Psychotherapy in light of internal multiplicity.

The view that the self has multiple parts and that these appear in people seeking psychotherapy--and people conducting psychotherapy--is shared by clinicians of various orientations and supported by psychological research. It is useful for clinicians to think of patients as multifaceted and pay attention to the changes between facets that occur during therapy. They can thus help hidden parts to surface, facilitate dialogue between parts not in contact with each other, and convince excessively dominant or oppressive parts to make room for other adaptive facets. The authors contributing to this

Paradox in psychotherapy: a cybernetic perspective.

Recent uses of the term "paradox" in the field of psychotherapy have distracted attention from the concept's broader implications. All therapy can be seen as paradoxical because of the recursive nature of self-evaluation. Using agoraphobic problems as illustrative, and starting from the notion that the core issue for the agoraphobic is a demand for absolute control of his environment, we argue that therapy requires the bonding of two observers--the therapist and the "client-as-observer". Negotiations issuing from this bond are crucial to outcome, even when behavioral techniques, often consider

Gestalt therapy with the dying patient: integrative work using clay, poetry therapy, and creative media.

This is a report on death therapy with a 36-year-old cancer patient. Gestalt therapy and creative media (clay, poetry, and colors) were used to facilitate an integration of life and to give the person a sense of balance with life. The author tries to communicate his thoughts and feelings as they were during the course of the therapy, to show that counseling the dying means walking along a stretch of the path together. The companion-therapist cannot avoid his own perplexity and confusion by simply falling back on his professional role. Once he has become involved in an interpersonal relationshi

The management and mismanagement of a suicidal patient.

The author begins with the premise that the actions of therapists and hospital staff can contribute to the likelihood that a suicidal patient will eventually take his life. He recounts a case history of a depressed 51-year-old man who was hospitalized following a suicide attempt and delineates eight "mistakes" that contributed to the patient's eventual suicide and that are common occurrences in dealing with a suicidal patient. He believes that if more therapists and mental health professionals share mistakes and experiences of this kind with each other, such errors might be minimized or even p

Coping-based intervention strategies.

Coping is explained as a circular process involving three repetitive steps: (1) perception of a change in well-being; (2) cognitive processing of information, in assessing its impact and possible consequences; and (3) coping as such, including many different modes. In general, active, task-oriented behaviour in which patients tackle problems, remain optimistic and seek attention and care is considered favourable. A passive and withdrawn attitude with ruminations, self-accusation and resignation is known to be unfavourable. Based on problem-solving theory, a coping-based intervention strategy i

[Subjective construction of psychiatric hierarchy].

A survey was undertaken amongst student nurses at a psychological clinic with the aim of reconstructing the way in which the participants project values and expectations onto the established hierarchy of the institution. The method used for the survey was the Repertory-Grid-Technique; the grids were evaluated in a formal logic way. It can be seen a) how students especially favour the team as a niche at the workplace and how they seek to dissociate it from other role-bearers at the clinic, b) how patients are categorized at the very bottom of the hierarchy of values, c) how in some grids an int

Constructing an account by contrast in counselling for childhood genetic testing.

Genetic counselling sessions are rich and complex sites of accounting practices for decision-making in which clinicians are meant to facilitate rather than control the decisions made by their clients. This often means the adoption of a non-directive stance as counsellors lay out various possible courses of action from which the client can choose, while both client and counsellor may need to bear in mind a wide range of practical and ethical issues. With regard to childhood predictive testing, the complexity of decision-making is manifest not only in relation to the severity of the genetic cond

Tenderness in Our Work. Orpha and the Orphic Psychoanalyst.

Towards the end of his life, the psychoanalyst Sándor Ferenczi proposed the notion of Orpha, the protective psychic fragment that we can perceive in patients who have experienced massive traumas. This clinical paper explores this concept and the role of the psychoanalyst's orphic function in the transferential relationship, and tenderness, a notion little used in psychoanalysis, which can appear in this link, like a tool enabling the relationship between analyst and patient.

A qualitative meta-analysis on therapist responsiveness to dissociation: Introducing the dissociation-responsive therapy model.

Although it is common to encounter dissociative clients in the therapy room, little is known about how clinicians adapt therapy for these clients. The present study is a qualitative meta-analysis investigating how therapists are responsive to dissociation-that is, how they tailor therapy to meet dissociative clients' needs in and across sessions. We conducted a systematic search for qualitative studies on therapists' experiences working with clients who dissociate, yielding 16 eligible studies with at least 239 therapists represented across the studies. We used a critical-constructivist ground

Some Vicissitudes of playing related to containment and disruption.

In agreement with Winnicott, the author considers all forms of playing as a place of transit between inner and outer reality and between unsymbolized and symbolized experience. Some forms of playing serve more exclusively to clarify the current state of the patient's separate mind, while other forms of playing additionally introduce and mark forms of separateness between patient and analyst that are more likely to cause temporary disruption. This paper aims to distinguish between these two forms of play. Through clinical vignettes, the author demonstrates how each form of play offers various f

How do people with a history of sexual abuse experience talking therapies for sexual trauma? A systematic review of qualitative findings.

OBJECTIVES: This systematic review investigated the qualitative experiences of sexual abuse survivors in talking therapy treatments with respect to the helpful and hindering aspects of therapy. METHODS: Searches were conducted on five databases; PubMed, Web of Science, PyscInfo, PsycArticles and PsycExtra and through searching reference lists. Papers were included where they provided qualitative primary data of client experiences of therapy, did not involve specialist clinical populations and were written in English. Papers were included irrespective of whether study participants had experien

The Impact of Interpersonal Style and Interpersonal Complementarity on the Therapeutic Alliance Between Therapists and Offenders in Sex Offender Treatment.

Therapist and treatment process variables affect the effectiveness of offender rehabilitation programs. This study examined the influence of therapists' and offenders' interpersonal styles (IPSs) and interpersonal complementarity on therapeutic alliance (TA). Seventy-five sex offenders and their therapists evaluated each other's IPSs and the TA after 3 weeks of treatment. Offenders evaluated the TA more positively than therapists. Regarding the impact of IPS, therapist affiliation was positively correlated and therapist control was negatively correlated with offenders' ratings of the TA; in ot

Silicon in 'pure gold'? Theoretical contributions and observations on teleanalysis by videoconference.

Ideas about psychoanalysis via videoconference-videoconference teleanalysis (VT)-are presented with the general understanding that these settings produce a twofold split between various degrees of recognition/negativisation of the absence of the other, on one hand, and the expectation of physical co-presence, on the other. This split has been put forward as dismantling the here, now, with me pre-reflexive unity of the analytic experience. This article suggests that both members of the analytic dyad will seek to reappropriate the experience through a forced ego integration that interferes with

The good doctor: professionalism in the 21st century.

Medical professionalism faces distinctive challenges in the 21st century. In this chapter, we review the history of professionalism, address specific challenges physicians face today, and provide an overview of efforts to address these issues, including behavioral and virtue ethics approaches. First, we discuss core features professions share and the development of codes of medical ethics that guide the practice of western medicine. Second, we address challenges related to the doctor-patient relationship, continuity of care, cultural competence, conflicts of interest, and the regulation of qua

Epistemology and learning disabilities: invited commentary on Hastings and Remington.

The effect on service provision of describing a variety of actions as challenging behaviour is discussed: it is suggested that studying stereotyped, aggressive and self-injurious acts in their own right has yielded more useful psychological debate, and ignoring such conceptual thinking leads to implicit theorizing where assumptions go unquestioned. Evidence for the relevance of staff actions on different topographies of client responses is critically reviewed, alongside discussion of the authors' rhetoric. The importance of adjusting language and frameworks to make room for alternative concept

Counselors' attitudes toward homosexuality: some tentative findings.

The attitudes of 52 counselors and trainees toward homosexuality showed respondents were less likely to accept homosexual persons in eroticized contexts, more likely in noneroticized contexts, with mixed acceptance for homosexuals in sensitive professional positions. Respondents generally accepted homosexuals' civil rights and saw homosexuality as nonpathological and unrelated to morality per se. Social distance theory may explain these results.
