A 16-year-old man was brought to the emergency department by his father with apparent alcohol intoxication. While in the department, it was noted that the patient had ongoing tachycardia, hypotension and abdominal pain. His recent medical history included a self-resolving illness of fatigue, muscle aches and sore throat. A CT scan of his abdomen revealed an enlarged spleen with a splenic infarct and rupture, likely secondary to infectious mononucleosis. The patient was treated conservatively for 12 days.

Colonoscopy is the gold standard for the study of colorectal pathology. Splenic injury is a rare but potentially fatal complication to consider. Therefore, we present two cases whose management was different and we show their clinical presentation, their diagnosis and their treatment in order to recognize this complication early to establish early treatment.

Systemic lupus erythaematosus (SLE) is known to involve the reticuloendothelial system, but spontaneous splenic rupture (SSR) in the context of the disease is a very rare complication. We observed a 61-year-old woman with an unremarkable previous medical history who presented with SSR and underwent an emergency splenectomy. The histopathological analysis of the specimen revealed signs of vasculitis. On review of symptoms with the patient, a history of oligoarthralgia, photosensitivity, xerostomia and Raynaud phenomenon was elicited. Laboratory investigations revealed lymphopaenia, mild protein…

A 47-year-old man presented with sudden onset of severe, generalised abdominal pain and collapse. He also had a 4-month history of lethargy and weight loss. On examination he was shocked with a distended tender abdomen. He had haemoglobin of 3.8 g/dl and a white cell count count of 280.3×10(9)/l with predominance of neutrophils. Arterial gases showed mixed metabolic-respiratory acidosis. A CT scan of abdomen showed active extravasation in splenic bed. In view of probable hyperviscosity syndrome, it was decided to attempt an angiographic embolisation. This was successfully carried out but 4 h l

Atraumatic splenic rupture is an uncommon but poorly defined clinical condition. Its diagnosis may be missed or delayed because of low clinical suspicion, especially in the absence of trauma. The primary aim of this study was to describe the experience with atraumatic splenic rupture in a district general hospital. Over a 6-year period (2004-2010), seven patients were identified, producing an incidence of 1.2/year. Computed tomography confirmed the diagnosis preoperatively in all the cases where it was performed (n=5). All patients required total splenectomy; indications included peritonitis, …

BACKGROUND: Scrotal pain and swelling is a common complaint encountered in emergency medicine. The differential diagnosis includes testicular, scrotal, and intra-abdominal pathology.
CASE REPORT: We present a case of an 80-year-old man on warfarin therapy presenting with acute atraumatic scrotal pain and swelling initially diagnosed as a hydrocele. The diagnosis was subsequently determined to be a communicating hematocele secondary to occult splenic rupture.
CONCLUSION: Intra-abdominal pathology can result in scrotal pain and swelling due to passage of intra-abdominal contents into the scrot…

Splenic rupture in pregnancy or postpartum is a rare and frequently misdiagnosed occurrence. Failure to recognise it is common, and can be fatal for both mother and child. With all our highly developed diagnostic methods and equipment, aetiology of splenic ruptures in pregnancy remains a dilemma in many cases.

Rapid evaluation of the hypotensive patient in the emergency department is essential. The availability of ultrasonography in the ED, performed by emergency physicians and surgeons, has made it easier to evaluate the hypotensive trauma patient. We describe a 44-year-old man transferred to our institution from a community hospital for evaluation of syncope and hypotension with no obvious cause. On arrival the patient began to complain of slight lower abdominal pain. The patient's physical examination revealed minimal abdominal tenderness. A rapid ultrasound examination performed at bedside revea

Perinatal death due to maternal injury is unusual unless associated with extensive maternal trauma or death. An unusual case of neonatal death due to in utero traumatic splenic rupture in the absence of significant maternal injury is presented. The case alerts physicians responsible for neonatal care to the existence of treatable causes of neonatal distress following maternal trauma.
