Radiation colitis and proctitis.

Radiation therapy is commonly utilized as a major component in the treatment of pelvic malignancy. Unfortunately, secondary toxicity to the lower gastrointestinal tract can occur. This most commonly affects the rectum, although injuries to the colon and small intestine are not uncommon. The presentation can be acute or chronic, and different mechanisms are responsible for each. Symptomatology is quite variable but can result in significant compromise for the patient. Numerous preventive and treatment strategies have been applied to this disease process. This article presents a summary of the c…

Treatment of radiation proctitis.

Late complications of pelvic radiotherapy occur in 5-20% of patients, particularly chronic radiation proctitis (CRP). Rectal bleeding is the most common symptom. Other symptoms include difficulty in defaecation or tenesmus because of loss of distensibility of the rectum or rectal structuring. Treatment options of CRP include oral therapy (5-aminosalicylates, metronidazole), rectal instillation therapy (hydrocortisone, sucralfate, 5-aminosalicylates, formalin), thermal therapy (argon plasma coagulation, heater probe or laser) and hyperbaric oxygen. It is difficult to recommend evidence-based th…

Radiation effects on development.

It has been widely reported that prenatal exposure to ionizing radiation can interfere with embryonic and fetal development, depending on dose and gestational age in which exposure occurs. According to several studies on animal models, different well-defined stages during prenatal life can be distinguished in relation to teratogenic effects. During the preimplantation stage, elevated doses of radiation can result in abortion, while lower doses may produce genomic damage that is usually repaired. On the other hand, during the organogenesis stage in mice (embryonic day 6.5 [E6.5] to E13.5), irra…

Radiation-induced moyamoya syndrome.

PURPOSE: The moyamoya syndrome is an uncommon late complication after radiotherapy (RT). METHODS AND MATERIALS: A PubMed search of English-language articles, with radiation, radiotherapy, and moyamoya syndrome used as search key words, yielded 33 articles from 1967 to 2002. RESULTS: The series included 54 patients with a median age at initial RT of 3.8 years (range, 0.4 to 47). Age at RT was less than 5 years in 56.3%, 5 to 10 years in 22.9%, 11 to 20 years in 8.3%, 21 to 30 years in 6.3%, 31 to 40 years in 2.1%, and 41 to 50 years in 4.2%. Fourteen of 54 patients (25.9%) were diagnosed with…

[Occupational exposure to radiation].

Since the discovery of X-rays , overexposures and radiation injuries have been reported in both patients and persons exposed in the course of their professional duties. Each year, more than 2,500 million diagnostic radiological examinations, 32 million nuclear medicine examinations or treatment procedures and 5.5 million radiotherapy sessions are performed worldwide. Despite all precautions, avoidable incidents and accidents occur throughout the world every year , albeit with low frequency. Whereas diagnostic radiology is generally safe for patients and staff, interventional procedures (e.g. c…

Burn injuries from radiation.

Accidental exposure to radiation leading to injury and illness occurs notwithstanding safety devices and protocols used for protection. The medical management of radiation casualties is a major concern. Radiation effects are principally thermal, similar to electrical burn injuries, but with some unique systemic expression. The pathological effects of radiation to the skin are known; it is often difficult to assess the level of severity, quickly and with accuracy, because of the delay between exposure and the appearance of lesions and obscured lesions. The severity depends mainly on the nature …

Radiation-associated cardiovascular disease.

As the number of cancer survivors grows because of advances in therapy, it has become more important to understand the long-term complications of these treatments. This article presents the current knowledge of adverse cardiovascular effects of radiotherapy to the chest. Emphasis is on clinical presentations, recommendations for follow-up, and treatment of patients previously exposed to irradiation. Medline literature searches were performed, and abstracts related to this topic from oncology and cardiology meetings were reviewed. Potential adverse effects of mediastinal irradiation are numerou…

Radiation safety in fluoroscopy.

Fluoroscopy is an important diagnostic and interventional imaging tool that enables clinicians to view dynamic, real-time images of anatomy and function unmatched by other imaging techniques. However, operators must take into account many factors that impact the amount of radiation delivered, particularly over the course of a lengthy interventional procedure. After completing this review, readers will: Understand basic concepts of radiobiology and fluoroscopy systems. Know the typical doses and dose calculations for patients and fluoroscopic personnel. Recognize the pathology of biological eff…

Radiation exposure of aircrews.

Information is provided about the radiation to which aircrews are exposed and possible health consequences. Recommended radiation exposure limits are given. Crewmembers on commercial aircraft are exposed to higher doses of ionizing radiation than normally received by members of the general population in most parts of the world. The principal ionizing radiation is galactic cosmic radiation. On infrequent occasions, radiation from the sun leads to an increase in the ionizing radiation at aircraft flight altitudes. Radioactive cargo is another possible source of exposure to ionizing radiation. Cr…

Nitroxides as radiation protectors.

Selective protection of normal tissues from the damaging effects of ionizing radiation is an important objective in cancer treatment research. Likewise, radioprotective agents may be useful in protecting the human population in the event of radiation-related accidents or military conflicts. Over the past decade, we have identified stable nitroxide compounds as a unique class of antioxidants with demonstrated radioprotective properties. We have shown that nitroxides at nontoxic concentrations are effective as in vitro and in vivo antioxidants when oxidation is induced by superoxide, hydrogen pe…

Radiation, radicals, and images.

Nitroxide stable free radicals exhibit varied chemical and biological properties. Their biological applications have been greatly expanded over the past few years. Not only have they been shown to exhibit potent antioxidant and radioprotective properties, but also they can serve as in vivo functional imaging probes that non-invasively report on the oxygen status and redox properties of tissue, which may have utility in clinical biomedical research.

[Radiation-induced intracranial meningiomas].

Based on the analysis of 112 cases a clinical characterization has been done of intracranial meningiomas developed secondary to X-ray irradiation of the head for fungus disease of skull integuments. X-ray irradiation increases the risk of intracranial meningioma development up to 4-fold. Radiation-induced intracranial meningiomas are characterized by particular features of clinical manifestations, by preponderance of hyperdense forms, by being of large size, as well as by a high frequency of changes in the adjacent bone presenting as hyperostosis, usuration, and destruction. The above meningio…
