Prosthetic valve endocarditis caused by Histoplasma capsulatum: the first case report in Thailand.

The authors report a rare case of fungal endocarditis caused by Histoplasma capsulatum in an immunocompetent woman with mitral valve prosthesis. The patient presented with chronic fever and embolic phenomenon. Transthoracic and transesophageal echocardiography revealed a mobile mass attached to mitral prosthetic valve and her blood cultures were negative for both bacteria and fungi. The diagnosis was made by presence of budding yeasts in the histopathological findings of the vegetation and recovery of H. capsulatum from tissue culture of the excised vegetation. The patient was improved after a

[Listeria infection of a prosthetic hip joint].

A 74-year-old male with type 2 diabetes, coronary artery disease with history of CABG, hypertension and chronic obstructive pulmonary disease had total left hip replacement in another medical facility five years prior to admission. Since the early postoperative period, he had manifestations of dormant infection and was with empirical antibiotics. On the index admission, his X-ray showed signs of loosening of the prosthetic components with resorption of the bone. Periprosthetic infection was managed by a two-stage surgery. First, the infected prosthesis was removed and replaced with a spacer wi

Prophylactic antibiotics prior to bacteremia decrease endovascular graft infection in dogs.

Endovascular placement of vascular stent grafts in the aorta and peripheral vessels has become a prominent tool in the armamentaria of the vascular surgeon. Despite, several reports of stent graft infection, no current guidelines exist regarding the administration of antibiotics prior to episodes of potential bacterial seeding. We sought to clarify the role of prophylactic antibiotics in preventing stent graft infection after the parenteral administration of Staphylococcus aureus (S. aureus) at various intervals following device placement. A stent graft device was constructed from a 4 mm thin-

Valve replacement for infective endocarditis: an overview.

Infective endocarditis, both in the native and prosthetic valve, presents a tremendous challenge to the cardiologist and cardiovascular surgeon, as well as the infection specialist. The timing of surgery is critical but it would appear that aggressive surgical intervention is indicated when there is persistent sepsis, continuing congestive heart failure, signs of nonfatal emboli, or in association with certain organisms such as staphylococcus, pseudomonas, or fungal organisms. Cardiac catheterization would not appear to add greatly to the diagnosis except to document the presence of coronary a

Does PGE₁ vasodilator prevent orthopaedic implant-related infection in diabetes? Preliminary results in a mouse model.

BACKGROUND: Implant-related infections are characterized by bacterial colonization and biofilm formation on the prosthesis. Diabetes represents one of the risk factors that increase the chances of prosthetic infections because of related severe peripheral vascular disease. Vasodilatation can be a therapeutic option to overcome diabetic vascular damages and increase the local blood supply. In this study, the effect of a PGE₁ vasodilator on the incidence of surgical infections in diabetic mice was investigated. METHODOLOGY: A S. aureus implant-related infection was induced in femurs of diabetic

The role of the gastrocnemius flap in implant retention strategies for acute periprosthetic joint infection following total knee arthroplasty: a systematic review.

BACKGROUND: Acute periprosthetic joint infection (PJI) represents a major cause of early failure following total knee arthroplasty (TKA). In selected cases, implant retention strategies such as debridement, antibiotics, and implant retention (DAIR), including modified techniques such as debridement, antibiotic pearls, and retention of the implant (DAPRI), may be considered. However, the success of these approaches is strongly influenced by the condition of the surrounding soft tissues. In the presence of compromised or tenuous wound conditions, soft tissue reconstruction may play a critical ad

Diagnosis and Treatment Influence Hospital Costs of Revision Total Hip Arthroplasty: A Systematic Review and Meta-Analysis.

BACKGROUND: This systematic review and meta-analysis aimed to synthesize the hospital costs of revision total hip arthroplasty (rTHA) by periprosthetic joint infection, periprosthetic fracture (PPF), aseptic loosening, and recurrent dislocation diagnoses. In addition, as several surgical options are available within some diagnoses, this study aimed to synthesize the evidence on hospital costs for septic rTHA between debridement, antibiotics, and implant retention and one-stage and two-stage rTHA, and for PPF, the cost between rTHA and open reduction and internal fixation treatment. METHODS: A

Spacers in two-stage strategy for periprosthetic infection.

In two-stage revision of infected implants, the first stage involves removing the implant and implanting a joint spacer, and the second stage involves implanting a new prosthesis at least 6 weeks later. Spacers have two main functions: local administration of high-dose antibiotics, and preservation of the joint space by reducing soft tissue retraction and improving patient comfort until reimplantation. The present review aims to detail the necessary characteristics of antibiotics added to cement to achieve good joint diffusion, to describe the steps of two-stage revision, and to present the ty

Comparison of the success rate after debridement, antibiotics and implant retention (DAIR) for periprosthetic joint infection among patients with or without a sinus tract.

BACKGROUND: The relevance between the presence of a sinus tract and the failure risk after debridement, antibiotics and implant retention (DAIR) for periprosthetic joint infection (PJI) after hip or knee arthroplasty is still unclear. This study aimed to compare the success rate of DAIR between patients with or without a sinus tract and to explore the possible risk factors for failure after DAIR in patients with a sinus tract. METHODS: Consecutive DAIR cases for PJI after hip or knee arthroplasty between January 2009 and June 2019 with a minimum 1-year follow-up in two tertiary joint arthropl

Usefulness of histology for predicting infection at the time of hip and knee revision in patients with rheumatoid arthritis.

INTRODUCTION: It remains unclear whether rheumatoid arthritis might be a cause of false positive of the histology for the diagnosis of prosthetic joint infection. Our aim was to evaluate the usefulness of the histology for the diagnosis of infection during hip and knee prosthesis revision in patients with rheumatoid arthritis. MATERIALS AND METHODS: All patients with the diagnosis of rheumatoid arthritis (RA) undergoing hip or knee revision surgery (total or partial) were retrospectively reviewed. Positive histology was considered when ≥ 5 neutrophils per high-power field (400×) were found in

Is surface modification effective to prevent periprosthetic joint infection? A systematic review of preclinical and clinical studies.

BACKGROUND: With increasing recognition of the importance of biofilm formation in the pathogenesis of periprosthetic joint infection (PJI), a push towards finding solutions to prevent PJI via surface modification of prostheses is occurring. Unlike the promising in vitro antimicrobial effects of these surface modifications, the preclinical and clinical prophylactic effects vary and are debated. Therefore, we performed this systematic review to answer: (1) what kinds of methods of surface modification are used in preclinical and clinical studies to prevent PJI, (2) whether these modifications ar

[Implant-associated infections - Diagnostics].

The diagnosis of implant-associated infections is challenging as chronic low-grade infections often only manifest as subtle clinical symptoms. Clinical evaluation, patient history, imaging, histopathological and microbiological examinations build the cornerstones of the diagnostics for implant-associated infections. New onset of pain at rest, local symptoms at the surgical site and early loosening of the prosthesis or pseudarthrosis should raise suspicion for an infection and prompt further evaluation. Percutaneous sinus tracts, purulent wound secretions and skin erosions with exposure of the

Potential use of erythromycin to prevent and treat prosthetic failure.

Prolonged prosthetic life span is a goal that orthopedic surgeons are pursuing. Both septic and aseptic loosening may lead to prosthetic failure. For decades, antibiotics have been applied in arthroplasty to prevent septic loosening, with excellent clinical results, although few therapies have been able to prevent aseptic loosening. Research on aseptic loosening indicates that osteoclasts play an essential role in this process. Thus, the exploration of osteoclastogenesis inhibitor has become of interest. Erythromycin is a broad-spectrum antimicrobial that has been used to prevent postoperative

Listeria monocytogenes infection in prosthetic joints.

Listeria monocytogenes is increasingly recognised as a cause of prosthetic joint infection. These infections tend to be more insidious and indolent in contrast to similar infections with Staphylococcus aureus. They can occur in patients who are immunocompromised due to malignancy or other illness or in nonimmunocompromised elderly patients. Listeria infections should not be treated with cephalosporins and prolonged antimicrobial treatment is generally advised for prosthetic joint infections. We noted a high affinity of L. monocytogenes for foreign bodies. Three of twenty-seven cases 11%) of li

A Clinical Review of Prosthetic Joint Infections.

Prosthetic arthroplasties are continuing to increase in the United States. While prosthetic joint infections are a less common adverse event, they can have a devastating impact on the patient as well as have a large economic burden on the health care industry. Prosthetic joint infections can be surgically managed by debridement, antibiotics, and implant retention, 1-stage revisions, 2-stage revisions, resection arthroplasties, arthrodesis, and amputations. Antimicrobial therapy typically includes 4 to 6 weeks of parenteral or highly bioavailable oral agents. Suppressive and chronic oral antimi

Biofilm and penile prosthesis infections in the era of coated implants: 2021 update.

William Costerton, the pioneer of bacterial biofilm research and Wilson published a review of this subject in 2012. Recent events and false claims have prompted an update for urologists regarding the science of penile implant biofilm. The recent biofilm literature has been investigated and new conclusions regarding penile implant biofilm physiology are clarified in this review. The timeline of biofilm formation is as follows. The wound is contaminated upon incision, and the inoculum of bacteria ceases with incision closure. Almost immediately planktonic bacteria attach to the implant and secre

Repeat two-stage revision for knee prosthetic joint infection results in very high failure rates.

INTRODUCTION: Two stage revision total knee arthroplasty (TKA) is the gold standard for treatment and eradication of knee prosthetic joint infections (PJI), but the literature is limited on the outcomes of repeat two-stage TKA after PJI recurrence. This study presents the outcomes of repeat two-stage revision TKA and investigates potential factors contributing to success or failure, aiming to assist counselling and decision-making. METHODS: A retrospective study was undertaken investigating all two-stage revision TKA performed at one institution between 2005 and 2020. Twenty cases underwent r
