy. The transportation of macromolecules like aptamers, recombinant proteins, and monoclonal antibodies to the posterior segment of the eye is challenging due to their high molecular weight, rapid degradation, and low solubility. Moreover, it requires frequent administration for prolonged therapy. The long-acting novel formulation strategies are helpful to overcome these issues and provide superior therapy. It

Expanding the horizon of oculomics: A narrative review on the emerging roles of the anterior segment and mobile health.

Oculomics uses the eye to gain insights into systemic health. Although initially focused on retinal imaging, oculomics now studies various ocular signs beyond the retina for their associations with systemic health. The posterior segment receives significant attention because of direct visibility of the optic nerve, retinal vasculature, and neurosensory retina; however, anterior segment oculomics may catalyze the growth of oculomics in clinical practice, particularly considering recent developments in mobile health (mHealth), increasing healthcare costs, and shifting medical paradigms. The ante

Ocular Manifestation in Systemic Sclerosis-A Literature Review.

Systemic sclerosis (SSc) is a chronic autoimmune connective tissue disease that affects more than 2 million people worldwide. It manifests through vasculopathy, an abnormal immunological response, and fibrosis leading to dysfunction of the multiple organs. The disease is categorized into two subtypes: limited cutaneous SSc and diffuse cutaneous SSc. Scleroderma can affect vital organs with respiratory, cardiac, renal, ocular, and dermatological complications. The ocular manifestations of the disease can occur in the anterior and posterior segments of the eye. Changes in the anterior segment re

Challenges and opportunities for drug delivery to the posterior of the eye.

Drug delivery to the posterior segment of the eye remains challenging even though the eye is readily accessible. Its unique and complex anatomy and physiology contribute to the limited options for drug delivery via non-invasive topical treatment, which is the prevalent ophthalmic treatment. To treat the most common retinal diseases, intravitreal (IVT) injection has been a common and effective therapy. With the advancement of nanotechnologies, novel formulations and drug delivery systems are being developed to treat posterior segment diseases. Here, we discuss the recent advancement in ocular d

Diagnosis of ocular disease in Ibadan by ultrasound.

15 patients with ocular disease, in whom views of posterior segment of the eyes could not be obtained underwent ultrasonography of the eyes to aid in the diagnosis and further management of the patients. In some cases the ultrasound was then compared with the histological diagnosis when the eye was removed and in other cases to the ophthalmoscopic examination when a better view of the posterior segment could be obtained. This was done as a pilot study to ascertain the level of accuracy of our ultrasonic diagnosis and how much it could be depended upon for the very important decision of removal

Latest trends & strategies in ocular drug delivery.

Ocular drug delivery is one of the most challenging routes of administration, and this may be attributed to the complex interplay of ocular barriers and clearance mechanisms that restrict therapeutic payload residence. Most of the currently approved products that ameliorate ocular disease conditions are topical, i.e., delivering therapeutics to the outside anterior segment of the eye. This site of administration works well for certain conditions such as local infections but due to the presence of numerous ocular barriers, the permeation of therapeutics to the posterior segment of the eye is li

A single molecule carrier for ocular posterior segment diseases.

Eye drops are envisaged as the most promising non-invasive formulation for the treatment of the ocular posterior segment diseases, while it is hindered by a series of complex ocular barriers, both static and dynamic in nature. In this context, we propose a single molecule nanomedicine based on host-guest chemistry to achieve highly efficient drug delivery targeted to ocular posterior segment. Sulfonated azocalix[4]arene (SAC4A) serves as the single molecule carrier, owing the multiple features of small size (24.0 Å in length, 21.2 Å in width, 14.8 Å in height with a Van der Waals volume of 930

Ocular Ciliopathies: Genetic and Mechanistic Insights into Developing Therapies.

Developing suitable medicines for genetic diseases requires a detailed understanding of not only the pathways that cause the disease, but also the identification of the genetic components involved in disease manifestation. This article focuses on the complexities associated with ocular ciliopathies - a class of debilitating disorders of the eye caused by ciliary dysfunction. Ciliated cell types have been identified in both the anterior and posterior segments of the eye. Photoreceptors (rods and cones) are the most studied ciliated neurons in the retina, which is located in the posterior eye. T

Computerized tomography in the evaluation of penetrating ocular injuries.

An evaluation was made of the cases of 80 consecutive patients (82 eyes) who underwent acute surgical intervention for penetrating eye injuries. Of these cases, 46 patients had computerized tomography (CT) of the orbit performed in addition to routine preoperative ophthalmic examination. Eyes with more severe injuries and eyes in which the posterior segment was not visualized on the preoperative ophthalmic examination were more likely to undergo CT evaluation. CT scanning frequently demonstrated posterior segment pathology not observable by ophthalmic examination. The following CT findings wer

Chronic myeloid leukemia diagnosed from the course of diabetic retinopathy.

We report a case of chronic myeloid leukemia (CML), which was diagnosed during the management of proliferative retinopathy (PR) in a patient with type I diabetes mellitus due to the fulminant nature of the PR. This case highlights the importance of vigilance on the part of the ophthalmologist in the diagnosis of co-existing systemic disorders, notably hematological malignancy, which aggravates the posterior segment vasculopathy of the eye and the management of which is crucial for the patient. We also describe a short literature review on the clinical features, mechanism of the posterior segme

Iris coloboma in one eye and pigment dispersion syndrome in the fellow eye.

We report a case of a 43-year-old patient with coloboma of the iris, zonule, ciliary body, choroid and retina in the right eye and pigment dispersion syndrome in the left eye. Considering the hypothesis of the pigment dispersion syndrome pathogenesis in which a difference of pressures in the anterior and posterior chambers creates a posterior convexity of the iris leading to reverse pupillary block, iris touch and consequently causing pigment dispersion, we suggest that the presence of an iris coloboma, by equalising the pressures in the two chambers, prevented the onset of syndrome in that ey
