Rhinophyma, an excessive hyperplasia of the sebaceous glands of the nose, results in disfigurement, occasional nasal blockage and recurrent infection. This condition is considered to be the culmination of acne rosacea and can degenerate to a basal cell or squamous cell carcinoma. UNLABELLED: Rhinophyma is a deforming disorder of the nose and considered the most severe stage of rosacea. Since 1950, a large variety of surgical and non-surgical treatments have been adopted to treat it. From January 2014 to June 2016, we treated ten patients (average age: 58 years) affected by moderate (4) and severe (6) degrees of rhinophyma. A nasal skin decortication was performed with a loop-shaped electrotome (Valleylab E1560 15 x 12 mm) in order to excise the hypertrophic tissue and restore a pleasing appearance to the nose.

In this article, we report a 66-year-old male case of rhinophyma who had a persistent lesion on his nose for two-years. Despite steroid therapy, the lesion continued to grow. Histopathological and immunohistochemical findings were consistent with cutaneous angiosarcoma. Rhinophyma-like features should be considered as an unusual clinical manifestation of cutaneous angiosarcoma.

Rhinophyma may present with either of two distinct histopathologic appearances. The most common shows histopathologic features of rosacea. The second pattern shows telangiectasia, diffuse dermal fibrosis with abundant mucin, and a virtual absence of pilosebaceous structures. A case of the Leser-Trélat sign associated with an underlying malignant haemangiopericytoma is described. In addition, the patient had profound hypoglycaemia and a rhinophyma-like nasal swelling which rapidly resolved post-operatively. The tumour appeared to be secreting a factor(s) which cross-reacted with both insulin receptors to induce hypoglycaemia and epidermal growth factor receptors inducing a profusion of eruptive seborrhoeic warts. It aslo had marked sebotrophic activity. The association of the Leser-Trélat sign with malignant haemangiopericytoma has not previously been described.

Rhinophyma is the end stage of acne rosacea, which results in a large nose due to a proliferation of sebaceous glands and fibrous tissue. In some patients, this disorder is a cosmetic problem. Use of the CO2 laser in the defocused mode to vaporize skin in a bloodless fashion is an effective treatment of rhinophyma, as demonstrated in three patients described in this report.

We present our experience with the Shaw scalpel in the treatment of a patient with extensive rhinophyma. The heated blade of the scalpel provides rapid maintenance of hemostasis with minimal tissue damage and thus allows for rapid and accurate removal of tissue. After removing the bulk of tissue with the Shaw scalpel, the carbon dioxide laser and dermabrader are used to refine nasal contours.

Rhinophyma affects predominantly the Caucasian population and is rare in those with Fitzpatrick skin type IV-VI. Diagnosis is based on clinical evaluation.
