Review of a 26-year experience with transduodenal sphincteroplasty and sphincterotomy was undertaken (1) to analyze critically the indications for and results of these procedures and (2) to determine which preoperative factors correlate with a good or poor outcome. The classical treatment of angiocholitis consists in desobstruction of the hepatic choledocus duct followed by external biliary drainage. However, this technique is unsuccessful in 66% of the cases, and this leads to difficult surgical reinterventions. The authors present an original technic of Oddi sphincterotomy. A nylon catheter introduced into the bile duct through the cystic duct is pushed through the papilla, screwed to the lower branch of a pair of scissors and withdrawn. The sphincter is thus divided simply and surely without using choledocotomy.

The physiology and pathophysiology of the sphincter of Oddi are poorly understood. The relationships of functional disorders of the sphincter to biliary and pancreatic disease and of organic lesions of the papilla to pancreatic inflammatory disease are subjudice to say the least.
