The effect of electroconvulsive therapy using propofol and succinylcholine on the intraocular pressure.

PURPOSE: Electroconvulsive therapy (ECT) is a therapeutic procedure in many mood and psychiatric disorders. After induction of general anesthesia by administering an induction dose of an intravenous anesthetic such as Propofol, intravenous succinylcholine is often used to prevent bone fractures and joint dislocations during ECT. Intraocular pressure (IOP) is raised by succinylcholine and tonic-colonic convulsion, and decreased by propofol administration. To our knowledge, there is no published paper on the effect of ECT using propofol and succinylcholine on the IOP. This study for the first tim…

The incidence of masseter muscle rigidity after succinylcholine in infants and children.

To determine whether the incidence of masseter muscle rigidity is affected by the anaesthetic induction sequence, we prospectively studied for ten months the anaesthetic course in 5,641 infants and children who received muscle relaxation to facilitate tracheal intubation. The anaesthetic induction sequence consisted of intravenous sodium thiopentone (STP) 5 mg.kg-1 alone, halothane induction alone 1-4%, or halothane followed by STP. Inhalational inductions with halothane included nitrous oxide and oxygen. Tracheal intubation was facilitated by either intravenous succinylcholine (Sch) at least …

Unusual response following use of succinylcholine in a horse anesthetized with halothane.

A syndrome similar to malignant hyperthermia developed in a 545-kg Quarter Horse while anesthetized with halothane for cataract removal. Succinylcholine administration caused prolonged, severe muscle fasciculations followed by tachycardia, and an elevated blood pressure. Later, while the horse was still under anesthesia, its body temperature rose 2 degrees C, and respiratory acidosis developed. Myositis developed after surgery, but the horse recovered.

Cardiovascular effects of nondepolarizing relaxants employed for pretreatment prior to succinylcholine.

A pregnant woman with severe pre-eclampsia experienced a hypertensive crisis following a pretreatment dose (20 mg) of gallamine. That episode initiated a study to determine the cardiovascular effects of non-depolarizing muscle relaxants in 58 nonobese, ASA physical status I and II adults. Subjects were assigned randomly to one of five treatment groups as follows: gallamine (0.29 mg X kg-1), d-tubocurarine (0.04 mg X kg-1), metocurine (0.014 mg X kg-1), pancuronium (0.007 mg X kg-1), or normal saline (control). Baseline measurements of systolic, diastolic, mean arterial pressure, heart rate (HR

Effect of diazepam pretreatment on succinylcholine-induced muscle fasciculation in the dog.

The efficacy of diazepam in attenuation of succinylcholine-induced muscle fasciculations was studied in the dog. Dosage levels of 0.11, 0.22, 0.55, or 1.1 mg of diazepam/kg of body weight were studied in 4 groups of 5 dogs each; another group of 5 dogs served as a control. There was no difference in the occurrence or severity of succinylcholine-induced muscle fasciculations between dogs given diazepam and nontreated controls. Using serum potassium as a quantitative index of physiologic effects of muscle depolarization, differences could not be detected between treated and nontreated dogs.

Genetic Pseudocholinesterase Deficiency Unmasked After Succinylcholine-Rivastigmine Interaction: A Case Report.

Succinylcholine is widely used in anesthesia for electroconvulsive therapy (ECT). Succinylcholine apnea is a recognized rare complication. A patient's dormant, undiagnosed, genetic pseudocholinesterase deficiency was clinically triggered and unmasked by the combination of long-term rivastigmine therapy and succinylcholine used in ECT, resulting in post-ictal succinylcholine apnea. In subsequent treatments, low-dose rocuronium was successfully used instead. Clinicians providing electroconvulsive therapy should consider potential interactions between cholinesterase-inhibiting drugs and succinylc…

The optimal succinylcholine dose for intubating emergency patients: retrospective comparative study.

BACKGROUND: Succinylcholine remains the drug of choice for satisfactory rapid-sequence tracheal intubation. It is not clear from the literature why the 1 mg/kg dose of succinylcholine has been traditionally used. The effective dose (ED95) of succinylcholine is less than 0.3 mg/kg. The dose of 1 mg/kg represents 3.5 to 4 times the ED(95). OBJECTIVES: To compare the effect of the traditionally used 1 mg/kg of succinylcholine with lower doses of 0.6 mg/kg and 0.45 mg/kg on intubation condition regarding the onset time, duration of action, duration of abdominal fasciculation, and the intubation g…

[Precurarization with rocuronium prevents fasciculations and biochemical changes after succinylcholine administration].

OBJECTIVE: To determine the efficacy of rocuronium to prevent fasciculations and biochemical changes after succinylcholine administration. PATIENTS AND METHODS: Prospective, randomized double-blind trial enrolling 60 ASA I-II patients scheduled for elective surgery under general anesthesia. The patients were assigned to 2 groups to receive either 0.06 mg x Kg(-1) of rocuronium or physiological saline solution 90 seconds before administration of 1.5 mg x Kg(-1) of succinylcholine. The incidence and severity of fasciculations and serum concentrations of potassium before anesthesia and 3, 5, and…

Low-dose succinylcholine facilitates laryngeal mask airway insertion during thiopental anaesthesia.

We have assessed the optimal dose of succinylcholine to provide satisfactory conditions for insertion of a laryngeal mask airway (LMA) without causing myalgia during induction of anaesthesia with thiopental. We studied 60 adult patients, allocated randomly to one of three groups: group 1 (n = 20) received normal saline, group 2 (n = 20) received succinylcholine 0.25 mg kg-1 and group 3 (n = 20) received succinylcholine 0.5 mg kg-1. Insertion of the LMA was performed 1 min after administration of succinylcholine or saline. Insertion conditions were significantly better in group 3 compared with …

Length-tension curves of human eye muscles during succinylcholine-induced contraction.

We have made intraoperative, continuous-registration, length-tension diagrams of detached eye muscles in 80 strabismus patients under general anaesthesia. In 47 of these we repeated the measurements after contraction evoked by succinylcholine chloride. In contracting horizontal and vertical rectus muscles, we found a linear relation between length and tension. In contracting oblique muscles, however, the relation between length and tension was frequently nonlinear. In superior oblique palsies, the superior oblique was found to be stiff after elongation and before injection of succinylcholine, …

Assessment of Mini-dose Succinylcholine Effect on FacilitatingLaryngeal Mask Airway Insertion.

INTRODUCTION: Laryngeal Mask Airway (LMA) has gained wide acceptance for routine airway management and with increasing emphasis on day care surgery it is widely used. The aim of this study was to assess the effects of mini dose succinylcholine (0.1mg/kg) with semi-inflated cuff on facilitation of laryngeal mask airway insertion in order to achieve more satisfaction yet less complications . METHODS: In a randomized double-blinded study, sixty ASA 1, 2 and 3 patients aged 20-60 years scheduled for urologic surgical procedures were included. Thirty patients received succinylcholine (Group S), an…

Methohexital and succinylcholine dosing for electroconvulsive therapy (ECT): actual versus ideal.

This report compares the actual doses of methohexital and succinylcholine used for optimal anesthesia and muscle relaxation in electroconvulsive therapy with written guidelines for dosing. The initial doses of methohexital and succinylcholine in milligrams per kilogram were reviewed and compared with subsequent doses of each agent after adjustments were made for individual patient responses during treatment. The dose of methohexital required to induce general anesthesia for most patients is 1.0 mg/kg. The dose of succinylcholine required to provide adequate muscle relaxation during electroconv…

[Tako-Tsubo syndrome after anaphylaxis caused by succinylcholine during general anaesthesia].

The Tako-Tsubo syndrome (or transient left ventricular apical balloning) is a new clinical entity, very similar to acute myocardial infarction, but different by its excellent short-term prognosis. It has been reported after a physical or an emotional stress, and it is diagnosed by a coronary angiogram and a left ventriculography. We report here a case of Tako-Tsubo syndrome related to an anaphylactic shock caused by succinylcholine during general anaesthesia of a female patient, wearing an unadjustable gastric band.
