Blunt trauma to the spleen.

BACKGROUND: The management of splenic injury resulting from blunt trauma in adults is controversial, with an increasing trend towards non-operative management and conservation of the spleen. A retrospective review was performed on adult patients treated in a single institution for splenic injury resulting from blunt trauma in an attempt to identify factors important in selecting an appropriate management option and predicting the success of that option. METHODS: Associated injuries (standardized using Injury Severity Scores), clinical signs at presentation, computed tomographic grading of spl…

[Inflammatory pseudotumor of the spleen].

A case of inflammatory pseudotumor of the spleen in a 69-year-old woman who was studied for elevated globular sedimentation rate is presented. The lesion was observed on echography as a mixed solid-cystic mass. Magnetic resonance showed a slight decrease in signal intensity in potentiated T1 sequences and a heterogenic mass with high signal areas in T2 images and protonic density. Splenectomy was performed and histopathologic study demonstrated an inflammatory pseudotumor of 10 cm in its greatest size. Two year follow up was satisfactory. On review of the literature only 34 cases of inflammato…

[Epidermoid cyst of the spleen].

Epidermoid cysts rarely occur in the spleen and are often found fortuitously. Epidermoid cyst of the spleen is an inclusion of epidermoid elements within a mesodermic structure. Treatment is partial conservative splenectomy followed by ultrasound surveillance.

[The spleen in hereditary spherocytosis].

Hereditary spherocytosis is the most common inherent, autosomal dominant hemolytic anemia. Mild splenomegaly, venostasis and common decrease of while pulp is characteristic for hereditary spherocytosis. Cords are filled with spherocytes, sinuses can be empty or squeezed. Both sinuses and veins include ghost erythrocytes that lost haemoglobin. They are seen light-microscopically in differential interferent contrast. Macrophages are numerous, sinus lining cells are hypertrophic. Erythrophagocytosis is hardly seen by electron microscope. Not all red blood cells are spherocytes. Spherocytes are se

Epidermoid cysts of the spleen.

Primary nonparasitic splenic cysts are very rare. Clinical manifestations vary but are often not very typical. Ultrasound and computed tomography are of use for establishing diagnosis. A microscopic examination of the surgical specimen is the only way to make the diagnosis of an epidermoid cyst. The histological characteristic of an epidermoid cyst is the presence of an epidermoid epithelial cyst lining of the inner surface. Treatment requires surgery and is necessary to prevent serious complications. Spleen saving surgical procedures are advocated. We present two cases of young patients with …

Epidermoid cyst of the spleen.

A sixteen-year-old girl treated by total splenectomy for epidermoid cyst of the spleen is presented: Epidermoid cysts of the spleen account for ten percent of non-parasitic cysts. Abdominal ultrasonography and computed tomography are the most reliable studies available in the diagnosis of these cysts. Partial splenectomy is the best mode of treatment for this disease, if feasible.

[Traumatic rupture of the spleen].

In the work the authors present their own experience in the treatment of spleen traumatic rupture in the period of 10 years (1978-1987). In that period in 13 patients spleen were removed because of traumatic rupture. Spleen iatrogen lesions, spontaneous spleen ruptures and spleen-removing caused by other indications were not considered. The 6 male and 7 female patients were operated on the 6 year old child was the youngest and 79 year old man was the oldest. Spleen traumatic rupture is mostly caused by falls, accidents (traffic) and then by blows and fights. 4 patients had isolated spleen inju…

Interventional radiology in the spleen.

Despite the widespread use of interventional radiologic techniques, there has been reluctance to apply these to the spleen. Concern for bleeding and difficulty in negotiating around the colon and pleura have limited its use. The authors report their experience with interventional radiology of the spleen in 35 cases, including percutaneous biopsy (n = 5), diagnostic and therapeutic fluid aspiration (n = 14), and catheter drainage of abscesses (n = 9), hematomas (n = 2), intrasplenic pancreatic pseudocysts (n = 2), and necrotic tumor (n = 1). Transsplenic fluid aspiration and biopsy of the pancr…

[Torsion of a wandering spleen].

The torsion of an ectopic (= wandering) spleen is a rare entity especially in children. Clinically it presents either with signs of an "acute abdomen" or as a fairly painful abdominal "tumor" of the chronic recurrent type. Ultrasonography, arteriography, and additional scintigraphy in special cases are of greatest value in the preoperative diagnostic management. Splenopexy, sometimes after resecting parts of a very enlarged organ, is the therapy of choice. The complete exstirpation of the ectopic spleen should be reserved only for exceptional cases. By presenting a case report, the etiologic f…

Circulatory dynamics of the spleen.

Circulatory dynamics are examined in terms of the open and closed circulation, pathologic changes in blood dyscrasia and portal hypertension, and the rationale of circulatory control (ischemic therapy) in treatment of hypersplenism.

Peliosis of the spleen with rupture.

A case of peliosis of the spleen without apparent involvement of the liver or other organs is presented. The peliosis was discovered following rupture of the spleen after minimal trauma and was successfully treated by surgery. None of the common associations of peliosis was present (i.e., tuberculosis or intake of steroid compounds). Drugs addiction and chronic alcoholism were the only coexisting debilitating conditions exhibited by this patient.
