INTRODUCTION: The aim of this study was to investigate the clinical consequences of scapholunate dissociation associated with fractures of the distal radius and the impact on wrist function. Fractures of the distal radius and scapholunate dissociation overlap in pathomechanics. The diagnosis however is frequently missed initially.
PATIENTS AND METHODS: We reviewed 45 consecutive patients with closed distal radius fractures with a mean follow up of 48 months. Ten patients underwent surgery and 35 patients were treated by fracture reduction and cast immobilization during 4.5 (range 3 to 8) week…

The essential problematic of the distal fracture of the radius lies in the quality of its reduction and retention. 35 patients with distal fractures of the radius and strain fractures of the styloid process of the ulna were followed up with regard to their functional and radiological results and their subjective symptoms. Patients with intra-articular fractures were found to have significantly poorer functional results compared to those with extra-articular fractures. 29 out of 35 patients developed a nonunion of the styloid process of the ulna in the form of a compensatory false position with

Wrist arthroscopy is increasingly becoming recognized as an important adjunct in the management of displaced intra-articular fractures of the distal radius. Anatomic restoration of the articular surface is possible under bright, magnified conditions. Osteochondral loose bodies and associated intracarpal soft tissue injuries that may not be apparent on plain radiographs are detected and simultaneously managed. A method of management for displaced distal radius fractures using wrist arthroscopy and a combination of percutaneous and limited open reduction techniques are described.

Forty patients with articular fractures of the distal radius in which anatomic reduction of the joint surface could not be obtained by closed manipulation or by ligamentotaxis with external fixators had a combination of percutaneous and/or open reduction techniques to restore articular congruity. X-ray films taken after treatment with an average follow-up of 4 years showed satisfactory extraarticular alignment in 85% of the cases, and 37 (92.5%) patients demonstrated an articular step-off of 1 mm or less at late follow-up examination. Radiographic evidence of radiocarpal arthritis was present

Over a two year period 6 children with a proximal radius fracture were treated by open reduction and internal fixation with Polydioxanon-pins. We discuss the indications for operative treatment and the technique of internal fixation with 3 resorbable pins.

Simultaneous fractures of the distal radius and scaphoid are uncommon injuries for which the treatment is controversial. Nine patients with these fractures are presented. Five patients were available for long-term followup. In the series, there were five Colles and four Smith's fractures of the distal radius. All scaphoid fractures occurred at the anatomic waist. All injuries resulted from falls of considerable force producing hyperextension at the wrist. The ages of the patients ranged from 21 to 90 years (median, 34). All fractures were placed in a thumb spica cast with the wrist in neutral

Diaphyseal fractures involving the radius and ulna, so called "both-bone" or "double-bone" forearm fractures are common orthopedic injuries. These injuries can result in significant loss of function if inadequately treated. As the upper extremity serves to position the hand in space, loss of forearm motion and/or muscle imbalance resulting from a poorly treated fracture can be particularly debilitating. Preservation of the anatomic relationships of the proximal and distal radioulnar joints as well as the interosseous space is critical to preserving function. This article overviews the manageme

The orthopaedic surgeons cannot predict the functional results after a distal intra articular radius fracture. The intra-articular incongrui … h and better range of motion than the openly treated group. The treatment of intra articular distal radius fractures with arthroscopic assistance is thus the guaranteeing of the most anatomical reduction of articular surface. Operative treatment of inadequately or imperfectly treated fractures of the distal radius can improve wrist and hand function substantially, but rarely restores the limb to normal. Patients with malunion of the distal radius present either with poor radiographic alignment before complete healing of the fracture (nascent malunion) or with functional problems that may be related to inadequate alignment of a healed fracture (mature malunion). Corrective osteotomy is offered to patients who have sufficient malalignment that the surgeon thinks problems are inevitable or to patients in whom the func

During a prospective one-year study (1988) all fractures of the distal radius in people over 20 years old were registered in Bergen, Norway. Six hundred patients with 609 fractures were treated. The incidence was 38/10000 population, and 79% of the fractures occurred in women. The age-specific incidence in women increased rapidly after the menopause and reached a maximum between 60 and 69 years. The incidence for women over 60 years of age is the highest reported. Among women over 50 years the incidence of fractures caused by minor falls varied depending on the season. The mean number of fract

Authors describe in connection with two cases of their own the vulnerability of the elbow joint originating from the physiological valgus position of this joint, i.e. the effect of the so-called "valgus stress". In the available literature no description of bilateral synchronous fracture of the radial head was found. They think, because of the bilateral injury, the correct operative therapy with the aim of reaching whole function, the stable osteosynthesis promising good result even in the fracture of the radial neck with major dislocation allowing also an early functional treatment, important

Guidelines for the choice of conservative or operative treatment of fractures of the proximal radius are derived from a study comprising 21 children. Postoperative complications and long-term failures are specifically stressed. True intraarticular fractures of the radial head are rarely observed in children. Major problems arise in the treatment of comminuted fractures of the radial head associated with dislocation of the elbow, since these show the worst prognosis. Displaced marginal fractures below 3 mm are suited for early functional treatment using conservative procedures. 18 patients suff

A 15-year-old boy sustained an ipsilateral scaphoid and radius fracture, a combination of injuries that has only received brief mention in previous reports. Closed reduction and cast immobilization produced a satisfactory result. The distal radius injury was a comminuted fracture with a vertical component that extended to the physis without involvement of the epiphysis. The mechanism of injury may have been a severe impact delivered to the base of the hand with the wrist fixed in dorsiflexion, a force that would make a distal radial epiphyseal fracture unlikely.
