42 male minor and 56 male severe head injury victims, together with their relatives, were assessed in order to understand the nature of the impact of head injury on a close family member. Relatives experienced significant psychosocial difficulties which persisted throughout the year. A two stage model for understanding the causation of relatives' maladjustment is proposed. Rehabilitation programmes for relatives of severe head injury patients should look for vulnerability factors in the relatives, such as their own illness experience, as well as assessing current life stresses, such as the pat

The consultation-liaison psychiatrist is confronted time after time with depression in the context of physical illness, and the link between the two is of practical and theoretical interest. In addressing this topic, current classification systems are ambiguous. The advent of research diagnostic criteria and the primary/secondary depression dichotomy have not clarified the problem as was hoped. The conventional view that physical illness only precipitates depression in the genetically predisposed has been challenged by recent studies in patients with stroke. These studies point to a specific r

PURPOSE: This study seeks to evaluate the comparative role of alpha-fluoromethylhistidine and diphenhydramine in the prevention of motion sickness.
METHOD: The role of histaminergic mechanisms in motion sickness were evaluated in a feline model. Twenty-six female cats were studied. A variety of doses of fluoromethylhistidine and diphenhydramine were administered before motion testing.
RESULTS: Fluoromethylhistidine was effective in preventing motion sickness. The efficacy was dose dependent. In contrast, diphenhydramine failed to prevent motion sickness in any of the tested doses.
CONCLUSIO

AIM: To examine quality of life outcome for persons who self-report chemical sensitivity, often referred to as multiple chemical sensitivity.
BACKGROUND: Multiple chemical sensitivity is poorly understood with few providers specialising in its treatment. This lack of treatment and the ubiquity of chemicals engender severe life impacts such as job loss, financial loss, social isolation and even homelessness for persons who experience these sensitivities.
DESIGN: Survey.
METHOD: We examined chemical incitants, symptoms and sickness-related behavioural dysfunction as measured by the Sickness I

OBJECTIVE: Impaired insight (anosognosia) is frequently present in Alzheimer's disease. However, established instruments for diagnosing anosognosia are lacking.
METHOD: This article reviews the literature on impaired insight in Alzheimer's disease with a focus on assessment methods that have been applied to its study.
RESULTS: The primary methods for assessing anosognosia are clinical judgment, comparison of patient and caregiver reports, and comparison of patients' self-assessment and task performance.
CONCLUSIONS: While simple approaches such as expert rating scales can be appropriate for

In rheumatology there is the problem of different terminologies and classification systems; additionally there are only few studies on differential diagnosis and course of diseases in people over 65. The analysis of pain is important for diagnosis; people often have problems in exactly describing their pains. The character of pain changes in the course of disease; therefore a continuous pain analysis is necessary.

This is the first in a series of three articles addressing nonpharmacologic therapies for management of recurrent migraine and tension-type headache. It provides an overview of the commonly employed nonpharmacologic therapies for recurrent headache, reviews scientific evidence of their efficacy, and identifies the psychosocial interventions that have the greatest potential to enhance the management of recurrent migraine and tension-type headache in primary care settings internationally. In the second article, the authors offer practical guidelines for the administration of recommended nonpharm

Blood-injury phobia is a unique and peculiar phobia. It is different from other phobias in that it evokes a diphasic cardiovascular response; it has a propensity to induce fainting and nausea more than fear and anxiety; and often there is a family history of a similar phobia. Although it may cause no great difficulty or social handicap in everyday life compared to social phobia, agoraphobia or other simple phobias, it can have grave implications and may even become life-threatening when it prevents essential medical procedures. Unfortunately, not many of its sufferers come for treatment until

A review of the literature indicates that fear of future illness shares many characteristics with a wide variety of conditions. Concepts such as hypochondriasis, adjustment disorders, generalized anxiety, and mass hysteria must be considered. Also, factors such as reporting bias, influential others, collective behavior, political groups, and litigation appear to induce symptom reporting from patients complaining of fear of illness and cancerphobia. Cancerphobia may result in impairment ranging from avoidance behaviors to cognitive dysfunctions. Various treatment modalities have been used succe

42 out of 45 adults aged 18-26 years (93%) diagnosed in childhood as having coeliac disease (CD) returned a questionnaire reviewing how they coped with the diet. 27 (64%) still continued to take a strict gluten elimination diet, five had failed completely and ten partly keep it up. Those adhering to the diet more often demonstrated a good knowledge of both CD and the diet. Neither age at the time of diagnosis nor social class, education of the patients or present social status was observed to have any positive correlation with compliance with the diet. The results indicate a need for more prac

The term neurasthenia, which had been widely used in Japan before the Second World War, came to be replaced by the term neurosis thereafter. With this change in terminology, there seems to have been a shift in the popular ideas of minor psychiatric disorders towards a more psychological view. Unlike in the West where psychoanalysis was a major contributing factor, in Japan it was Shoma Morita who contributed to this change by questioning the somatic basis of conditions then diagnosed as neurasthenia and by developing the concept shinkeishitsu in the early 1920's, rejecting the concept of neura

Data from the 5-year Oldenburg Longitudinal Study were used for investigating the causes of differences in psychological well-being in a national sample of 1000 cardiac patients. In a subsample of 400 married patients, for whom data from the spouse were available, exogenous influences, cognitive mediators, and two measures of psychological distress were linked up in models explaining/predicting the level of anxiety and depression. A major hypothesis was the salience of the illness-related cognitions of the patient as a determinant of long-term behavior and mood state. Early patient subjective

We studied the role of somatosensory amplification, as measured by a self-report questionnaire, in symptomatology, overall discomfort, and disability in 115 patients with upper-respiratory-tract infections who visited an adult medical walk-in clinic. Multiple regression analyses indicated that amplification was a statistically significant predictor of the patients' localized but not systemic symptoms, of reported overall discomfort, and of their social and vocational disability. These relationships held true while controlling for differences in medical morbidity and sociodemographic characteri
