Previous studies have shown the benefit of sputum purulence screens in routine bacterial culture. We have correlated 944 sputum smear and Legionella pneumophila culture results and demonstrated that 47 to 84% of L. pneumophila-positive samples would have been discarded by using established sputum purulence screens. We recommend acceptance of all specimens submitted for Legionella culture.

A method based on DNA amplification (PCR) and hybridization for the detection of M. tuberculosis was used to test 86 sputum specimens from 52 patients in whom tuberculosis was suspected. M. tuberculosis was detected in 35 specimens, with 1 false positive. The finding was negative in 51 specimens, of which 4 were false negative. Pulmonary tuberculosis is not uncommon in the elderly, particularly those in institutions, and is notoriously difficult to diagnose. Few bacilli are excreted by individuals with non-cavitating disease and positive cultures are usually required to confirm the diagnosis. We report a case in which Cryptosporidium oocysts were detected during routine auramine screening of a sputum specimen for acid-fast bacilli. The patient was a 28-year-old male with acquired immune deficiency syndrome and intestinal cryptosporidiosis. Microbiology laboratory workers should be alert to the possibility that these oocysts may be present in sputum specimens from patients with acquired immune deficiency syndrome.
