Successful replacement of the sound conducting mechanism of the ear has been hampered by infection, perforation, prosthesis instability and extrusion. In this animal study on thirty rats a silicone prosthesis was inserted between two layers of fascia as a total replacement of the tympanic membrane and ossicular chain. After four months the graft was found to be intact without evidence of infection or extrusion in all but one of the 30 rats. These results justify a trial of a similar silicone prosthesis in humans.

This report is concerned with tissue reaction to silicone and, in particular, long-term foreign body reaction. A histopathologic study was done on the larynx of a patient who had been injected 12 years before in the right vocal fold with room temperature vulcanizing silicone. The examination revealed particles of silicone encapsulated by thin fibrous tissue. There were few giant cells of the foreign body type and little cell infiltration. We report 20 years of experience with conservative and cautious injection of small amounts of medical-grade liquid silicone in 347 postrhinoplasty patients for a total of 1,937 treatments. Medical-grade silicone was found to be a safe, effective, and valuable adjunct to cosmetic rhinoplastic procedures. Microdroplet technique with very small doses is recommended for the correction of nasal defects that may not be amenable to revisional surgery. Controlled release of this material to the medical profession after appropriate evaluation is recommended, along with proper training prior to its use.

Intraocular silicone oil tamponade is being used with increasing frequency as an adjunct to conventional vitrectomy techniques in the treatment of proliferative vitreoretinopathy. Tesicon, one of the commercially available 'silicone rubber' lenses, was used in the correction of aphakic patients. In 74% of cases the lenses were considered successful for a daily wear regimen by the patient. Furthermore, a small number of patients could wear this lens without interruption for 3 to 6 days at a time. Despite this good acceptance by patients, corneal problems (mainly staining) and lens problems (dry surfaces) were frequently encountered.
