INTRODUCTION: Sickle cell trait, once thought to be a benign condition, has been associated with complications which may be of concern to the diver, such as splenic infarction and exercise-related sudden death. Medical clearance to dive for those with this common hemoglobinopathy has been a source of debate, with differing recommendations for both civilian and military candidates. To assess this disparity, a review was conducted of published findings and opinions regarding sickle cell trait and fitness to dive.
METHODS: Literature search was conducted using PubMed/Medline. Keywords included s…

A case report of a traumatic hyphema in a patient with sickle cell trait is presented. A review of the published literature in PubMed was performed and medical management strategies and surgical treatment indications for traumatic hyphema are discussed. We support the case for temporary trabeculectomy in patients with traumatic hyphema and sickle cell disease.

Although sickle cell trait protects against severe disease due to Plasmodium falciparum, it has not been clear whether sickle trait also protects against asymptomatic infection (parasitemia). To address this question, the authors identified 171 persistently smear-negative children and 450 asymptomatic persistently smear-positive children in Bancoumana, Mali (June 1996 to June 1998). They then followed both groups for 2 years using a cohort-based strategy. Among the 171 children with persistently negative smears, the median time for conversion to smear-positive was longer for children with sick…

Generally the sickle-cell trait is not regarded as a risk factor for morbidity or mortality and consequently does not entail occupational restrictions on physical activity (except for military pilots or divers). However many reports have described cases of visceral infarction, disseminated intravascular coagulation, and sudden death. High altitude, strenuous exertion, dehydration and fever may increase the risk. For getting more date, the author propose a medical follow-up program for athletes and soldiers with sickle-cell trait and the implementation of a national registry for sudden deaths i

Leukoreduction of cellular blood products by filtration has been shown to decrease the incidence of febrile nonhemolytic transfusion reactio … tions. This review concludes with a discussion of different approaches to screening blood units for sickle cell trait.

BACKGROUND: Patients with sickle cell trait and concomitant systemic disease are known to be at risk for proliferative retinopathy. However, there are reports of retinopathy in patients with sickle cell trait without systemic disease. There are no population-based studies addressing the risk of sickle cell retinopathy in this group. We performed a study to clarify the relation between sickle cell trait and retinopathy in healthy subjects.
METHODS: We reviewed the medical records of 100 children with sickle cell disease who attended the Sickle Cell Clinic at the Hospital for Sick Children, Tor…

The picture of the blood films of subjects with the sickle cell trait is still controversial. While some authors report the presence of sickled cells in the blood films of these subjects, others disagree. This paper submits that sickled cells could be seen in the blood films of subjects with the sickled cell trait, especially in those resident in high altitude areas.

A 56-year-old man with confirmed sickle-cell trait AS underwent successful triple-vessel myocardial revascularization with internal mammary artery and vein conduits. Variations in routine technique included limitation in hypothermia (32 degrees C), avoidance of intraoperative autologous salvage, hypoxia, severe anemia (< 25% hematocrit), acidosis and postoperative reinfusion of mediastinal drainage.

In brief Although sickle cell trait is generally benign and consistent with peak athleticism, it poses a small risk of gross hematuria and splenic infarction at altitude. More alarming is the growing evidence that, in some people with sickle cell trait, maximal exercise-especially in hot weather or when new at altitude-can evoke a life-threatening syndrome of sickling, fulminant rhabdomyolysis, lactic acidosis, collapse, acute renal failure, and hyperkalemia. More than 30 such cases are described, along with practical tips for recognition and prevention.

Dactylitis commonly occurs in patients with homozygous hemoglobin S disease (sickle cell anemia), sickle cell-hemoglobin C disease or sickle cell-beta-thalassemia. A case is reported of dactylitis associated with sickle cell trait, a very rare occurrence. It may be that in this patient the disorder was secondary to severe diarrhea and dehydration.

Changes in Department of Defense regulations now permit persons with sickle cell trait to serve in all service branches. However, for purposes of the regulation, sickle cell trait is defined as 41% or less S hemoglobin. Our screening experience, based on 397 individuals with sickle cell trait, with quantitative scan of cellulose acetate electrophoretic sheets, indicates that 20-40% (depending on definition of terms) of individuals with sickle cell trait would be excluded by this criterion.

A Caucasian with sickle cell trait (54% A, 43% S hemoglobin) had symptoms and signs of chronic hemolytic anemia as well as splenic infarction while living at very high altitude (in excess of 2,800 m). Trips from sea level back to high altitude repeatedly precipitated pulmonary infarctions. All symptoms and evidence of hemolysis resolved at low altitude.

Prospective and retrospective experience with physical complaints of 112 oral contraceptive and 67 intrauterine device Black family planning patients with sickle cell trait was evaluated by comparing their cumulative complaint rates with a randomly selected control group of non-sickle cell trait family planning patients having the same method, age and parity distribution as the sickle cell trait patients. Physical complaints considered are: dysmenorrhea, mental depression, vaginal discharge, fatigue, headaches, and nausea-vomiting. No evidence of a significant difference at level .05 is found …
