BACKGROUND: The prognosis of humeral sarcomas has improved greatly in the past 20 years. As a result, the challenges for orthopedic oncologists include long-term handicap and local morbidity.
METHODS: Shoulder reconstruction after the resection of sarcomas with the proximal humerus was evaluated for 55 patients from 1990 to 2007 at two universities. After S34(5)B resections (intra-articular resection of the proximal humerus and the abductor mechanism), the shoulders were reconstructed with arthrodesis, prosthesis, or allograft-prosthesis composites (APC).
RESULTS: At the time of the latest f

BACKGROUND: Advancements in surgical planning, technique, and prosthesis design have improved adaptation to patient anatomy in reverse total shoulder arthroplasty (rTSA). Postoperative changes in deltoid and rotator cuff muscle length are important and may vary based on preoperative indications and prosthesis selection. The purpose of this study is to demonstrate the changes in deltoid and rotator cuff muscle length for planned rTSA using the spectrum of prosthesis configurations in both glenohumeral arthritis (GHOA) and rotator cuff tear arthropathy (RCA).
METHODS: Ten shoulder arthroplasty

The authors report their experience of skin expansion of the upper limb based on a series of 42 operated patients: shoulder (8 cases), arm (19 cases), forearm (5 cases) and hand (8 cases). The operative protocol must be rigorous in terms of the choice of prosthesis (shape and size) and the type of valve (internal or external) and in terms of the technique of insertion of the expansion prosthesis (short incision in healthy skin) and harvesting of the expanded flap (sliding-draping flap rather than rotation flap). The indications are varied: unsightly scars and sequelae of burns, congenital naev

BACKGROUND: Bony increased-offset-reverse shoulder arthroplasty (BIO-RSA) has shown promising short- to medium-term results; however, long-term outcomes are lacking. The goal of the present study was to report on survival and long-term (minimum 10-year follow-up) outcomes of BIO-RSA.
METHODS: Retrospective single-center study with prospectively collected data, including 192 patients (196 shoulders) who underwent primary BIO-RSA for the treatment of osteoarthritis with cuff deficiency between 2006 and 2014. Mean age at surgery was 73 ± 7 years, and 70% were female. A structural bone graft, har

The results after reverse total shoulder arthroplasty for cuff tear arthropathy are superior and the complications fewer than for other etiologies, such as rheumatoid arthritis, fracture, fracture sequelae or even revision. The improvements in function and pain are excellent whereas rotation may be unaffected. Revisions and complications can be encountered in the first 3 years and the survival curve of the prosthesis is still good after 10 years with 90%. Progressive functional and radiological deterioration is observed after 9 years in approximately 30% of the patients without apparent proble

The purpose of this multicenter study was to analyze the results of shoulder arthroplasty for the treatment of the sequelae of proximal humerus fractures and establish an updated classification system and treatment guidelines for these complex situations. Seventy-one sequelae of proximal humerus fractures were treated with shoulder replacement with the use of the same nonconstrained, modular, and adaptable prosthesis: the Aequalis prosthesis (Tornier Inc, St Ismier, France). The average time between initial fracture and shoulder arthroplasty was 5 years and 5 months. On the basis of anatomic c

The treatment of cuff tear arthropathy (CTA) of the shoulder is based on the patient's subjective complaints, functional deficits, and the treatment goals that result from both. Only in the second line is the radiologic morphology of joint destruction important for the surgeon's decision. Based on these aspects it is possible to discriminate three types of CTA that obviously have similar functional deficits and radiological characteristics. This symptom-based classification can offer decision guidelines for the choice of an appropriate joint replacement. For the most frequent, stable, and osteo

BACKGROUND: Augmented baseplates can be effective at addressing eccentric glenoid wear in reverse total shoulder arthroplasty. However, these implants often come in a limited number of predetermined shapes that require additional reaming to ensure adequate glenoid seating. This typically involves complex instrumentation and can have a negative impact on implant stability. Modular baseplate augmentation based on intraoperative measurements may allow for more precise defect filling while preserving glenoid bone. The purpose of this investigation was to assess the stability of a novel ringed base

BACKGROUND: Implant breakage after shoulder arthroplasty is a rare complication after aseptic loosening, infection or persistent pain, resulting in malfunction of the components requiring revision surgery. This correlates with a high burden for the patient and increasing costs. Specific data of complication rates and implant breakage are available in detailed arthroplasty registries, but due to the rare occurrence and possibly underestimated value rarely described in published studies. The aim of this systematic review was to point out the frequency of implant breakage after shoulder arthropla

Glenoid implants used in anatomic total shoulder arthroplasties typically incorporate peripheral pegs as a design feature to support eccentric loads. These peripheral pegs and the implant-cement-bone interface undergo substantial cyclic tensile-compressive loads during normal activity. Therefore, these pegs are of interest in translating the micromechanics of local implant fixation failure to the biomechanics of gross anatomic failure of the glenoid implant after total shoulder arthroplasty. This study used an in vitro peg-cement-bone construct which incorporated bone tissue acquired from oste

BACKGROUND: Short-stem implants in shoulder arthroplasty were recently developed and reported clinical outcomes are good. However, radiological analysis often reveals humeral stem misalignment in the frontal plane, along with high filling ratios that can lead to proximal bone remodeling under stress shielding. The aim of this cadaveric study was to test whether using compactors for standard-length (> 100 mm) stems to implant short (< 100 mm) stems reduces the risk of stem misalignment without compromising in terms of a higher filling ratio.
METHODS: In a cadaveric study, twenty short stems we

BACKGROUND: The indications for hemiarthroplasty in glenohumeral joint diseases remain controversial and depend mainly on the original underlying diagnosis. Our objective was to investigate the influence of the primitive aetiology on long-term prosthesis survival and on the Constant-Murley score.
MATERIALS AND METHODS: We studied 272 shoulders with the following diagnoses: fracture sequelae (n=73), primary osteoarthritis (n=67), cuff tear arthropathy (n=43), avascular necrosis (n=40), rheumatoid arthritis (n=31), and other (n=18). Of the 272 shoulders, 139 were evaluated after at least 8 year

BACKGROUND: Loosening of the glenoid component continues to be the main cause of medium and long-term failure of shoulder replacements. The purpose of this study was to evaluate the early clinical and radiographic results following use of an all-polyethylene pegged glenoid component designed for hybrid (biological and cement) fixation.
METHODS: Eighty-three shoulders in 77 patients (mean age, 68.6 years) underwent total shoulder arthroplasty with a pegged hybrid-fixation component (bone-ingrowth glenoid and cemented peg). Outcomes were determined with the American Shoulder and Elbow Surgeons
