Precision medicine for depression: Improving treatment response and remission.

This review synthesises current knowledge to improve understanding of the pathophysiology of major depressive disorder (MDD) and optimise diagnostic, therapeutic and prognostic approaches. It examines the interplay between genetic, epigenetic, inflammatory, neurotransmitter and gut microbiome factors, together with environmental stressors and different clinical symptom presentations, in shaping MDD presentation and treatment response. Studies have revealed potential biomarkers predictive of treatment response, allowing differentiation of MDD subtypes and facilitating remission monitoring. Whil

Multiomics strategy-based obesity biomarkers discovery for precision medicine.

Obesity is a multifaceted metabolic disorder characterized by dysregulated glucose and lipid metabolism, often comorbid with conditions such as diabetes, hypertension, hyperlipidemia, cardiovascular diseases, and cancers. Due to its diverse etiological factors and inherent heterogeneity, obesity poses significant challenges for management. The advent of multiomics technologies has opened new avenues for a deeper exploration of the molecular underpinnings and clinical biomarkers of obesity, improving our ability to predict and monitor associated metabolic syndromes. However, a holistic understa

Characterizing PTSD Using Electrophysiology: Towards A Precision Medicine Approach.

Objective. Resting-state EEG measures have shown potential in distinguishing individuals with PTSD from healthy controls. ERP components such as N2, P3, and late positive potential have been consistently linked to cognitive abnormalities in PTSD, especially in tasks involving emotional or trauma-related stimuli. However, meta-analyses have reported inconsistent findings. The understanding of biomarkers that can classify the varied symptoms of PTSD remains limited. This study aimed to develop a concise set of electrophysiological biomarkers, using neutral cognitive tasks, that could be applied

Pathophysiology of dilated cardiomyopathy: from mechanisms to precision medicine.

Dilated cardiomyopathy (DCM) is a complex disease with multiple causes and various pathogenic mechanisms. Despite improvements in the prognosis of patients with DCM in the past decade, this condition remains a leading cause of heart failure and premature death. Conventional treatment for DCM is based on the foundational therapies for heart failure with reduced ejection fraction. However, increasingly, attention is being directed towards individualized treatments and precision medicine. The ability to confirm genetic causality is gradually being complemented by an increased understanding of gen…

A RAG Chatbot for Precision Medicine of Multiple Myeloma

The advent of precision medicine has revolutionized cancer treatment by integrating individual genetic, lifestyle, and environmental factors to tailor patient care (Huang et al., 2020; Ginsburg and Phillips, 2018). However, the complexity and heterogeneity of diseases like Multiple Myeloma (MM) pose significant challenges in leveraging the vast amounts of genomic data and biomedical literature available for personalized treatment planning (Rajkumar, 2014; Röllig et al., 2015). To address this, we present an innovative Retrieval-Augmented Generation (RAG) based chatbot framework that harnesses …

Precision medicine: a new era for inner ear diseases.

The inner ear is the organ responsible for hearing and balance. Inner ear dysfunction can be the result of infection, trauma, ototoxic drugs … and targets cannot be controlled. Overcoming these limitations is crucial for successful inner ear precision medicine. Recently, notable advancements in microneedle technologies offer the potential for safe sampling of inner ear fluids and local treatment. Ultrasharp microneedles can reach the inner ear fluids with minimal damage to the organ, collect μl amounts of …

NF1 alterations in cancers: therapeutic implications in precision medicine.

INTRODUCTION: NF1 is a tumor suppressor gene encoding neurofibromin, an inhibitor of the RAS/MAPK and PI3K-AKT-mTOR signaling pathways. NF1 germline pathogenic variants cause the tumor predisposition syndrome neurofibromatosis type 1. Targeted therapies (MEK inhibitors) have been approved for benign nerve sheath tumors in neurofibromatosis type 1 patients. NF1 somatic alterations are present in ~5% of all human sporadic cancers. In melanomas, acute myeloid leukemias and lung adenocarcinomas, the NF1 somatic alteration frequency is higher (~15%). However, to date, the therapeutic impact of NF1

Resiniferatoxin: Nature's Precision Medicine to Silence TRPV1-Positive Afferents.

Resiniferatoxin (RTX) is an ultrapotent capsaicin analog with a unique spectrum of pharmacological actions. The therapeutic window of RTX is broad, allowing for the full desensitization of pain perception and neurogenic inflammation without causing unacceptable side effects. Intravesical RTX was shown to restore continence in a subset of patients with idiopathic and neurogenic detrusor overactivity. RTX can also ablate sensory neurons as a "molecular scalpel" to achieve permanent analgesia. This targeted (intrathecal or epidural) RTX therapy holds great promise in cancer pain management. Intra

Quantitative ctDNA Detection in Hepatoblastoma: Implications for Precision Medicine.

Hepatoblastoma is characterized by driver mutations in CTNNB1, making it an attractive biomarker for a liquid biopsy approach utilizing circulating tumor DNA (ctDNA). This prospective observational study sought to ascertain the feasibility of ctDNA detection in patients with hepatoblastoma and explore its associations with established clinical indicators and biomarkers, including serum Alpha-fetoprotein (AFP). We obtained 38 plasma samples and 17 tumor samples from 20 patients with hepatoblastoma. These samples were collected at various stages: 10 at initial diagnosis, 17 during neoadjuvant ch

Precision Medicine for Kienböck Disease in the 21st Century.

One of the most popular treatment algorithms for Kienböck disease is based on a progression of successive radiographic changes that was developed in the 1970s. Since then, 2 other important classifications systems have been introduced. One is centered on contrasted magnetic resonance imaging findings and the other, articular cartilage degeneration. All 3 systems have been used, in isolation, to recommend specific treatment modalities. To coordinate all available clinical data and to simplify the treatment selection process, we combined the 3 classification systems into 1 unified classification

Monogenic diabetes: a gateway to precision medicine in diabetes.

Monogenic diabetes refers to diabetes mellitus (DM) caused by a mutation in a single gene and accounts for approximately 1%-5% of diabetes. Correct diagnosis is clinically critical for certain types of monogenic diabetes, since the appropriate treatment is determined by the etiology of the disease (e.g., oral sulfonylurea treatment of HNF1A/HNF4A-diabetes vs. insulin injections in type 1 diabetes). However, achieving a correct diagnosis requires genetic testing, and the overlapping of the clinical features of monogenic diabetes with those of type 1 and type 2 diabetes has frequently led to mis
