Cytokine expression of IL-8 before and after root canal treatment in patients with symptomatic apical periodontitis: A clinical trial.

UNLABELLED: This study's objective was to compare the cytokine expression of IL-8 in periapical tissues of single-rooted teeth with symptomatic apical periodontitis (SAP) before and after root canal treatments. As well as, comparing IL-8 levels in peri-apical tissues between vital and necrotic teeth with SAP. METHODOLOGY: Thirty-six patients were allocated according to their pulp status into two experimental groups (n = 18) receiving the same treatment protocol; group 1: Vital pulps with SAP, and group 2: non-vital pulps with SAP. Conventional endodontic treatment was done on two visits; isol

Endodontic therapy in a mandibular second bicuspid with four canals.

Success in endodontic therapy often depends on the removal of the critical concentration of pulp tissue or necrotic debris within the root canal system. The best method to ensure this objective is to locate and instrument all of the canals in the root of the tooth. Therefore, a thorough knowledge of root canal morphology is essential to reduce the chances of failure. The purpose of this article is to report a case of mandibular second bicuspid with four distinct canals.

Impact of root canal preparation using two single-file systems on the intra-radicular microbiome of teeth with primary apical periodontitis.

OBJECTIVES: This study aimed to describe the effects of two single-file systems on the diversity of the endodontic microbiome of teeth with primary asymptomatic apical periodontitis. MATERIALS AND METHODS: The root canals from single-rooted teeth with apical periodontitis were prepared using either the Reciproc Blue (RB) or the XP-endo Shaper (XPS) instrument system. The latter was followed by a supplementary step with the XP-endo Finisher (XPF) instrument. For irrigation, 5.25% sodium hypochlorite was used. Root canal samples were taken at the baseline (S1), after preparation (S2), and after

Correlation between the clinically diagnosed inflammatory process and periapical index scores in severely painful endodontically involved teeth.

AIM: To assess and correlate three distinct states of severely painful endodontically derived inflammation with their depiction on periapical radiographs using periapical index (PAI) scores. METHODOLOGY: During a period of 15 months, 368 consecutively enrolled patients with suspected endodontic emergency conditions were examined at the University of Zurich, Center of Dental Medicine. Cases with a severely painful (numeric rating scale, NRS-11 > 6) endodontically involved tooth and a clear pulpal and apical diagnosis (n = 162) were selected (one tooth per patient). Teeth were divided into thre

[Clinical and radiographic rationale for endodontic treatment of permanent teeth with periapical destruction].

In the course of the present study four endodontic treatment schemes were tested and the most effective scheme was determined through the use of calcium hydroxide-based paste being applied three times at an interval of 7-14 days with the subsequent long-term temporary obturation using calcium hydroxide with iodoform, ultrasonic activation of sodium hypochlorite in the root canal, hydrodynamic irrigation and a diode laser. During the entire treatment period, the most effective scheme has revealed a reduction in defect size by 2,57±0,17 (p>0,0001) as well as a 1,84 degree (p>0,0001) decrease in

Treating apical fenestration in a previously endodontically treated tooth.

Apical fenestration is a defect in the alveolar cortical plate, exposing the root without involving the alveolar bone margin, often linked to trauma, periodontal disease, and orthodontic treatment, leading to symptoms such as pain and abscesses from endodontic infections. This case report describes managing a mucosal fenestration in an endodontically treated tooth with nonsurgical root canal therapy and periodontal surgery. A 44-year-old male presented with mucosal fenestration and pain in the upper front jaw due to trauma and an inadequately treated root canal. Examination revealed an ill-fit…

Revascularization Technique for the Treatment of External Inflammatory Root Resorption: A Report of 3 Cases.

The current external inflammatory root resorption treatment protocol, which uses calcium hydroxide dressing, usually comprises multiple and long-term applications. In addition to the need for multiple appointments for calcium hydroxide replacement, the long-term maintenance of this compound in the root canal weakens dental structures. A modification of this therapy would be advisable. In this clinical investigation, 3 patients with external inflammatory root resorption were submitted to revascularization therapy protocol usually used in teeth with necrotic pulp and open apices. The teeth were

Management of Separated Instruments Extruded into the Maxillary Sinus and Soft Tissue: a Case Series.

Extrusion of separated endodontic instruments is a frustrating complication that can occur during root canal treatment and is difficult to handle. This report aimed to introduce different methods to retrieve such separated instruments through three cases with different locations of fragments. Fragments extruded completely into the maxillary sinus, partially into the maxillary sinus and lying in the soft tissue were retrieved using a lateral window approach, ultrasonic method and minimally invasive surgery, respectively. These methods can be recommended for retrieving fragments in certain cases

Periapical bacterial disinfection is critical for dental pulp regenerative cell therapy in apical periodontitis in dogs.

BACKGROUND: Application of pulp regenerative cell therapy for mature teeth with periapical lesions is a critical clinical challenge. The bacterial infection in inaccessible location within the root canal system and in the periapical lesions could cause resistance and impediment, leading to limitations in successful therapy. Thus, the aim of this study was to examine the effect of residual bacteria on the outcome of pulp regeneration in mature teeth with apical periodontitis in dogs. METHODS: Periapical lesions were induced in 32 root canals of 4 dogs in two different models in severities, mod

Fe₃O₄-MXene as a dual-function root canal agent for disinfection and osteoclast inhibition in persistent apical periodontitis.

Persistent apical periodontitis (PAP) is characterized by periapical inflammation and alveolar bone resorption resulting from microorganisms that are resistant to conventional root canal disinfection. The treatment goals for PAP are to effectively eliminate resistant microorganisms and inhibit bone resorption. Root canal therapy (RCT) is the primary treatment for PAP, with chemical disinfection being a fundamental step in the process. This study developed Fe3O4‒MXene, a new root canal disinfectant with photothermally induced antibacterial properties and the potential to inhibit bone resorption…

Endodontic treatment of a two-rooted mandibular first premolar with four root canals: a case report.

BACKGROUND: Mandibular first premolar has a complex and variable anatomy of the root canal system, which often leads to failure of endodontic treatment due to missing root canals. Identifying the complete structure of the root canal system to ensure that all root canals are perfectly cleared and filled becomes critical to the success of root canal therapy. This report introduced a unique case of endodontic treatment of a two-rooted mandibular first premolar in the buccolingual direction with a total of four canals. CASE PRESENTATION: An adult male patient with a lower left first premolar was …

Three-dimensional inlay-guided endodontics applied in variant root canals: A case report and review of literature.

BACKGROUND: Root canal retreatment is common after root canal therapy in clinical situations. Especially, completing the retreatment of variant root canals can be challenging. This is particularly true for the molars located at the end of the dental arch. However, advancements in digital dental diagnosis and treatment techniques can solve these problems. Here, we describe a case of a maxillary second molar with a variant distobuccal root canal treated via a novel "inlay-guided endodontics" technique based on improved computer-generated programs. CASE SUMMARY: A 63-year-old man complained of a…

Histological examination of experimentally infected root canals after preparation by Er:YAG laser irradiation.

The influence of Er:YAG laser irradiation on periodontal tissues along the root surface and apical region during root canal preparation was histologically evaluated using experimentally infected root canals of rats. Eighty experimentally mesial infected root canals of mandibular first molars in rats were divided into four groups. In three groups, root canals were irradiated using an Er:YAG laser at 2 Hz with 34, 68, or 102 mJ/pulse for 30 s. Non-irradiated canals served as controls. The influence of laser irradiation on periodontal tissues along the root surface and apical area was evaluated h

The Preventive Effect of A Magnetic Nanoparticle-Modified Root Canal Sealer on Persistent Apical Periodontitis.

Persistent apical periodontitis is a critical challenge for endodontists. Developing root canal filling materials with continuous antibacterial effects and tightly sealed root canals are essential strategies to avoid the failure of root canal therapy and prevent persistent apical periodontitis. We modified the EndoREZ root canal sealer with the antibacterial material dimethylaminododecyl methacrylate (DMADDM) and magnetic nanoparticles (MNPs). The mechanical properties of the modified root canal sealer were tested. The biocompatibility of this sealer was verified in vitro and in vivo. Multispe…
