Toxicity with proton therapy for oral and/or oropharyngeal cancers: A scoping review.

BACKGROUND: Oral and/or oropharyngeal cancers account for approximately 2% of all malignancies, with variation across age groups, genders, and geographic locations. Treatments for oral and/or oropharyngeal cancers usually consist of a combination of surgical excision most commonly followed by radiotherapy ± chemotherapy and/or immunotherapy/biotherapy depending on the nature of the malignancy. The significant morbidity caused by high-dose radiotherapy to the head and neck region is widely observed. Proton therapy is a promising treatment option that localises a proton beam to direct radiation …

Clinical implementation and validation of an automated adaptive workflow for proton therapy.

BACKGROUND AND PURPOSE: Treatment quality of proton therapy can be monitored by repeat-computed tomography scans (reCTs). However, manual re-delineation of target contours can be time-consuming. To improve the workflow, we implemented an automated reCT evaluation, and assessed if automatic target contour propagation would lead to the same clinical decision for plan adaptation as the manual workflow. MATERIALS AND METHODS: This study included 79 consecutive patients with a total of 250 reCTs which had been manually evaluated. To assess the feasibility of automated reCT evaluation, we propagate…

[Therapeutic success of proton pump inhibitors in the therapy of contact granulomas].

According to the current S2k guideline "Gastroesophageal Reflux Disease (GERD)" of 05/2014, an empirical proton pump inhibitor (PPI) therapy in double standard dose (e. g. Pantoprazole 40 mg 2 ×/day) is recommended for the extraesophageal GERD manifestation (e. g. with formation of a contact granuloma, CG) for 8 weeks. However, valid study data don't exist. In a prospective study from 05.2015 to 12.2019 39 patients consecutively randomized with endoscopically proven KG received PPIs in single (1 × PPI, n = 22) or double standard dose (2 × PPI, n = 17) for 8 weeks. Potential proton beam therapy for recurrent endometrial cancer in the vagina.

Proton beam radiotherapy mainly has been used in the gynecological field in patients with cervical cancer. The efficacy of proton beam therapy in patients with recurrent endometrial cancer has not yet been determined. A 77-year-old endometrial cancer patient presented with recurrence in the vagina without distant metastasis following hysterectomy. A hard mass measuring 6 cm originated from the apex of the vagina, surrounded the vaginal cavity, and infiltrated the proximal and distal vagina. The patient received proton beam radiotherapy using a less invasive particle treatment system while mini

The potential of proton beam radiation therapy in intracranial and ocular tumours.

A group of oncologists and hospital physicists have estimated the number of patients in Sweden suitable for proton beam therapy. The estimations have been based on current statistics of tumour incidence, number of patients potentially eligible for radiation treatment, scientific support from clinical trials and model dose planning studies and knowledge of the dose-response relations of different tumours and normal tissues. In intracranial benign and malignant tumours, it is estimated that between 130 and 180 patients each year are candidates for proton beam therapy. Of these, between 50 and 75

Assessment and therapy monitoring of Leigh disease by MRI and proton spectroscopy.

In a 2-year-old boy with Leigh disease, spasticity, dysarthria, and optic atrophy gradually developed. Computed tomography and magnetic resonance imaging disclosed progressive, symmetric basal ganglia lesions. In muscle tissue, a defect of pyruvate dehydrogenase complex was found. Magnetic resonance volume selective proton spectroscopy (MRVS) of the basal ganglia demonstrated an abnormal lactate peak. A ketonemic diet coincided with clinical stabilization and arrest of progressive brain lesions. Lactate could no longer be demonstrated by MRVS. It reappeared with a new brain lesion coinciding w

Bragg-peak proton-beam therapy for arteriovenous malformations of the brain.

Patients with arteriovenous malformations of the brain, who are subject to disabling or fatal recurrent hemorrhage, seizures, severe headache, and progressive neurologic deficits, may be considered unsuitable for conventional therapies (craniotomy with excision or embolization), usually because of the location, size, or operative risk of the lesion. We have treated such patients with stereotactic Bragg-peak proton-beam therapy and report the follow-up of 74 of the first 75, 2 to 16 years after treatment. Proton-beam therapy is intended to induce subendothelial deposition of collagen and hyalin

Technical Considerations on Proton Therapy in NRG's Multi-Institutional Clinical Trials.

PURPOSE: Integrating proton therapy into multi-institutional clinical trials introduces distinct physical and biological complexities. Modern pencil beam scanning enables highly conformal dose delivery but increases sensitivity to setup and range uncertainties, patient motion, interfractional anatomic changes, and variability in relative biological effectiveness. These factors complicate treatment standardization and outcome interpretation across institutions. METHODS AND MATERIALS: Under the leadership of NRG Oncology, current proton therapy clinical trial protocols were reviewed to identify…

Hypofractionated Proton Beam Therapy for Localized Residual or Recurrent Unresectable Hepatoblastoma.

BACKGROUND: We evaluated hypofractionated proton beam therapy (PBT) feasibility for unresectable hepatoblastoma (HBL). OBSERVATIONS: Six patients with residual/relapsed localized HBL received PBT (50 to 60 Gy (RBE) in 10 fractions) between April 2019 and September 2024, with follow-up through September 2025. After 55-month follow-up, 5 patients remained alive without disease. One marginal recurrence was successfully salvaged by re-irradiation, and another bridged to hepatectomy (10% viable tumor). Toxicities were minimal, with one grade 2 duodenitis. One patient died of treatment-unrelated my

Proton beam therapy for external auditory canal and middle ear cancers.

Few studies have evaluated the efficacy of proton beam therapy (PBT) for external auditory canal cancer (EACC) or middle ear cancer (MEC). Therefore, the present study aimed to evaluate the efficacy and toxicity of PBT for EACC and MEC. Between December 2009 and August 2018, 15 patients (seven males and eight females) underwent PBT for EACC or MEC. The median patient age was 64 years (range: 40-82 years). Ten patients had EACC, and five patients had MEC. PBT was administered using the passive scattering method. The median total dose of the BED10 was 88.8 Gy relative biological effectiveness (R

Increased cell killing effect in neutron capture enhanced proton beam therapy.

Thermal neutrons generated in the body during proton beam therapy (PBT) can be used to cause boron neutron capture reactions and have recently been proposed as neutron capture enhanced PBT (NCEPBT). However, the cell killing effect of NCEPBT remains underexplored. Here, we show an increase in the cell killing effect of NCEPBT. Using Monte Carlo simulations, we showed that neutrons generated by proton beam irradiation are uniformly spread on tissue culture plates. Human salivary gland tumor cell line (HSG), human osteosarcoma cell line (MG63), human tongue squamous cell carcinoma cell line (SAS

Severe skin ulcer caused by taking lenvatinib after proton beam therapy.

A 69-year-old man was treated with lenvatinib after three sessions of proton beam therapy (PBT) for hepatocellular carcinoma. Five months after administration of lenvatinib, a dermatitis with huge skin ulcer formed in the site of PBT irradiation. Lenvatinib was immediately withdrawn, but the skin ulcer continued growing until about 2 weeks later. With topical and antibiotic treatment, the skin ulcer resolved after about 4 months. After administration of lenvatinib, potential skin damage due to PBT at the irradiated site may have become apparent. This is the first report describing skin ulcer b
