PURPOSE: Although many scaphoid fractures may be treated by immobilization, complex scaphoid fractures generally require bone grafting with internal fixation. Fractures result in increased metabolism of the bone and increased uptake of certain radioisotopes as demonstrated by bone scanning. By comparison, X-rays reveal a fracture only by a disturbance in skeletal structure. In this study, scaphoid fractures which were difficult to detect initially on X-rays were demonstrated by bone scan eight days later. The value of this diagnostic technique was demonstrated in several cases.

Author reviews possibilities in the treatment of nonunited scaphoid fractures. Different techniques with cancellous bone grafting are described and his own function-stable grafting osteosynthesis presented. Indications for his operation are well delineated and with regards to good results their accurate application is emphasized. 352 cases were treated using his grafting method in 94 per cent good results was obtained passing by 14 per cent the average results of 80 per cent with other grafting techniques.

Fractures of the scaphoid, particularly its proximal pole, are at risk of progressing to nonunion because of the tenuous, retrograde blood supply. A chronic nonunion of a proximal pole fracture of the scaphoid was treated by curettage of the nonunion, single K-wire fixation, and implantation of 50 mg of human bone morphogenetic protein followed by 12 weeks of cast immobilization without any conventional corticocancellous bone grafting or rigid screw fixation. Radiographs showed signs of bony healing by 12 weeks and a magnetic resonance imaging scan 6 years after surgery showed no signs of avascular necrosis. The potential future applications of human bone morphogenetic protein in hand surgery are discussed.

Magnetic resonance imaging (MRI) has become increasingly useful in the evaluation of musculoskeletal problems, including those of the wrist. In patients with a wrist injury, MRI is used mainly to assess vascularity of scaphoid non-union. However, the use of MRI in patients in the acute phase following carpal injury is not common. Three-phase bone scintigraphy is routinely performed from at least 72 h after injury in patients with suspected scaphoid fracture and negative initial radiographs.
