Imaging of tuberculosis of the spine.

Tuberculosis (TB) is the leading cause of death worldwide that can be attributed to a single infectious agent. With the onset of the AIDS epidemic, there has been a resurgence of TB in recent years. Skeletal TB constitutes 1% to 3% of extrapulmonary cases, and typically involves the spine. TB of the spine should be considered in the differential diagnosis of many spinal conditions affecting patients of all ages. The pathophysiology, clinical and imaging features of TB of the spine are reviewed, with illustrations of findings on radiography, computed tomography and magnetic resonance imaging. F…

[Congenital anomalies of the spine].

Congenital anomalies of the spine usually originate in (toxic) disturbances during pregnancy. There are occasional hereditary types. These are characterized by the presence of multiple anomalies. Congenital scolioses are often associated with other anomalies like spinal dysraphy (20-30%) and fusion of the ribs. Furthermore heart defects, Sprengel's deformity, cleft palates, hemimelias, clubfeet or congenital vertical talus are frequently associated with congenital scoliosis. We classify in failures of formation, segmentation and combined types. The type of malformation determines the prognosis…

Granulocytic sarcoma of the spine.

OBJECTIVE: Granulocytic sarcomas (chloromas) are tumors consisting of primitive myeloid cells. They are rare manifestations of acute and chronic leukemias and can occasionally precede the development of systemic disease by weeks to years. Spinal complications of chloromas, such as cord compression secondary to epidural tumor or cauda equina syndrome, have been described but are uncommon. METHODS: We present eight new cases of spinal chloroma. Three patients displayed significant motor deficit in the form of paraparesis or paraplegia. All patients complained of lower back pain, with other comp

Extradural infections of the spine.

OBJECTIVES: We have observed a recent increase in the incidence of spinal extradural infections. To determine postoperative outcome and prognosis we have undertaken a retrospective study on patients with spinal extradural abscesses between 1978 and 1996 treated in the Department of Neurosurgery at the Nordstadt Hospital in Hannover, Germany. METHODS: Case records, outpatient files, operation notes, neuroradiological examinations and pathological reports were analysed. Neurological function was documented using a score system for each symptom. RESULTS: Twenty-two patients underwent 24 operati

[Complex injuries of the spine].

3 different types of complex spinal trauma are defined: Type I means a multilevel contiguous or non contiguous unstable injury, type II is d … ome" with lesions of the upper abdominal organs and a flexiondistraction injury of the upper lumbar spine. The main problem is to make the diagnosis of both components initially. Most of the patients may be treated in one operation by first taking care of the abdominal injury and than stabilizing the spine. The prognosis of this combination is favorable …

Imaging of the cervical spine.

The emergence of multiplanar spiral computed tomography and high-resolution magnetic resonance imaging has resulted in the ability to see cervical spine anatomy and pathologic conditions in detail. Appropriately chosen and performed, these imaging studies can provide an anatomic basis for a clinical diagnosis and a therapeutic plan. In the evaluation of cervical spondylosis, magnetic resonance imaging is more commonly performed than computed tomography because of its superior depiction of soft tissue anatomy, including intervertebral discs and spinal cord disease. However, computed tomography …

[Sagittal balance of the spine].

PURPOSE OF THE STUDY: The authors examined the modifications of sagittal shape and muscular functions in different vertebral diseases in order to investigate their correlations and to specify the role of hip extension in standing posture. MATERIALS AND METHODS: We included 57 patients presenting various diseases, among which lumbar kyphosis, spondylosis and spondylolisthesis, backache and lumbar stenosis. We measured joint mobility (hip extension, lumbar flexion), muscular retractions (ilio-psoas, hamstrings, rectus femoris), and muscular strength (spine flexors and extensors, hamstrings, qua…

Eosinophilic granuloma of the spine.

Twenty patients treated for eosinophilic granuloma of the spine were studied. Only 40% demonstrated the classical radiographic picture of vertebra plana. In 60% a lytic lesion of the vertebral body or the posterior elements was found. Seven patients underwent surgery; the indications were neurological involvement or failure of the biopsy to disclose the diagnosis. At an average follow-up period of 7 years, 17 patients are well and alive with no residual spinal pain, neurological compromise, recurrent disease, or extraskeletal involvement. Vertebral body collapse underwent some regeneration but…

Extracoelomic approach to the spine.

A modified transthoracic approach to the thoracic vertebral column is described. In this method, the parietal pleura is detached from the chest wall and retracted with the visceral pleura and its contents. A direct approach to the vertebral bodies is thus achieved without transgression of the intrapleural space. The technique can be extended to include exposure of the thoracolumbar spinal column, utilising a thoracoabdominal approach with extrapleural and extraperitoneal dissection. Management of the costophrenic detachment is thus simplified. This approach has significant advantages for ortho

Osteoblastomas of the cervical spine.

Osteoblastomas of the cervical spine frequently occur with anterior vertebral body involvement despite the classical teaching, which suggests that involvement is usually confined to the posterior elements. A review of osteoblastomas that involved the cervical spine was conducted at a single institution over 20 years. Four patients were identified with osteoblastoma of the cervical spine from a total of 13 spinal osteoblastomas, and their conditions were assessed to determine the anatomic extent of neoplastic involvement, the surgical margins obtained at resection, methods of spinal stabilizati…

The spine in osteogenesis imperfecta.

The natural history and treatment of spinal deformity is presented in light of the radiographic severity of the disease. The radiographic findings are grouped according to the changes over time and are prognostically significant for ambulation, spinal deformity, and life span.

[Fractures of the cervical spine].

A gentle traction is often sufficient to repose unstable lesions of the cervical spine, when a retention by means of conservative methods is not possible. Therefore a ventral, intracorporal fusion with autologous, corticospongious chip material combined with a plate fixation is an adequate solution to obtain a stable, movable blocking of the lower six cervical vertebrae. In case of a completely ruptured dorsal ligamentous apparatus, a ventral bone-chip blocking is generally insufficient. Ventral spondylodesis should be combined with a dorsal traction belt in these cases. Fresh, instable fractu…

Sagittal profiles of the spine.

The sagittal plane of the growing spine was studied from standardized radiographs of 670 normal subjects to establish standards of reference for thoracic kyphosis, lumbosacral lordosis, and sacral inclination. Cobb's method of measuring spinal deformities was compared with new indices of kyphosis and lordosis that take into consideration the length and width of each curve. Boys and girls had comparable degrees of kyphosis. Although girls initially had higher degrees of lordosis and sacral inclination, by maturity these measurements were similar in both sexes. Significant correlations were foun…

Cryomicrotomy of the lumbar spine.

To investigate the pathogenesis of lumbar canal stenosis, a morphologic study was carried out by cryomicrotomy using 10 fresh lumbar spine specimens from the 3rd lumbar vertebra to the 1st sacral vertebra. The cryomicrotomy findings confirmed that anatomical relations were clearly demonstrated between dura, nerve root, osseous structure, disc and ligamentum flavum in frontal, sagittal, and transverse section. Furthermore, in sagittal section specimens, thickness of ligamentum flavum varies from extension to flexion. These conditions play a dynamic factor in the pathogenesis of spinal canal ste…
