PURPOSE: Operative treatment of adult spinal deformity (ASD) has been shown to improve patient health-related quality of life (HRQOL). Selection of the uppermost instrumented vertebra (UIV) in either the upper thoracic (UT) or lower thoracic (LT) spine is a pivotal decision with effects on operative and postoperative outcomes. This review overviews the multifaceted decision-making process for UIV selection in ASD correction.
METHODS: PubMed was queried for articles using the keywords "uppermost instrumented vertebra", "upper thoracic", "lower thoracic", and "adult spinal deformity".

AIM OF THE STUDY: Postural deformities are common in Parkinson's disease (PD) patients. Several treatment options have been reported, but responses to these treatments appear unpredictable. Istradefylline is a novel drug for PD. Cases of PD patients whose postural deformities were improved after withdrawal of dopamine agonists and initiation of istradefylline are presented.
MATERIALS AND METHODS: Four consecutive patients with postural deformities including antecollis, Pisa syndrome, and camptocormia were recruited and treated with istradefylline in combination with withdrawal of dopamine ago

Loss of bone mass and an increased risk of fractures is an issue of major concern for physicians treating inflammatory bowel disease patients. Multiple causes for decreased bone mass have been identified in the past, but the role of inflammation may be of pivotal importance. We here discuss the latest insights into the pathogenesis of inflammatory bowel disease-related bone loss and the implications for treatment.

Postoperative sagittal-plane cervical spine deformities are a concern when laminectomy is performed for tumor resection in the spinal cord. These deformities appear to occur more commonly after resection of intramedullary spinal cord lesions, compared with laminectomy for stenosis caused by degenerative spinal conditions. Postlaminectomy deformities are most common in pediatric patients with an immature skeletal system, but are also more common in young adults (< 25 years of age) in comparison with older adults. The extent of laminectomy and facetectomy, number of laminae removed, location of

To understand the role seating plays in the support of posture and spinal articulation, it is necessary to study the interface between a hum … res. This method of lumbar curvature prediction shows potential for use in the assessment of seated spinal curvatures and the corresponding design of seating to accommodate those curvatures; however, additional inputs will be necessary to better predict the postures as lordosis is increased.

PURPOSE: Idiopathic scoliosis (IS) is defined as a structural lateral spinal curvature ≥ 10° in otherwise healthy children and is the most common pediatric spinal deformity. IS is known to have a strong genetic component; however, the underlying etiology is still largely unknown. Animal models have been used historically to both understand and develop treatments for human disease, including within the context of IS. This intended audience for this review is clinicians in the fields of musculoskeletal surgery and research.
METHODS: In this review article, we synthesize current literature of ge

By definition, congenital spine deformities-scoliosis, kyphosis, and lordosis-are due to abnormal vertebral development. Thus, affected children tend to have a curvature noted much earlier in life than typical patients with idiopathic scoliosis. This early development of the deformity has resulted in a tendency for the young child with congenital deformities to receive less than optimal care. These curves must not be allowed to progress. In many cases, early fusion is necessary, which is preferable to allowing severe curves to develop.

Scoliosis is a three-dimensional deformity of the vertebral column that commonly emerges during growth, yet the biological mechanisms that initiate and drive curve progression remain poorly understood. Serotonin (5-hydroxytryptamine [5-HT]) has long been linked to scoliosis through its relationship with pineal melatonin signaling and through genetic associations with human disease, but its role in spinal morphogenesis has remained unclear. Here, using zebrafish, we show that serotonergic signaling is required for normal spine morphology. Transient exposure to 5-HT during development induces la

Background/Objectives: Strength training is a widely recommended form of physical activity due to its extensive health benefits and positive effects on musculoskeletal function, although improper technique and balance deficits may increase injury risk. While sex differences in spinal curvature and postural control have been identified in the general population, it remains unclear whether these differences persist among recreationally strength-trained individuals. This cross-sectional study investigated sex-specific differences in sagittal spinal alignment and static balance and examined potent

Camptocormia is an involuntary flexion between 15-45° of the thoracolumbar spine when standing, walking or sitting, which disappears completely in the supine position. Is associated to Parkinson's disease and other neurodegenerative conditions, being its physiopatology unknown. We report three cases of atypical anti-psychotic induced camptocormia.

Camptocormia is a postural affliction characterised by abnormal flexion of the thoracolumbar spine, which appears upon standing, increases with walking, and disappears in the supine position. Only five cases of camptocormia secondary to idiopathic inflammatory myopathies have been described in the literature. In this case report, we describe the case of a male patient 67 years of age who suffered from polymyositis for 18 years, which was associated with manifestations compatible with camptocormia. The clinical manifestations were stable and did not progress. Polymyositis is an idiopathic infla

Background/Objectives: The lumbopelvic complex plays a key role in postural regulation, core stabilisation and biomechanical coordination of … variability, and the majority of participants presented abnormalities in lumbopelvic alignment and spinal curvatures. The most common finding was flattening of physiological spinal curvatures, particularly reduced thoracic kyphosis coexisting with normal or reduced lumbar lordosis. No significant associations were found between temperament and most of the analysed …

The relation between LBP and SBO was studied in 1244 subjects, of which 800 had LBP and 444 served as controls. Sex, age, occupational characteristics, spine curvatures, spinal movements and work history were compared. It is concluded that SBO does not play a causative role, does not cause a proneness to LBP and does not influence the chronicity of LBP. Some data seem to indicate that the severity of LBP may be increased by SBO.

Comprehensive data on sagittal spinal alignment, regional mobility, trunk muscle strength and imbalances in 9-11-year-old children participating in intensively organized sport (IOS) are limited. This cross-sectional study evaluated the association between IOS participation and the aforementioned postural parameters, compared with a non-intensive comparison cohort. A total of 143 children were included (IOS: n = 66; controls: n = 77). Sagittal spinal curvatures (thoracic kyphosis and lumbar lordosis) and regional mobility were assessed using Spinal Mouse®, software-generated sagittal spinal cur…

BACKGROUND: Abnormal posture and spinal mobility have been demonstrated to cause functional impairment in the quality of life, especially in the postmenopausal osteoporotic population. Most of the literature studies focus on either thoracic kyphosis or lumbar lordosis, but not on the change of the entire spinal alignment. Very few articles reported the spinal alignment of Chinese people. The purpose of this study was threefold: to classify the spinal curvature based on the classification system defined by Satoh consisting of the entire spine alignment; to identify the change of trunk mobility;

AIMS: Double crush syndrome is a clinical condition composed of neurological dysfunction due to compressive pathology at multiple sites along a single peripheral nerve. The aims were to investigate the characteristics and disabilities of women with double crush syndrome, to compare the spinal alignment to healthy women, and to determine the cut-offs for the spinal alignment characteristics.
METHODS: Twenty women with double crush syndrome (age: 49.50 ± 8.64 years) and 21 asymptomatic healthy controls (age: 44.76 ± 7.82 years) were included in the study. The physical characteristics, pain inte
