Management of the high-risk lupus pregnant patient.

Pregnancy in the lupus patient presents a unique clinical challenge. Pregnancy may interact with lupus nephritis and adds preeclampsia to the differential diagnosis of hypertension. Lupus may result in pregnancy loss, fetal growth restriction, and prematurity. The obstetric management of these complex issues is presented for the nonobstetrician.

Familial breast cancer: management of 'lower risk' referrals.

Up to 40% of referrals from primary care to 'breast cancer family clinics' prove to be of women whose assessed risk falls below the guidelines' threshold for management in secondary or tertiary care, despite recommendations that they should be screened out at primary care level. A randomised trial, involving 87 such women referred to the Tayside Familial Breast Cancer Service compared two ways of communicating risk information, letter or personal interview. Both were found to be acceptable to referred women and to their family doctors, although the former expressed a slight preference for inte

Improving environmental risk management through historical impact assessments.

A historical contaminant impact assessment was conducted at 48 heavy manufacturing facilities located in 20 different states for a U.S.-based company. The facilities evaluated were industrial manufacturing sites that operated for as long as 100 yr and used several types of hazardous substances, including solvents and degreasers, oils and other petroleum products, paints and pigments, and heavy metals. The purpose of conducting the impact assessment was to provide direction and guidance on future environmental objectives and pollution prevention initiatives. The impact assessment involved exami

The costs of future polio risk management policies.

Decisionmakers need information about the anticipated future costs of maintaining polio eradication as a function of the policy options under consideration. Given the large portfolio of options, we reviewed and synthesized the existing cost data relevant to current policies to provide context for future policies. We model the expected future costs of different strategies for continued vaccination, surveillance, and other costs that require significant potential resource commitments. We estimate the costs of different potential policy portfolios for low-, middle-, and high-income countries to d

Acute coronary syndromes: risk stratification and initial management.

For patients who have acute coronary syndromes (ACS), risk stratification is key to initiating appropriate treatment. For ST-segment elevation MI, immediate reperfusion therapy is needed, and thus rapid identification of ST elevation on the ECG is critical. Then, having a standardized protocol for rapid treatment- with either primary percutaneous coronary intervention or thrombolysis - is critical. For unstable angina/non-ST elevation ACS, after first identifying the patients who have a higher likelihood of actually having an ACS (as opposed to noncardiac chest pain) stratification to high ver

Shoulder dystocia: etiology, common risk factors, and management.

Shoulder dystocia and brachial plexus injury occur in 0.5% to 1.5% of all births. Risk factors for both include maternal obesity, excessive prenatal weight gain, maternal diabetes, protracted labor, and fetal macrosomia. These factors are involved in only about 50% of births complicated by shoulder dystocia or brachial plexus injury. Shoulder dystocia has a low recurrence rate (9.8%-16.7%), although history of previous shoulder dystocia is the most reliable predictor of occurrence. Brachial plexus injury is the most common morbidity associated with shoulder dystocia, but 50% of newborns who pr

A dental risk management protocol for electroconvulsive therapy.

This article describes the results of an evaluation of a system to use trained junior psychiatric doctors to perform the pre-ECT oral assessment. All junior doctors were given a 1-hour training session in making oral assessments, and their ability to diagnose was tested by dental follow-up visits to patients they had assessed. Seventy-one patients were seen by both doctor and dentist, and the sensitivity for doctors ranged between 92% for the presence of dentures or removable bridges to 8% for the presence of teeth vulnerable to fracture. Where the doctors made a positive diagnosis, these were

Management of the high-risk pediatric burn patient.

BACKGROUND/PURPOSE: Inhalation injury, flame burn exceeding 30%, and age under 48 months all have been cited as independent risk factors for mortality; the combination of all 3 risk factors is unusual. The authors have experienced an overall reduction in mortality rate and chose to examine this high-risk group to define techniques useful in improving outcome in pediatric burns. METHODS: A review was done of children with all 3 risk factors over a recent 9-year interval. All were treated with a system of care emphasizing precise fluid repletion, early wound excision and closure, and avoidance

High-risk metastatic gestational trophoblastic tumors. Current management.

Aggressive multimodality therapy with an appropriate combination of chemotherapy and adjuvant radiotherapy and surgery has resulted in a cure for most patients with high-risk, metastatic gestational trophoblastic tumors. The EMA-CO chemotherapy regimen, employing etoposide, high-dose methotrexate, actinomycin D, cyclophosphamide and vincristine, is highly effective and well tolerated. Complete response rates of 80-94% and survival rates of 82-100% have been reported. For patients with central nervous system metastases, whole brain irradiation is given simultaneously with the initiation of comb

Hypertension and coronary heart disease risk factor management.

The primary goal in the treatment of essential hypertension is to reduce all end organ damage, not simply to reduce blood pressure. Hypertension is associated with an increased risk of cerebrovascular, cardiovascular and renal morbidity and mortality. Pharmacological therapy has reduced some, but not all, of these complications. In order to achieve maximal decrease in morbidity and mortality in hypertensive related diseases the overall impact of antihypertensive drug therapy on the pathogenesis of damage to each end organ must be considered. The pharmacological therapy of mild hypertension has

Cardiovascular Risk in Psoriatic Arthritis: Mechanisms, Risk Assessment, and Long-Term Management Implications.

Beyond its typical synovio-entheseal manifestations, psoriatic arthritis (PsA) is a systemic immune-mediated disease that carries a substantial, independent risk of major adverse cardiovascular events (MACE). The complex interaction of chronic systemic inflammation, a high prevalence of traditional risk factors, and PsA treatment drugs results in this increased cardiovascular burden, which is commonly underestimated in cardiology practice. Adipokine balance, insulin signalling, and lipid metabolism are all impacted by cytokine-driven systemic inflammation, which promotes metabolic abnormalitie

Applications of Capstone depleted uranium aerosol risk data to military combat risk management.

Risks to personnel engaged in military operations include not only the threat of enemy firepower but also risks from exposure to other hazards such as radiation. Combatant commanders of the U.S. Army carefully weigh risks of casualties before implementing battlefield actions using an established paradigm that takes these risks into consideration. As a result of the inclusion of depleted uranium (DU) anti-armor ammunition in the conventional (non-nuclear) weapons arsenal, the potential for exposure to DU aerosols and its associated chemical and radiological effects becomes an element of the com

Evaluation of Physical Risk during Necropsy and Morgue Activities as Risk Management Strategy.

Physical risk assessments allow us to understand work-related critical issues, thus representing a useful tool in risk management strategies. In particular, our study focuses on the identification of already known and emerging physical risks related to necropsy and morgue activities, as well as crime scene investigations. The aim of our study is, therefore, to identify objective elements in order to quantify exposure to such risk factors among healthcare professionals and working personnel. For the research of potentially at-risk activities, data from the Morgue of Policlinico Umberto I Hospit

Reducing cardiovascular risk in patients with familial hypercholesterolemia: Risk prediction and lipid management.

Familial hypercholesterolemia (FH) is a frequent genetic disorder characterized by elevated low-density lipoprotein (LDL)-cholesterol (LDL-C) levels and early onset of atherosclerotic cardiovascular disease. FH is caused by mutations in genes that regulate LDL catabolism, mainly the LDL receptor (LDLR), apolipoprotein B (APOB) and gain of function of proprotein convertase subtilisin kexin type 9 (PCSK9). However, the phenotype may be encountered in individuals not carrying the latter monogenic defects, in approximately 20% of these effects of polygenes predominate, and in many individuals no m

The role of assessing frailty in cardiometabolic risk management.

AIMS: Frailty is highly associated with morbidity and mortality in type 2 diabetes mellitus (T2DM). This viewpoint article reviews the importance of considering frailty in T2DM and cardiometabolic risk factor management and recommends its systematic integration into clinical practice. We propose that frailty is not merely a complication of metabolic disease, but an independent modifier of risk, treatment response, and outcomes. DATA SYNTHESIS: We conducted a literature review on the epidemiology of frailty in T2DM, bidirectional biological mechanisms linking frailty to T2DM and cardiometaboli

Dosage of lipoproteins for risk management according to guidelines.

LDL cholesterol (LDL-C) measurement is a key component in assessing cardiovascular disease risk and managing dyslipidemia. Despite irrefutable evidence that LDL-C-targeted strategies effectively reduce the risk of cardiovascular events, there is considerable variability among individuals in response to lipid-lowering therapies and in the resulting reduction of cardiovascular risk. Focusing solely on LDL-C assessment and management is no longer an optimal strategy for all patients. A major concern also lies in the potential for substantial errors in risk estimation, given the recognized measure

Candida in the ICU, Risk Management and Patient Safety.

Endogenous candidiasis remains an underrecognized yet clinically relevant complication in non-neutropenic critically ill patients. This study examines Candida spp. infections in the intensive care unit (ICU) within a patient-safety and risk-management framework, focusing on the identification of patients at highest risk and the development of an early diagnostic and therapeutic strategy. The target population comprises long-stay ICU patients requiring prolonged mechanical ventilation who develop multiple organ dysfunction syndrome (MODS) associated with immunoparalysis, typically reflected by
