One-Muscle Strabismus Surgery: A Review.

Proper management of a patient with small to moderate horizontal deviations continues to be challenging for the strabismus surgeon. The use of one-muscle surgery for comitant strabismus has been controversial because of concerns that it may result in a significant number of undercorrections and/or ocular incomitance. Recent literature on unilateral rectus muscle surgery has shown that this surgery is a safe and effective procedure for small and moderate angle horizontal deviations. It has the advantage of limiting surgery to one eye, reducing operative time and possible cost to the family, and…

A Conjunctival Retractor during Strabismus Surgery.

The use of a conjunctival retractor that expands a small incision in the conjunctiva/Tenon's capsule facilitated the performance of strabismus surgery through a fornix incision. A wide field of exposure revealed the entire area of the muscle insertion. While there is a risk of conjunctival tearing in elderly patients, the use of the retractor is valuable.

Restrictive Strabismus Following Frontotemporal-orbitozygomatic Craniotomy.

The frontotempotal-orbitozygomatic craniotomy (FTOZ) is a standard approach for large sphenoid wing meningiomas (SWMs). Nevertheless, resection of these tumors is not without ophthalmologic risks. This series presents two patients with acute postoperative restrictive strabismus following tumor resection and orbital wall reconstruction. Forced duction testing and postoperative imaging revealed impingement of the lateral rectus muscle caused by an alloplastic implant and/or residual bone, prompting immediate orbitotomy and restoration of normal extraocular muscle function. This report highlights…

Infectious Pseudomonas scleritis after strabismus surgery.

Scleral inflammation and necrosis are rare but serious complications of ocular surgery. We present a case of Pseudomonas infectious scleritis following strabismus surgery with an exuberant, presumably sterile endophthalmitis in an immunocompromised 62-year-old man that was responsive to topical and oral antibiotics.

Intelligence Quotient (IQ) in Congenital Strabismus.

PURPOSE: To evaluate intelligence quotient (IQ) in patients with congenital strabismus. METHODS: All patients with congenital strabismus scheduled for surgery were enrolled consecutively over a one year period in a cross-sectional study and were evaluated for verbal, performance and total IQ scores, and compared to the mean normal IQ of 100±15. RESULTS: During the study period, 109 patients with mean age of 18.4±10.5 (range, 4-63) years were included. Educational status in most patients (80%) was less than high-school. Most patients (80%) lived in urban areas and 46 patients (42.2%) had some…

Anterior segment angiography in strabismus surgery.

BACKGROUND: Anterior ciliary arteries travelling along recti muscles provide anterior segment vascularization, which can be compromised by surgery involving more than 2 muscles. PATIENTS AND METHODS: We studied retrospectively the files of 10 patients in whom a fluorescein angiography of the iris had been performed as a pre-operative assessment prior to a second or third oculomotor surgery. RESULTS: The median age of the patients was 47.5 years (range 15 to 73 years). Relative iris ischemia was present in 4 patients following multiple surgeries, none of them presenting any general cardiovasc

Formaldehyde-induced keratopathy after strabismus surgery.

We report on a patient who developed corneal haze and permanent ptosis after strabismus surgery. Formaldehyde-disinfected sodium hyaluronate which was used to protect the corneal epithelium during surgery was thought to have induced keratopathy and other findings. To our knowledge, this is the first report of corneal haze formation together with ptosis secondary to formaldehyde toxicity as a complication of strabismus surgery.

Diplopia and strabismus following ocular surgeries.

Postoperative diplopia and strabismus may result from a variety of ocular surgical procedures. Common underlying mechanisms include sensory disturbance, scarring, direct extraocular muscle injury, myotoxicity from injections of local anesthesia or antibiotics, and malpositioning of extraocular muscles by implant materials. The most common patterns are vertical and horizontal motility disturbance. Treatment options include prisms, botulinum, occlusion, or surgery.

An unusual complication after strabismus surgery.

Orbital cellulitis is rarely reported after strabismus surgery; fewer than 10 cases have been reported. Nonetheless, orbital cellulitis is a potentially sight- and life-threatening condition. A high index of suspicion, use of a multidisciplinary approach, early diagnosis, aggressive treatment, and close monitoring are all important means of avoiding potentially irreversible visual loss and systemic complications. We report a case where early use of aggressive treatment to manage a post-strabismus surgery infection led to a good outcome.

Chiari 1 malformation presenting as strabismus.

INTRODUCTION: Strabismus has been previously reported as a rare presenting feature of the Type 1 Chiari malformation. CASE REPORTS: We report a case series of twelve patients with Chiari 1 malformations with either strabismus or diplopia as part of their initial presentation. Ten patients had diplopia at the time of presentation, while 2 young children (ages 2 and 6) presented with esotropia without complaints of diplopia. Of the 10 patients with diplopia, 7 were constantly or frequently tropic while 3 had symptomatic phorias. One or more unusual features of the strabismus led to further inve…

Augmented Hummelsheim procedure for paralytic strabismus.

PURPOSE: To report a modification of the Hummelsheim procedure for use in the management of paralytic strabismus. METHODS: Eight patients with paralytic strabismus secondary to third nerve palsy (n=1), sixth nerve palsy (n=3), combined cranial nerve palsy (n=1), or extraocular muscle damage (n=3) were treated using a modification of the Hummelsheim transposition procedure. The procedure involves half-tendon transpositions of the adjacent rectus muscles to the insertion of the paralyzed muscle, coupled with resection of the transposed halves. Further augmentation was achieved by surgical or ph…

Botulinum toxin therapy in dysthyroid strabismus.

We report our experience with the use of Botulinum toxin injection in 38 patients (64 injections) with severe dysthyroid strabismus. Three quarters of the injections led to a decrease in the angle of the squint by a mean 75% of the initial deviation. The average duration of effect was two months. Twenty six patients went on to surgery after stabilisation of their squint and endocrine status. Six patients achieved a stable long-term result with Botulinum toxin only. We suggest these results of treatment of early dysthyroid myopathy are more consistent with the characteristics of inflammatory sp…

Combined intraocular lens and strabismus surgery.

Ten cases of combined strabismus surgery and intraocular lens implantation were reviewed to determine if there were increased risks and complications. A review of these cases revealed the contrary was true; i.e., there were no complications arising from the combined surgery and there were benefits in the decreased hospitalization and operative time, less mental trauma, and improved muscle surgical outcome. From these results, we conclude that combined intraocular lens and strabismus surgery is efficacious and indicated in patients with strabismus and cataracts or aphakia.
