Breast reconstruction with SGAP and IGAP flaps.

BACKGROUND: Perforator flaps represent the latest in the evolution of soft-tissue flaps. They allow the transfer of the patient's own skin and fat in a reliable manner with minimal donor-site morbidity. The powerful perforator flap concept allows transfer of tissue from numerous, well-described donor sites to almost any distant site with suitable recipient vessels. Large-volume flaps can be reliably supported with perforators from areas such as the buttock and transferred microsurgically for breast reconstruction. INDICATIONS: The ideal tissue for breast reconstruction is fat with or without …

Lymphaticovenular anastomoses training model for multiple stages of lymphedema by using efferent lymphatic plexus of the mesenteric lymph node of rats.

INTRODUCTION: Lymphaticovenular anastomosis (LVA) has transformed lymphedema treatment and has become an important part of the surgical therapy. LVA requires supermicrosurgical skills and unique nontraumatic techniques as the lymphatic vessel diameter of varies with the progression of lymphedema from 0.3 to 0.8 mm. However, even though several supermicrosurgical vessel anastomosis training models have been reported, only few focus on LVA including both various sizes of lymphatic vessels and lymphatic dissection. We report the establishment of a novel in-vivo LVA training model using the rat ef

Temporary arteriovenous shunt prior to free myoosseous flap transfer.

The use of temporary arteriovenous shunt constructed in the lower extremity prior to a free microvascular transfer of an internal oblique/iliac crest flap, is described. The indications for the technique are presented. Conventional free flap transfer may require maturation of a suitable inflow source or outflow conduit, which is provided by a temporary shunt, subsequently used for recipient vessels. The two-stage procedure is applicable when local vessels are inadequate for immediate transfer.

[Vascular anatomy of donor and recipient in living kidney transplantation].

OBJECTIVE: To review the vascular anatomy of the donor and the recipient for the living kidney transplantation. METHODS: The recent literature about the vessels of donor and recipient in clinical applications was extensively reviewed. RESULTS: The pertinent vascular anatomy of the donor and recipient was essential for the screening of the proper candidates, surgical planning and long-term outcome. Early branching and accessory renal artery of the donor were particularly important to deciding the side of nephrectomy, surgical technique and anastomosing pattern, and their injuries were the mos

Colour oscillations in arterioarterial anastomoses reflect natural differences in donor and recipient oxygenation and hematocrit.

Our aim was to show that the colour difference between brighter and darker red, occasionally observed as an oscillating boundary in the recipient and donor parts of an arterioarterial anastomosis in severe twin-twin transfusion syndrome (TTTS), is a consequence of natural differences in blood oxygenation and hematocrit developing between donor and recipient twins. As method we defined a theoretical model of the placenta with dimensions from pathology examination. From literature we determined the optical absorption and scattering properties of all tissue components, and hematocrit and oxygen s

Debunking the "Vessel Depleted Neck"-Study of 52 Patients With More Than Three Reconstructions.

BACKGROUND: Recurrences, complications, or unfavorable results in head and neck cancer can be treated surgically, including major flap recon … (3.9%), with three vein grafts to the contralateral neck. In 35 out of 178 free flaps (19.7%), the recipient vessels were from the contralateral neck. A prior history of radiotherapy is associated with an increased use of contralateral recipient vessels (Odds ratio 4.3, p = 0.001). CONCLUSION: The concept of a "vessel depleted neck" remains ambiguous, as viable r …

Reversal of TTTS after fetoscopic laser photocoagulation for communicating vessels.

We report a case of reversal of twin-to-twin transfusion syndrome (Stage IV TTTS) at 24 weeks of gestation after fetoscopic laser photocoagulation (FLP) for communicating vessels. The pathology of reversal of TTTS found that the 'initial' donor developed polyhydramnios whereas the 'initial' recipient developed oligohydramnios. With careful follow-up, the hydrops fetalis of the 'initial' recipient improved at 31 weeks of gestation. Because of the onset of labour pain at 34 weeks of gestation, two live and healthy babies were delivered by emergency caesarean section.

The posterior approach to the thoracodorsal vessels for microsurgical free tissue transfer.

The latissimus dorsi (LD) muscle is a workhorse flap in reconstructive surgery (e.g., breast reconstruction, chest wall defects, and lower limb trauma). The thoracodorsal artery and its venae comitantes supply this muscle. Recipient vessel options for microsurgical free flap reconstruction of the posterior thorax and lower back are limited. The thoracodorsal vessels are an excellent option due to their reliable anatomy and ease of access. In circumstances when the patient is best positioned prone, the posterior approach to access the thoracodorsal vessels is advantageous. We describe the techn

Donor/Recipient Delta Age: A Possible Risk for Arterial Stenosis in Renal Transplantation.

Different arterial wall properties can significantly increase the risk of blood turbulent fluxes leading to complications such as atherosclerosis. Since the mechanical properties of arterial vessels are influenced by age, we investigated, in a retrospective study, the effects on renal artery stenosis of an age difference >15 years between donor and recipient in a cohort of 164 patients undergoing renal transplantation between 1981 and 1991. The age difference between donor and recipient was ≤15 years in 87 patients (53.0%) (Group A) and >15 years in 77 patients (47.0%) (Group B, p = ns). None

The Use of the "Flange Joint" Anastomotic Technique for Hepatic Artery Reconstruction in Liver Transplantation.

OBJECTIVES: A successful liver transplantation requires hepatic artery inflow reconstitution. In living donor liver transplantation, there are additional challenges in the form of a short and small-caliber donor vessel stump, exacerbating the challenges of microsurgery. Meanwhile, in deceased donor liver transplantation, vessel diameter mismatch remains a potential challenge since the liver graft is procured with a long segment of the celiac axis with branches of differing calibers. Other challenges include the poor condition of the recipient vessels due to previous trans-arterial chemoemboliz…

Reconstruction of the lower extremity using free flaps.

BACKGROUND: The aim of lower-extremity reconstruction has focused on wound coverage and functional recovery. However, there are limitations in the use of a local flap in cases of extensive defects of the lower-extremities. Therefore, free flap is a useful option in lower-extremity reconstruction. METHODS: We performed a retrospective review of 49 patients (52 cases) who underwent lower-extremity reconstruction at our institution during a 10-year period. In these patients, we evaluated causes and sites of defects, types of flaps, recipient vessels, types of anastomosis, survival rate, and comp…

Salvage Reconstruction With Recycled Flap Pedicles in Head-and-Neck Surgery: A Report of Two Cases.

Free-flap reconstruction of head-and-neck defects is often complicated by a vessel-depleted neck after prior surgery or radiotherapy. Conventional alternatives-such as using contralateral vessels, distant recipient vessels, or interpositional vein grafts-are technically demanding and associated with additional risks. We present two salvage reconstructions in which the vascular pedicle of a previously transferred free flap was reused as recipient vessels when standard options were unavailable. A 79-year-old man developed exposure of a titanium mandibular plate 6 years after mandibular resection…

Surgical Complexities in Kidney Transplant of a Redo Donor and Recipient: A Case Report.

Performing surgery on a previously operated kidney can be difficult and technically demanding. Like re-operative renal surgery, redo donor nephrectomy is also risky, because normal tissue planes are obscured by perinephric fibrosis and adhesions, and dissection of hilar vessels can result in serious vascular damage. Revisiting an aborted implantation site in a recipient is also very demanding, and presence of arterial calcification adds to the difficulty. We report a unique case involving a donor and recipient who had previously proceeded with transplant surgery, but the transplant had been ab

Free muscle transfer.

Eight cases of free muscle transfer with the use of microsurgical technique are reported, comprising five patients with Volkmann's contracture of the forearm, two with poliomyelitis, and one with an old brachial plexus injury. Operation was successful in the six patients with traumatic injuries, but proved unsuccessful in the two with poliomyelitis. Generally, the vessels and nerves of the recipient site were anastomosed with their counterparts in the grafted muscle. In one patient with an old brachial plexus paralysis in whom nerves in the recipient site could not be used, muscle graft an int
