[Smith-Lemli-Opitz syndrome; a special defect in cholesterol metabolism].

In a male neonate dysmaturity, microcephalia, a high nasal bridge, a long philtrum, broad dental ridges, schisis of the palatum molle, retrognathia, a small penis with a chorda, a small scrotum, bilateral inguinal hernia and bilateral syndactyly of the second and third toes were observed. The presence of the Smith-Lemli-Opitz (SLO) syndrome was suspected. By gas chromatography a severely decreased plasma cholesterol level (0.27 mmol/l) was found and an increased plasma 7-dehydrocholesterol level (0.24 mmol/l). The SLO syndrome is caused by a block in the cholesterol biosynthesis due to the aut

Temporomandibular joint contracture in spastic quadriplegia: effect on oral-motor skills.

This study assessed the temporomandibular joint of 34 children with spastic quadriplegic cerebral palsy for contractures to determine whether joint abnormalities were related to oral-motor dysfunction. Subjects were found to have restrictions in mandibular range of motion, increased overjet and decreased overbite. Increased overjet corresponds to retrognathia and decreased overbite relates to fixed posterior rotation of the mandible. The severity of many of these abnormalities correlated with the severity of oral-motor dysfunction. Abnormalities in overbite were particularly significant. The a

A novel ASPH variant extends the phenotype of Shawaf-Traboulsi syndrome.

Shawaf-Traboulsi syndrome (or Traboulsi syndrome; MIM 601552) is an infrequently reported entity characterized by a typical face (long face, large nose, convex nasal ridge, underdeveloped malae, crowded teeth, retrognathia), skeletal signs (long and slender fingers, sometimes pectus deformation and hypermobile joints), and ectopia lentis with conjunctival blebs, shallow anterior chamber and iridocorneal adhesions. The entity is caused by homozygous variants in ASPH. Here, we report on a boy with the clinical diagnosis of Shawaf-Traboulsi syndrome, in whom exome sequencing allowed identificatio

Oro-dental features as useful diagnostic tool in Rubinstein-Taybi syndrome.

Rubinstein-Taybi syndrome (RTS; OMIM # 180849) is a well-known disorder characterized by mental and growth retardation, broad thumbs and great toes, and unusual facial characteristics. We studied oro-dental findings in a group of RTS patients: 12 from the UK, 2 from Greece, and 26 from France. All were examined by two investigators, using the Diagnosing Dental Defects Database record form to document these. Various oro-dental features were found: small mouth, retrognathia, micrognathia, highly arched and narrow palate, talon cusps, expressed crowding, screwdriver incisors, cross bites, and ena

Lambotte syndrome: microcephaly, holoprosencephaly, intrauterine growth retardation, facial anomalies, and early lethality--a new sublethal multiple congenital anomaly/mental retardation syndrome in four sibs.

We report on an Arabic sibship originating from Morocco in which four children manifest an undiagnosed sublethal multiple congenital anomaly/mental retardation (MCA/MR) syndrome of intrauterine growth retardation (IUGR), microcephaly, large soft pinnae, telecanthus or true hypertelorism with squint, flat face, unusual hooked nose, very narrow mouth, retrognathia, and extremely severe neurologic impairment. One child was stillborn. Three others died in a cachectic state during their second year. One child had a severe cerebral malformation compatible with semilobar holoprosencephaly. Other inco

Pierre Robin sequence coexisting with ectopic kidney, bilateral developmental hip dysplasia in a child - A rare case report.

Pierre Robin sequence is a rare congenital disorder with variable associations. A two-year-old female child was brought in with complaints of an abnormal gait and right lower limb shortening. A comprehensive clinical evaluation unveiled facial characteristics resembling those of Pierre Robin Sequence, including micrognathia, a cleft palate, and glossoptosis. The child also exhibited retrognathia, a low posterior hairline, bilateral developmental dysplasia of the hips, right foot syndactyly, and a left ectopic kidney as confirmed by ultrasonography. Radiological examination showed bilateral dev

Sleep disturbances in 22q11.2 deletion syndrome: a case with obstructive and central sleep apnea.

The 22q11.2 deletion syndrome is characterized by wide phenotypic variability, frequently involving characteristic craniofacial features, cardiac malformations, and learning difficulties. Skeletal anomalies are also common and include an obtuse angle of the cranial base, retrognathia, and cervical spine abnormalities. Despite these anomalies, sleep-disturbed breathing is not reported frequently in patients with 22q11.2 deletion syndrome. We describe a patient with an obstructive sleep disturbance that was successfully treated with a tonsillectomy followed by mandibular distraction osteogenesis

[A method for the surgical treatment of maxillary micro- and retrognathism].

The proposed method of surgical treatment of maxillary micro- and retrognathia radically differs from the known methods: osteotomy of the osseous base of the hard palate is carried out parallel to the vomer to reach the level of the first premolars by a median dissection of the mucosa and periosteum, and vertical osteotomy of the anterior portion of the vomer is carried out. Horizontal osteotomy is performed from the vestibule of the mouth. Subsequent osteotomy of the anterior and lateral surfaces of the upper jaws under mucosal periosteal "tunnels" and detachment of the maxillary tuberosities

A rare type of low birthweight dwarfism: the Dubowitz syndrome.

Two patients with the Dubowitz syndrome are presented. This presumably recessive inherited syndrome was first defined by Grosse et al. (1971). So far 11 patients with this syndrome have been described. Major clinical findings are intrauterine and postnatal growth retardation, considerable microcephaly, mild mental retardation, hyperactivity, hyperextensibility of joints, eczema and a characteristic appearance of the face due to marked epicanthic folds, blepharophimosis, broadening of the bridge and tip of the nose and retrognathia. Minor anomalies as clinodactylyl of the firth digits, cutaneou

Otorhinolaryngologic manifestations of Hartsfield syndrome: Case series and review of literature.

Diagnosis of Hartsfield syndrome includes recognition of three distinct clinical anomalies: holoprosencephaly, ectrodactyly, and bilateral cleft-lip and palate syndrome. A family including three male siblings all affected by Hartsfield syndrome presented to our institution for care. An autosomal dominant variant in Fibroblast Growth Factor Receptor 1 (FGFR1) was identified. This report focuses on otorhinolaryngologic manifestationsof Hartsfield syndrome, previously undescribed, including midline defects of holoprosencephaly, bilateral cleft-lip and palate, retrognathia, gastroesophageal reflux

Mild form of treacher collins syndrome imitating juvenile otosclerosis.

Treacher Collins syndrome (TCS) is an inherited developmental disorder. More than 40% of individuals with TCS have conductive hearing loss attributed to external and middle ear anomalies. Mild cases of TCS often pass undiagnosed at birth or early childhood. The disease may be manifested as conductive hearing loss in teenagers and may resemble juvenile otosclerosis. Patients could suffer from slight facial variabilities including retrognathia (as in our case) and others, which point out to a possible middle ear anomaly. Surgical corrections of middle ear anomalies including TCS generally lead t

Guadalajara camptodactyly type III: a new probably autosomal dominant syndrome.

A Mexican family is presented with the main clinical features of camptodactyly, a distinctive facial appearance because of ocular hypertelorism, telecanthus, symblepharon and spinal defects. Other clinical manifestations included: multiple nevi, simplified ears, retrognathia, congenital shortness of the sternocleidomastoid muscle, thin hands and feet, a small penis and mild mental retardation. Radiographic studies revealed spina bifida occulta at cervical and dorso-lumbar levels, increased bone trabeculae, cortical thickening and delayed bone age. To our knowledge, there have been three prior reports of patients found, with trypsin-Giemsa banding, to be monosomic for the terminal q segment of chromosome 4. Described herein is a fourth patient with this chromosome abnormality. Comparison of these four patients suggests a characteristic phenotype in the 4q- syndrome: cleft palate, satyr deformity of the pinnae, snub nose, retrognathia and micrognathia, hypertelorism, oropharyngeal hypothonia or upper airway obstruction, cardiac defect, clinodactyly of the fifth fingers with absence of a flexion crease, simian lines, displaced or clinodact

[High oblique sagittal split osteotomy].

INTRODUCTION: Bilateral sagittal split osteotomy has become the standard mandibular surgery for the treatment of dental and facial deformities, even for small ones. The morbidity must be as low as possible. TECHNICAL NOTE: We describe a technique with reduced split surfaces, with an oblique bone section rather than a split. The bone is cut complete, on the posterior edge of the ramus, following an oblique line from above the lingula downward and laterally towards the supra-angular region. DISCUSSION: This section preserves the inferior alveolar nerve. It greatly decreases the risk of neurolo

Characteristic dysmorphic features in congenital disorders of glycosylation type IIb.

Over 100 types of congenital disorders of glycosylation (CDG) have been reported and the number is rapidly increasing. However, each type is very rare and is problematic to diagnose. Mannosyl-oligosaccharide glucosidase (MOGS)-CDG (CDG type IIb) is an extremely rare CDG that has only been reported in three patients from two unrelated families. Using targeted exome sequencing, we identified another patient affected by this condition. This patient had increased serum trisialotransferrin levels. Importantly, a review of the features of all four patients revealed the recognizable clinical hallmark

Partial trisomy for the distal part of the long arm of chromosome 15--a new syndrome?

A malformed male neonate with partial trisomy 15q dist (q22----qter), because of malsegregation of a balanced translocation (7; 15) (p22; q22) in his mother, is described. Comparison of this patient with thirteen previously published cases of this trisomy reveals a pattern of common features including: peculiar craniofacial dysmorphism--facial asymmetry, antimongoloid slant, narrow or short palpebral fissures, prominent nose, long upper lip, micro or retrognathia, high arched palate, low set ears, malformed ears, protuberant occiput--, abnormal fingers and toes, short neck, mental and growth r

Airway management in Escobar syndrome: A formidable challenge.

Escobar syndrome is a rare autosomal recessive disorder characterized by flexion joint and digit contractures, skin webbing, cleft palate, deformity of spine and cervical spine fusion. Associated difficult airway is mainly due to micrognathia, retrognathia, webbing of neck and limitation of the mouth opening and neck extension. We report a case of a 1 year old child with Escobar syndrome posted for bilateral hamstrings to quadriceps transfer. The child had adequate mouth opening with no evidence of cervical spine fusion, yet we faced difficulty in intubation which was ultimately overcome by se

Biodegradable osteosynthesis in mandibular advancement: a pilot study.

In this pilot study four patients are presented who underwent bilateral sagittal split osteotomies of the mandibular ramus for mandibular advancement. Biodegradable osteosyntheses were used for internal fixation. All patients were from the same dentofacial category: mandibular retrognathia with a low to normal mandibular plane angle. In two patients cylindrical self-reinforced polyglycolide (PGA) rods were used for internal fixation of the osteotomy segments. Long-term evaluation showed a complete ossification at the sites of biodegradable rods. In two other patients, self-reinforced poly(L-la
