Aural Acoustic Stapedius-Muscle Reflex Threshold Procedures to Test Human Infants and Adults.

Power-based procedures are described to measure acoustic stapedius-muscle reflex threshold and supra-threshold responses in human adult and infant ears at frequencies from 0.2 to 8 kHz. The stimulus set included five clicks in which four pulsed activators were placed between each pair of clicks, with each stimulus set separated from the next by 0.79 s to allow for reflex decay. Each click response was used to detect the presence of reflex effects across frequency that were elicited by a pulsed broadband-noise or tonal activator in the ipsilateral or contralateral test ear. Acoustic reflex shif

Prediction of hearing loss from acoustic-reflex thresholds in the older adult population.

The clinical applicability of prediction of hearing loss in 126 ears of 83 adults more than 44 years of age was investigated. The method of hearing-loss prediction was based on that proposed by Silman, Silverman, Showers, and Gelfand (J Speech Hear Res 1984;27:12-9). This method, which utilized the acoustic reflex thresholds for hearing-loss prediction, was modified to create a more clinically feasible procedure; 5 dB rather than 1 dB intensity increments and visual monitoring of needle deflection rather than the use of a graphic level recorder were used. This modified method yielded a high pr

Ipsilateral acoustic reflex stimulation in normal and sensorineural impaired ears: a preliminary report.

Ipsilateral acoustic reflex thresholds were measured at 1,000 Hz and 2,000 Hz in 38 subjects with normal hearing and in 107 subjects with a symmetrical sensorineural hearing loss. Acoustic reflex thresholds were approximately six dB better for ipsilateral vs. contralateral stimuli for both subject groups. High ipsilateral reflex thresholds were associated with larger static compliance values in the sensorineural hearing impaired subjects. Reversed reflexes (increasing compliance with muscle contraction) were observed for ipsilateral stimulation in five per cent (two) of the normal hearing subj

The Role of the Clinically Obtained Acoustic Reflex as a Research Tool for Subclinical Hearing Pathologies.

The acoustic reflex (AR) shows promise as an objective test for the presence of cochlear synaptopathy in rodents. The AR has also been shown to be reduced in humans with tinnitus compared to those without. The aim of the present study was twofold: (a) to determine if AR strength (quantified as both threshold and growth) varied with lifetime noise exposure, and thus provided an estimate of the degree of synaptopathy and (b) to identify which factors should be considered when using the AR as a quantitative measure rather than just present/absent responses. AR thresholds and growth functions were

Inhaled toluene can modulate the effects of anesthetics on the middle-ear acoustic reflex.

Toluene (Tol) is an organic solvent widely used in the industry. It is also abused as an inhaled solvent, and can have deleterious effects on hearing. Recently, it was demonstrated that Tol has both anticholinergic and antiglutamatergic effects, and that it also inhibits voltage-dependent Ca(2+) channels. This paper describes a study of the effects of inhaled Tol on rats anesthetized with isoflurane, pentobarbital, or a mixture of ketamine/xylazine. Hearing was tested using distortion product oto-acoustic emissions (DPOAEs) associated with a contralateral noise to evaluate contraction of the m

Observations on the acoustic reflex threshold in institutionalized retarded adults taking mellaril and/or thorazine.

The present study was designed to investigate whether mellaril and thorazine, when used alone and in combination with other medications by mentally retarded clients, either abolish acoustic reflexes or compromise estimates of hearing sensitivity based on acoustic reflex measures. Acoustic reflex thresholds were determined in 35 normal-hearing institutionalized mentally retarded subjects who were regular users of the medications under investigation. Five sensitivity prediction methods based on acoustic reflex thresholds were applied to the data. The results of the study showed that acoustic ref

[A contribution to the "acoustic-mechanical effect" of the ipsilateral registered stapedius-reflex (author's transl)].

The ipsilateral registered stapedius reflex is often reduced for intensity levels greater than 105 dB (SPL). Because of a reduced impedance, that is not influenced by the reflex arc, an "inverse" reflex curve can be registered. This phenomenon described by Lehnhardt et al. (1) as a "acoustic'mechanical effect" occurs without latency and interferes at levels higher than 105 dB (SPL) with the reflectory impedance change. This effect often dominates and causes a pronounced inverse ipsilateral reaction characterized by an impedance reduction. Frequently middle ears with a steep gradient of the tym

Effects of stimulus duration and stimulus off time on the auditory and acoustic reflex thresholds.

The effects of stimulus duration and stimulus off time on the thresholds of hearing and the acoustic reflex were investigated in 10 normal-hearing subjects and 10 subjects with sensorineural hearing loss. A 1000-Hz stimulus with on-off times of 500-500 msec and 30-30 msec was used to obtain hearing sensitivity and acoustic reflex thresholds via a tracking method. Auditory threshold was poorer for the 30-30 msec tone than the 500-500 msec stimulus in both groups. Using the different stimuli, no significant difference in acoustic reflex threshold was observed in either group. These results sugge

RotaRod and acoustic startle reflex performance of two potential mouse models for Meniere's disease.

Meniere's disease (MD) is a disorder of the inner ear characterized by chronic episodes of vertigo, tinnitus, increased aural pressure, and sensorineural hearing loss. Causes of MD are unknown, but endolymphatic hydrops is a hallmark. In addition, 5%-15% of MD cases have been identified as familial. Whole-genome sequencing studies of individuals with familial MD identified DTNA and FAM136A as candidate genes for autosomal dominant inheritance of MD. Although the exact roles of these genes in MD are unknown, FAM136A encodes a mitochondrial protein, and DTNA encodes a cytoskeletal protein involv

Prepulse inhibition of the acoustic startle reflex vs. auditory brainstem response for hearing assessment.

The high prevalence of noise-induced and age-related hearing loss in the general population has warranted the use of animal models to study the etiology of these pathologies. Quick and accurate auditory threshold determination is a prerequisite for experimental manipulations targeting hearing loss in animal models. The standard auditory brainstem response (ABR) measurement is fairly quick and translational across species, but is limited by the need for anesthesia and a lack of perceptual assessment. The goal of this study was to develop a new method of hearing assessment utilizing prepulse inh

Tympanometric and acoustic stapedius reflex measures in older adults: the Blue Mountains Hearing Study.

Tympanometric peak pressure, peak compensated static acoustic admittance (peak Ytm) and acoustic stapedius reflex (ASR) thresholds were obtained for a representative sample of 1565 older Australians who were participants in the Blue Mountains Hearing Study (BMHS). No significant age or gender effects were found for tympanometric peak pressure. Peak Ytm measures, however, decreased with age in the left ear only across all age groups and were consistently higher for men than for women. After allowing for hearing loss, the effect of age on ASR thresholds was inconsistent. An increase in ASR thres

Ipsilateral acoustic-reflex adaptation testing for detection of facial-nerve pathology: three case studies.

Abnormal acoustic-reflex adaptation monitored in the same ear for both contralaterally and ipsilaterally presented tonal activators is reported in three cases. One case had Bell's palsy, whereas the other two cases had no clinically observable evidence of seventh-nerve involvement. These cases show that the existence of abnormal acoustic-reflex adaptation in the absence of Bell's palsy does not necessarily implicate the presence of eighth-nerve pathology.

Real-ear calibration of ipsilateral acoustic reflex stimuli from five types of impedance meter.

The Hearing Level scale has been established in national and international standards for tones presented via supra-aural earphones. However, the standards do not encompass stimuli from the probes of ipsilateral impedance meters. This study transfers the calibration of standardised earphones to the probes of five types of impedance meter and therefore enables the intensities of their ipsilateral stimuli to be defined in terms of Hearing Levels. The method used for transfer of calibration is based on equality of acoustic reflex responses elicitied by the impedance meter probe and by a standardis

Effects of preloads on middle-ear transfer function and acoustic reflex in ossiculoplasty with PORP.

INTRODUCTION: Surgical outcomes in ossiculoplasty with partial ossicular replacement prostheses (PORPs) are greatly influenced by the amount of preload imposed on the PORP. In this study, the attenuation of the middle-ear transfer function (METF) was experimentally investigated for prosthesis-related preloads in different directions, with and without concurrent application of stapedial muscle tension. Different PORP designs were assessed to determine functional benefits of specific design features under preload conditions. METHODS: The experiments were performed on fresh-frozen human cadaveri

Lesions of the paraventricular thalamic nucleus attenuates prepulse inhibition of the acoustic startle reflex.

The paraventricular thalamic nucleus (PVT) is a midline nucleus with strong connections to cortical and subcortical brain regions such as the prefrontal cortex, amygdala, nucleus accumbens and hippocampus and receives strong projections from brain stem nuclei. Prepulse inhibition (PPI) is mediated and modulated by complex cortical and subcortical networks that are yet to be fully identified in detail. Here, we suggest that the PVT may be an important brain region for the modulation of PPI. In our study, the paraventricular thalamic nuclei of rats were electrolytically lesioned. Two weeks after

Acoustic reflexes are common but not pervasive: evidence using a diagnostic middle ear analyser.

OBJECTIVE: The objective of this study is to determine whether acoustic reflexes are pervasive (i.e. known with 95% confidence to be observed in at least 95% of people) by examining the frequency of occurrence using a friction-fit diagnostic middle ear analyser. DESIGN: Adult participants with very good hearing sensitivity underwent audiometric and middle ear testing. Acoustic reflexes were tested ipsilaterally and contralaterally in both ears across a range of elicitor frequencies. Reflex elicitors were 700 ms tones presented at maximum level of 100 dB HL. Two automated methods were used to

Acoustic stapedial reflexes in healthy neonates: normative data and test-retest reliability.

BACKGROUND: The acoustic stapedial reflex (ASR) test provides useful information about the function of the auditory system. While it is frequently used with adults and children in a clinical setting, its use with young infants is limited. Presently, there are few data for neonates and inadequate research into the test-retest reliability of the ASR test. PURPOSE: This study aimed to establish normative data and evaluate the test-retest reliability of the ASR test in healthy neonates. RESEARCH DESIGN: A cross-sectional experimental design was used to establish ASR normative data and assess the
