Cryptic stage of sleeping-sickness trypanosome developing in choroid plexus epithelial cells.

Electronmicrographs of the choroid plexus from rats infected with Trypanosoma brucei rhodesiense showed that trypomastigotes from the perivascular spaces may penetrate and undergo multiple division in the ependymal cells which locally constitute the blood-brain barrier. Progressive degeneration of the ependymal cell liberates trypomastigotes back into the perivascular space, from which re-entry into the blood may occur. Re-entry to the blood does not take place from any tissues other than the brain and its membranes. These findings suggest that the ependymal cells of the choroid plexus are the

Evaluation of a Contactless Sleep Monitoring Device for Sleep Stage Detection at Home in a Healthy Population: Prospective Study in Free-Living Conditions.

BACKGROUND: Sleep is essential for overall health and well-being, but assessing sleep architecture is often costly and time-consuming, relying primarily on polysomnography (PSG). While wrist-worn wearables offer alternatives, they face limitations regarding user compliance, such as battery charging and physical discomfort. Nearable devices address these burdens, but they regularly lack rigorous validation, especially in real-world settings. OBJECTIVE: This study evaluates the accuracy and reliability of the Withings Sleep Analyzer (WSA), a contactless sleep monitoring device, compared to PSG

Unconstrained deep learning-based sleep stage classification using cardiorespiratory and body movement activities in adults with suspected sleep apnea.

This study assessed the feasibility of unconstrained deep-learning-based sleep stage classification using cardiorespiratory and body movement activities derived from piezoelectric sensors installed under a bed mattress. Heart and respiratory rates and their respective variabilities, cardiorespiratory coupling index, and body movement were simultaneously acquired through polysomnography (PSG) for 106 untreated participants with suspected sleep apnea. We used a bidirectional long short-term memory network to predict the five sleep stages using five different input feature combinations. The best

Inter-sleep stage variations in corrected QT interval differ between obstructive sleep apnea patients with and without stroke history.

Obstructive sleep apnea (OSA) is related to the progression of cardiovascular diseases (CVD); it is an independent risk factor for stroke and is also prevalent post-stroke. Furthermore, heart rate corrected QT (QTc) is an important predictor of the risk of arrhythmia and CVD. Thus, we aimed to investigate QTc interval variations in different sleep stages in OSA patients and whether nocturnal QTc intervals differ between OSA patients with and without stroke history. 18 OSA patients (apnea-hypopnea index (AHI)≥15) with previously diagnosed stroke and 18 OSA patients (AHI≥15) without stroke histo

Probable sleep bruxism in children in the mixed dentition stage: prevalence and association with the risk for obstructive sleep apnea-a cross-sectional study.

BACKGROUND AND AIM: Sleep bruxism's (SB) association with obstructive sleep apnea (OSA) is still not conclusively established. The present study aimed to assess the prevalence of probable sleep bruxism (PSB) in a sample of Lebanese children aged six to ten years, and evaluate the association between PSB and OSA risk. MATERIALS AND METHODS: Children recruited from schools were examined for PSB's intra-oral signs, occlusal and mucosal factors. Parents/guardians filled one survey addressing their child's anamnesis, socio-demographic and daily habits' information and another [Pediatric Sleep Ques

Clinical Validation of Artificial Intelligence Algorithms for the Diagnosis of Adult Obstructive Sleep Apnea and Sleep Staging From Oximetry and Photoplethysmography-SleepAI.

Home sleep apnea tests (HSATs) have emerged as alternatives to in-laboratory polysomnography (PSG), but Type IV HSATs often show limited diagnostic performance. This study clinically validates SleepAI, a novel remote digital health system that applies AI algorithms to raw oximetry data for automated sleep staging and obstructive sleep apnea (OSA) diagnosis. SleepAI algorithms were trained on over 10,000 PSG recordings. The system consists of a wearable oximeter connected via Bluetooth to a mobile app transmitting raw data to a cloud-based platform for AI-driven analysis. Clinical validation wa

Self-applied somnography: technical feasibility of electroencephalography and electro-oculography signal characteristics in sleep staging of suspected sleep-disordered adults.

Sleep recordings are increasingly being conducted in patients' homes where patients apply the sensors themselves according to instructions. However, certain sensor types such as cup electrodes used in conventional polysomnography are unfeasible for self-application. To overcome this, self-applied forehead montages with electroencephalography and electro-oculography sensors have been developed. We evaluated the technical feasibility of a self-applied electrode set from Nox Medical (Reykjavik, Iceland) through home sleep recordings of healthy and suspected sleep-disordered adults (n = 174) in th

Automatic Sleep-Stage Scoring in Healthy and Sleep Disorder Patients Using Optimal Wavelet Filter Bank Technique with EEG Signals.

Sleep stage classification plays a pivotal role in effective diagnosis and treatment of sleep related disorders. Traditionally, sleep scoring is done manually by trained sleep scorers. The analysis of electroencephalogram (EEG) signals recorded during sleep by clinicians is tedious, time-consuming and prone to human errors. Therefore, it is clinically important to score sleep stages using machine learning techniques to get accurate diagnosis. Several studies have been proposed for automated detection of sleep stages. However, these studies have employed only healthy normal subjects (good sleep

Serum brain-derived neurotrophic factor (BDNF) in sleep-disordered patients: relation to sleep stage N3 and rapid eye movement (REM) sleep across diagnostic entities.

Experimental and clinical evidence suggests an association between neuroplasticity, brain-derived neurotrophic factor and sleep. We aimed at testing the hypotheses that brain-derived neurotrophic factor is associated with specific aspects of sleep architecture or sleep stages in patients with sleep disorders. We included 35 patients with primary insomnia, 31 patients with restless legs syndrome, 17 patients with idiopathic hypersomnia, 10 patients with narcolepsy and 37 healthy controls. Morning serum brain-derived neurotrophic factor concentrations were measured in patients and controls. In p

The Impact of Sunlight and Artificial Light at Night on Sleep Stages: Evidence From a 7-Day Sensor-Based Observational Study.

BACKGROUND: Exposure to circadian entrainers, such as sunlight, positively impacts sleep architecture, while exposure before bedtime to circadian disruptors, such as artificial light and smartphone use, can negatively affect sleep. However, real-world evidence from longitudinal observational studies that simultaneously capture these factors alongside electroencephalography-derived sleep stages remains limited. OBJECTIVE: This study aimed to investigate the effects of specific environmental and behavioral factors on sleep metrics and architecture by using sensor-based measurements over 7 conse

Accelerometer-derived real-world sleep stages and risk of incident diseases: A UK Biobank cohort study and phenome-wide association analysis.

BACKGROUND: Previous studies utilizing self-reported data or polysomnography have found that poor sleep quality is associated with increased risk of morbidity and mortality. However, the association between real-world sleep patterns, including sleep stages, duration, and fragmentation, measured objectively with physiological data and the risk of incident disease among middle-aged and older adults has not yet been systematically evaluated. METHODS AND FINDINGS: In this cohort study, we analyzed wrist-worn accelerometer data from 95,559 Biobank participants and derived key metrics of real-world

An explainable deep learning approach for sleep staging in sleep apnea patients across all age subgroups from pulse oximetry signals.

Deep-learning (DL) approaches have been developed using pulse rate (PR) and blood oxygen saturation (SpO2) recordings from pulse oximetry to streamline sleep staging, particularly for obstructive sleep apnea (OSA) patients. However, lack of interpretability and validation across patients from a wide range of ages (children, adolescents, adults, and elderly OSA individuals) are two major concerns. In this study, a DL model based on the U-Net framework (POxi-SleepNet) was tailored to accurately perform 4-class sleep staging (wake, light sleep, deep sleep, and rapid-eye movement sleep) in OSA pat

Sleep spindles, K-complexes, limb movements and sleep stage proportions may be biomarkers for amnestic mild cognitive impairment and Alzheimer's disease.

PURPOSE: Sleep disturbances are common in Alzheimer's disease (AD) and mild cognitive impairment (MCI) patients. Non-rapid eye movement stage 3 (N3), rapid eye movement stage (REM), spindle density, and K-complex (KC) density are decreased in MCI and AD patients. Periodic limb movements in sleep (PLMS) are increased in other neurodegenerative diseases. We aimed to distinguish amnestic mild cognitive impairment (aMCI) patients from the overall population of MCI patients by comparing the N3 and REM proportions, the morphological characteristics of spindles and KCs and the periodic limb movement

Multi-Task Learning for OSA Detection and Sleep Staging via Multi-Scale Modeling.

Obstructive sleep apnea (OSA) and sleep fragmentation are closely linked physiological phenomena that play crucial roles in the diagnosis and management of sleep disorders. While numerous deep learning models have been developed for either OSA detection or sleep stage classification, few attempts have been made to address both tasks simultaneously. To this end, we propose MT-TASPPNet (Multi-Task Triple Atrous Spatial Pyramid Pooling Network), a unified multi-modal multi-task network that jointly performs automatic OSA event detection and sleep staging. The model integrates modality-specific fe

Energy-efficient real-time 4-stage sleep classification at 10-second resolution.

Sleep stage classification is critical for diagnosing and managing disorders like sleep apnea and insomnia. However, conventional methods like polysomnography are costly and impractical for long-term, home-based monitoring. This study presents an energy-efficient approach for detecting four sleep stages (wake, rapid eye movement (REM), light sleep, deep sleep) using a single-lead electrocardiogram (ECG) signal. We evaluate various machine learning and deep learning models, introducing two windowing strategies: (1) a 5-minute window with 30-second steps for machine learning and (2) a 30-second

Single-channel EEG-based sleep stage classification via hybrid data distillation.

Objective. With the advancement of deep learning technologies, more and more researchers have begun developing end-to-end automatic sleep stage classification frameworks. However, these frameworks typically require access to large electroencephalogram (EEG) datasets for training, which imposes a significant computational burden. Furthermore, EEG data contains patient privacy information, and using such data for training raises concerns about privacy infringement. To address these issues, we propose a hybrid data distillation method. We aim to enable single-channel EEG sleep stage classification

Four State Sleep Staging From a Multilayered Algorithm Using Electrocardiographic and Actigraphic Data.

PURPOSE: Sleep studies are important to evaluate sleep and sleep-related disorders. The standard test for evaluating sleep is polysomnography, during which several physiological signals are recorded separately and simultaneously with specialized equipment that requires a technologist. Simpler recordings that can model the results of a polysomnography would provide the benefit of expanding the possibilities of sleep recordings. METHODS: Using the publicly available sleep data set from the multiethnic study of atherosclerosis and 1769 nights of sleep, we extracted a distinct data subset with en
