A Review of Environmental Barriers to Obtaining Adequate Sleep in the Military Operational Context.

INTRODUCTION: Sleep loss is ubiquitous in military settings, and it can be deleterious to cognitive, physiological, and operational functioning. This is especially true in the military operational context (e.g., training, garrison, combat) where continuous operations prevent adequate time for rest and recuperation. Furthermore, even when servicemembers do have opportunities for sleep, environmental disruptors in the military operational context make it difficult to obtain restorative sleep. Such environmental disruptors are potentially preventable or reversible, yet there is little public awar

Impact of Sleep and Associated Factors on the Prevalence of Early Childhood Caries: An Analytical Cross-Sectional Study.

BACKGROUND: Early childhood caries (ECC) occur soon after the eruption of teeth and can progress rapidly, causing severe impact on the physical, physiological, and psychological well-being of the child. Irregularities in sleep patterns can result in decreased salivary flow rate and the immune response, which ultimately can affect the rate of progression of early childhood caries. AIM: To assess the prevalence of early childhood caries and its association with sleep practices, the habit of frequent snacking, and the usage of phones and smart devices by children before bedtime in the age group

Sleep Health Issues for Children with FASD: Clinical Considerations.

This article describes the combined clinical experience of a multidisciplinary group of professionals on the sleep disturbances of children with fetal alcohol spectrum disorders (FASD) focusing on sleep hygiene interventions. Such practical and comprehensive information is not available in the literature. Severe, persistent sleep difficulties are frequently associated with this condition but few health professionals are familiar with both FASD and sleep disorders. The sleep promotion techniques used for typical children are less suitable for children with FASD who need individually designed in

Sleep problems.

Sleep complaints are common in the elderly, and are often related to physiological changes with ageing, or secondary to illness and drug therapy. Primary sleep disorders, such as sleep apnoea and movement disorders, are also common. Management should be specific to the problem. Attention to underlying factors and sleep hygiene, as well as reassurance and education about age-related sleep changes, will often be sufficient. Hypnotics, which usually offer only temporary symptomatic relief, may be counterproductive, and should be prescribed only in the short term, except in limited specific cases.

Health-related quality of life among veterans in addictions treatment: identifying behavioral targets for future intervention.

BACKGROUND: US veterans report lower health-related quality of life (HRQoL) relative to the general population. Identifying behavioral factors related to HRQoL that are malleable to change may inform interventions to improve well-being in this vulnerable group. PURPOSE: The current study sought to characterize HRQoL in a largely male sample of veterans in addictions treatment, both in relation to US norms and in association with five recommended health behavior practices: regularly exercising, managing stress, having good sleep hygiene, consuming fruits and vegetables, and being tobacco free.

The Effect of Behavioral Sleep Interventions on Measures of Obesity, Dietary Intake and Physical Activity in Adults With Poor Sleep Health: A Systematic Review, Meta-Analysis and Meta-Regression Analysis.

BACKGROUND: As poor sleep negatively impacts on adiposity, there is interest in examining whether improving sleep can improve adiposity markers. We reviewed studies aimed at improving sleep using (1) cognitive behavioral therapy for insomnia (CBT-I) and/or sleep hygiene and (2) sleep extension on adiposity markers, dietary intake, and physical activity. METHODS: Literature searches were performed on MEDLINE, Embase, CINAHL, and Cochrane Library. We included studies featuring adults ≥ 18 years without OSA, a sleep intervention as well as preintervention and postintervention measures. RESULTS:

Homelessness, Personal Hygiene, and MRSA Nasal Colonization among Persons Who Inject Drugs.

Methicillin-resistant Staphylococcus aureus (MRSA) infection is a leading cause of hospitalization and medical visits among individuals experiencing homelessness and also among persons who inject drugs (PWID), populations with significant overlap in urban centers in the USA. While injection drug use is a risk factor for MRSA skin infections, MRSA is also known to transmit easily in crowded, public locations in which individuals have reduced personal hygiene. Individuals in urban centers who experience homelessness or drug addiction may spend significant amounts of time in environments where MR

Simplified sleep restriction for insomnia in general practice: a randomised controlled trial.

BACKGROUND: Insomnia is common in primary care. Cognitive behavioural therapy for insomnia (CBT-I) is effective but requires more time than is available in the general practice consultation. Sleep restriction is one behavioural component of CBT-I. AIM: To assess whether simplified sleep restriction (SSR) can be effective in improving sleep in primary insomnia. DESIGN AND SETTING: Randomised controlled trial of patients in urban general practice settings in Auckland, New Zealand. METHOD: Adults with persistent primary insomnia and no mental health or significant comorbidity were eligible. In

Digitally delivered cognitive behavioral therapy for insomnia (CBT-I) for patients with chronic pain and insomnia (The Back2Sleep Trial): study protocol for a randomized controlled trial.

BACKGROUND: More than half of individuals with chronic pain also experience insomnia. Cognitive behavioral therapy for insomnia (CBT-I) is an effective and safe first-line treatment; however, access remains a major barrier to widespread implementation. This study aims to evaluate the effectiveness of a 9-week app-delivered CBT-I intervention, compared to an app-delivered sleep hygiene education program (active control), in reducing insomnia and pain severity in patients with disabling chronic pain and comorbid insomnia. METHODS: The trial is a 1:1 randomized, single-blind, superiority trial r

Case Report: Resolution of Insomnia in a 32-Year-Old Male with Generalized Anxiety Disorder Following Discontinuation of Mint-Flavored Toothpaste.

BACKGROUND: Insomnia is a common sleep disorder frequently comorbid with anxiety and is often attributed to psychophysiological hyperarousal. While internal and behavioral factors are typically emphasized, environmental sensory exposures are less commonly recognized as potential contributors to sleep disturbance. Menthol, a widely used compound in oral hygiene products, activates transient receptor potential melastatin 8 (TRPM8) receptors, which are known to influence acute sensory and arousal responses. However, their role in chronic sleep disturbance remains unclear. AIM: This case report a

Clinical features, diagnosis and treatment of narcolepsy.

Narcolepsy is characterized by excessive daytime sleepiness, cataplexy, sleep paralysis, and hypnagogic/hypnapompic hallucinations. It is currently believed to be caused by a deficiency in hypocretin-producing neurons in the lateral hypothalamus. Diagnosis is by the presence of appropriate clinical symptoms and confirmation by a polysomnogram followed by a multiple sleep latency test. There are nonpharmacologic (eg, scheduled naps, following proper sleep hygiene) and symptom-directed pharmacologic (eg, central nervous system stimulants, modafinil, sodium oxybate, certain antidepressants) treat

Sleep disorders in myotonic dystrophies.

Myotonic dystrophies (DM), the most common muscular dystrophies, are known to have significant sleep disturbances. We analyzed the literature on sleep and excessive daytime sleepiness (EDS) in DM over the past 30 years. In this review we provide a brief overview of sleep, sleep disorders, and methods of assessment. We also analyze data regarding major sleep disorders in DM patients, including: sleep-disordered breathing (SDB), with both central and obstructive sleep apneas (CSA,OSA); EDS; sleep-related movement disorders; and poor sleep quality. We review the possible pathogenesis of these dis

Insomnia among community dwelling elderly in Alexandria, Egypt.

BACKGROUND: Insomnia is a common problem in the elderly population. Poor sleep quality is associated with decreased memory and concentration, increased risk of falls, cognitive decline, and higher rate of mortality. Inadequate sleep hygiene such as irregular sleep schedules, use of stimulants, and daytime naps may predispose to insomnia. AIM: The aim of this study was to determine the prevalence of insomnia among community dwelling elderly in Alexandria and to assess some of the risk factors and comorbid conditions related to insomnia. PARTICIPANTS AND METHODS: This is a cross-sectional stud

Common Sleep Disorders Affecting Older Adults.

Sleep disorders in older adults increase with aging, likely due to increased sleep latency, decreased sleep efficiency, and total sleep time. Common sleep issues include chronic insomnia, circadian rhythm sleep-wake disorders, sleep-related movement disorders, and sleep-disordered breathing. Diagnostic tools, such as a comprehensive sleep history and questionnaires, or a sleep log for more specific complaints, are commonly used. Polysomnography is not recommended as a routine test; however, it can be used for abnormal behaviors during sleep or if treatment fails. Sleep disorder management is b

Dyssomnias, parasomnias, and sleep disorders associated with medical and psychiatric diseases.

Sleep disorders can be intrinsic, as are insomnia or narcolepsy, or can be accounted for by external factors, such as noise, altitude, drug or alcohol abuse, or shift work. The arousal disorders, common in children, are usually benign and disappear by puberty. Sleep-wake transition disorders such as sleep starts are benign as well, and may occur at any age. The parasomnias comprise different entities such as nightmares, REM-sleep behavior disorder, sleep enuresis, and bruxism. Diagnosis and treatment often require a multidisciplinary approach. Virtually every psychiatric, neurologic, or medica

Sleep problems in advanced disease .

Sleep problems are relatively common in patients with advanced disease, and are associated with significant morbidity in these groups of patients. The focus of this article is sleep problems in patients with advanced cancer, and specifically insomnia, 'vivid' dreams and nightmares. However, other sleep problems are also relatively common in this group of patients, including sleep-related breathing disorders and circadian rhythm sleep-wake disorders. Healthcare professionals should screen all patients with advanced diseases for sleep problems and, equally, initiate appropriate (evidence-based)
