[Chilean health system: health response and challenges in the face of COVID-19].

This study analyzes the Chilean health system response to COVID-19 from 2020 to 2023, highlighting moments (according to their epidemiological and sociopolitical context) and its main measures. It also reflects on the opportunities and limits of the response of this system to the epidemic to reform it. This case study included a literature review, document analysis, and 19 semi-structured interviews with relevant institutional stakeholders. Results indicate that, amidst political instability, national health capacities received investments. The adopted measures highlighted the relevance of the

Local governance effectiveness in crisis management: evidence from a first-class municipality in the Philippines.

INTRODUCTION: The COVID-19 pandemic placed unprecedented pressure on governance systems worldwide, underscoring the critical role of local governments in crisis management and public service delivery. In decentralized systems such as the Philippines, Local Government Units (LGUs) functioned as frontline institutions responsible for implementing public health measures, distributing emergency assistance, and maintaining essential services. This study examines the relationship between local governance mechanisms and service response quality during the COVID-19 pandemic in Sta. Cruz, Laguna. METH

The Importance of Being FAIR and FAST - The Clinical Epidemiology and Study Platform of the German Network University Medicine (NUKLEUS).

The COVID-19 pandemic has urged the need to set up, conduct and analyze high-quality epidemiological studies within a very short time-scale to provide timely evidence on influential factors on the pandemic, e.g. COVID-19 severity and disease course. The comprehensive research infrastructure developed to run the German National Pandemic Cohort Network within the Network University Medicine is now maintained within a generic clinical epidemiology and study platform NUKLEUS. It is operated and subsequently extended to allow efficient joint planning, execution and evaluation of clinical and clinic

Carbapenem-resistant Enterobacterales in Europe: strengthening AMR resilience through coordinated system-level action - lessons from Ukraine.

Carbapenem-resistant Enterobacterales (CRE) have emerged as one of the most serious antimicrobial resistance (AMR) threats in Europe. Rising incidence, substantial mortality, and marked heterogeneity across the EU/EEA pose a growing challenge. In Ukraine, an already substantial AMR burden and pre-existing limitations in diagnostic and infection prevention capacity, infectious diseases and antimicrobial stewardship (AMS) expertise, workforce resources, and access to effective antimicrobials have been further exacerbated by the war, including through damage to healthcare infrastructure and disru

Preparedness of the Ghana Health Service for field epidemiology and applied biostatistics: a systematic review of infectious disease surveillance, outbreak investigation methodologies, and statistical modeling capacities in resource-limited settings.

INTRODUCTION: Infectious disease outbreaks continue to threaten global health security, with resource-limited settings facing disproportionate challenges in surveillance, outbreak investigation, and applied biostatistical capacity. The WHO International Health Regulations 2005 framework defines preparedness through measurable core capacities, and seminal multi-country analyses of IHR State Party Self-Assessment Annual Reporting data from 182 and 186 countries consistently show that sub-Saharan African countries report the largest preparedness gaps globally. Ghana, despite hosting the first Fie

Readiness of health public facilities to diagnose, manage, and prevent the Ebola epidemic along border districts in Southwestern Uganda.

BACKGROUND: Ebola Virus Disease (EVD) outbreaks typically start from a single case of a probable zoonotic transmission and evolve into an epidemic because of the local and international movement of people, causing a global health security threat. This study aimed to assess the readiness of health facilities in the border districts of Southwestern Uganda towards prevention, diagnosis and management of EVD. METHODS: This was a cross-sectional study conducted between July and September 2024. The study was conducted in the districts that share a border with the Democratic Republic of Congo, which

When formal governance persists but resilience does not: health system governance and resilience in Yemen across a decade of conflict and crisis (2014-2025).

BACKGROUND: Yemen's health system has faced prolonged shocks, including war, epidemics, and COVID-19, challenging both governance and resilience. This study examines how formal governance structures and institutional capacities shaped the system's ability to anticipate, absorb, adapt, learn, and transform across a decade of crisis (2014-2025). METHODS: We conducted a longitudinal qualitative analysis based on interviews with health system leaders and technical actors. Using a resilience-capacity framework, we analyzed governance functions across four phases: pre-conflict period (pre-2014), co

Developing a framework for primary health care resilience in pandemic conditions for Iran.

INTRODUCTION: Primary Health Care (PHC) services are people's first point of contact with the health system. PHC helps to ensure health system resilience and health community, playing a vital role in preventing and detecting diseases early, especially during pandemics. By doing so, PHC helps reduce mortality rates significantly. This study aimed to explore the challenges affecting the resilience of PHC during pandemics and to develop a Framework for strengthening that resilience. METHODS: This qualitative research was conducted in three phases in 2024. In the first phase, semi-structured inte

Overcoming Barriers to One Health Surveillance Implementation.

The zoonotic origin of the COVID-19 pandemic renewed calls for establishing and improving One Health surveillance capabilities globally, yet a lack of universally recognized best practices and clear guidance on how to implement coordinated surveillance across multiple sectors has hindered progress. Without shared surveillance goals, multisectoral data sharing policies and standards, and interoperable tools to aid the establishment and implementation of coordinated surveillance using a One Health approach, many countries are left piecing together systems together without a way to measure succes

Strengthening the health systems at national level for malaria elimination in the Greater Mekong Subregion countries: a qualitative study.

BACKGROUND: Countries in the Greater Mekong Subregion (GMS) have committed to eliminating malaria by 2030. The success of a national malaria programme's transition from malaria control to elimination is dependent on the readiness of the health system to implement malaria elimination strategies. Understanding the readiness of health systems and what needs to be adapted is key to identifying barriers in achieving malaria elimination goals. This study aims to assess health system needs for malaria elimination, identify national-level barriers to effective implementation, and provide recommendatio

The Fleming Fund: Key Features of Design and Approach to Strengthening Antimicrobial Resistance Surveillance in Low- and Middle-Income Countries.

This paper examines the Fleming Fund as a major long-term investment in strengthening antimicrobial resistance (AMR) surveillance in low- and middle-income countries. Established by the UK Department of Health and Social Care in 2015, the Fund committed up to £498 million to support surveillance capacity across 25 countries in Africa and Asia through a One Health approach spanning human, animal and environmental health. The paper situates the Fleming Fund within wider global concern about AMR and highlights surveillance as essential for stewardship, policy and international coordination. It de
