Treatment of progressive supranuclear palsy with methysergide. A clinical study.

We present a clinical study of a patient aged 69 years with a clinical history of severe difficulty in walking and voluntary conjugate eye movement reduction in sideways glance and absence in vertical gaze. These symptoms led to diagnosis of Parkinson disease and treatment with L-Dopa + anticholinergics drugs. The treatment with antiparkinsonian drugs was suspended and no change in his clinical condition resulted. Methysergide therapy was initiated and the patient's response was monitored by video recording. On the basis of our experience and the data reported in the literature we believe that

Longitudinal cognitive outcomes in two progressive supranuclear palsy clinical trials.

INTRODUCTION: Progressive supranuclear palsy (PSP) causes executive dysfunction, fluency deficits, and behavioral changes. We examined longitudinal changes in PSP cognition using the Repeatable Battery for the Assessment of Neuropsychological Status (RBANS). METHODS: Using the RBANS and executive function and fluency tests from 486 and 377 participants in the PASSPORT (NCT03068468) and tilavonemab (NCT02985879) clinical trials, we assessed linear mixed models of cognitive subtest score change, controlling for disease duration, age, sex, and treatment group. RESULTS: The greatest declines occ

Functional recovery strategies in progressive supranuclear palsy with cerebellar predominance.

This report details a case of a patient in his 60s who exhibited ataxic gait, frequent falls, limb incoordination, tremors and rigidity, leading to a diagnosis of progressive supranuclear palsy with cerebellar predominance (PSP-C). Clinical assessment indicated slurred speech, increased muscle tone, deep tendon reflexes that were mildly reduced asymmetrically on the left side and reduced strength, while sensations remained intact. PSP-C is an uncommon variant of PSP often misidentified as multiple system atrophy-cerebellar type (MSA-C) because of shared characteristics. The patient participate

Richardson Syndrome Variant of Progressive Supranuclear Palsy: A Case Report.

Progressive supranuclear palsy (PSP) is a chronic neurodegenerative disease, an atypical parkinsonism with four-repeat tau neuropathology. PSP-Richardson syndrome (PSP-RS) is the most prevalent clinical variant of PSP, originally described as the prototypical form of the disease. It is defined by vertical supranuclear gaze palsy (most prominently affecting downward gaze), early postural instability with frequent falls, and a subcortical-frontal pattern of cognitive impairment. The diagnosis of PSP is mainly based on clinical data and is only definitively confirmed at autopsy. Neuroimaging play

Does Midbrain Atrophy Distinguish Progressive Supranuclear Palsy from Frontotemporal Dementia?

BACKGROUND: The diagnostic value of midbrain atrophy for distinguishing behavioral variant frontotemporal dementia (bvFTD) from progressive supranuclear palsy (PSP) is unclear. OBJECTIVE: To investigate whether measures of midbrain atrophy differentiate PSP from bvFTD. METHODS: We included four groups: healthy controls (n = 19), PSP-Richardson syndrome (n = 20), bvFTD (n = 19) and Parkinson's disease (PD; n = 12). The following quantitative and qualitative measures were calculated: Hummingbird sign rating scale [HBS-RS], global midbrain atrophy [GMA], midbrain area, midbrain/pons ratio, the

The potential significance of hepcidin evaluation in progressive supranuclear palsy.

INTRODUCTION: Hepcidin is a peptide associated with controlling the distribution of iron in tissues. Growing interest is linked with its impact on neurodegenerative diseases, as disruption of the iron regulation may be considered an initiatory element of pathological protein accumulation. The possible impact of hepcidin was not previously sufficiently explored in progressive supranuclear palsy (PSP). METHODS: Twelve patients with PSP-Richardson's syndrome (PSP-RS), 12 with PSP-Parkinsonism Predominant (PSP-P), and 12 controls were examined using Unified Parkinson's Disease Rating Scale-III pa

Functional connectivity abnormalities in clinical variants of progressive supranuclear palsy.

Progressive supranuclear palsy (PSP) can present with different clinical variants which show distinct, but partially overlapping, patterns of neurodegeneration and tau deposition in a network of regions including cerebellar dentate, superior cerebellar peduncle, midbrain, thalamus, basal ganglia, and frontal lobe. We sought to determine whether disruptions in functional connectivity within this PSP network measured using resting-state functional MRI (rs-fMRI) differed between PSP-Richardson's syndrome (PSP-RS) and the cortical and subcortical clinical variants of PSP. Structural MRI and rs-fMR

Frequent and Unexplained Falls: A Case of Progressive Supranuclear Palsy.

Progressive supranuclear palsy (PSP) is the most common neurodegenerative form of atypical parkinsonism. Although its prevalence has increased recently, it remains underdiagnosed. PSP is characterized by parkinsonism, downward gaze disorder, and a tendency to fall due to degeneration of the basal ganglia, brain stem, and cerebellum. Various clinical presentations have been linked to this entity, often making its diagnosis difficult, which can take three to four years to be established. PSP diagnosis mainly relies on clinical data and can only be confirmed with the neuropathological findings at

Correlation between clinical and neuropathological subtypes of progressive supranuclear palsy.

INTRODUCTION: Progressive supranuclear palsy (PSP) is characterized by pathology prominently in the basal ganglia, the tegmentum of the brainstem, and the frontal cortex. However, pathology varies according to clinical features. This study aimed to statistically verify the correspondence between the clinical and pathological subtypes of PSP. METHODS: We identified patients with a pathological diagnosis of PSP and classified the eight clinical subtypes of the Movement Disorders Society criteria for the clinical diagnosis of PSP (MDS-PSP criteria) into the Richardson, Akinesia, and Cognitive gr

A Modified Progressive Supranuclear Palsy Rating Scale for Virtual Assessments.

BACKGROUND: The reliability of the Progressive Supranuclear Palsy Rating Scale (PSPRS) using teleneurology has not been assessed. OBJECTIVES: To test whether removing items inadequately assessed by video would impact measurement of PSP severity and progression. METHODS: We performed secondary analyses of two data sets: the phase 2/3 trial of Davunetide in PSP and a large single-center cohort. We examined two modifications of the PSPRS: (1) removing neck rigidity, limb rigidity, and postural stability (25 items; mPSPRS-25) and (2) also removing three ocular motor items and limb dystonia (21 i

Motor Speech Disorders and Communication Limitations in Progressive Supranuclear Palsy.

Purpose This study describes motor speech disorders and associated communication limitations in six variants of progressive supranuclear palsy (PSP). Method The presence, nature, and severity of dysarthria and apraxia of speech (AOS) were documented, along with scores on the Apraxia of Speech Rating Scale-Version 3 (ASRS-3) for 77 (40 male and 37 female) patients with PSP. Clinician-estimated and patient-estimated communication limitations were rated using the Motor Speech Disorders Severity Rating (MSDSR) Scale and the Communicative Effectiveness Survey (CES), respectively. Depression and Apathy across Different Variants of Progressive Supranuclear Palsy.

BACKGROUND: Apathy and depression commonly occur in patients with progressive supranuclear palsy (PSP)-Richardson's syndrome variant; depression often requiring treatment. Little is known, however, about apathy and depression among other PSP variants. METHODS: We prospectively studied 97 newly diagnosed PSP patients. All were classified into a PSP variant using the 2017 Movement Disorder Society-PSP criteria and administered the Geriatric Depression and Apathy Evaluation Scales. Differences in apathy and depression frequency and severity across six variants, and secondarily across PSP-Richard

Progressive supranuclear palsy responding to intravenous thiamine: superimposed Wernicke's encephalopathy?

Progressive supranuclear palsy (PSP) may be a risk factor for thiamine deficiency. The classic symptoms of PSP (postural instability, supranuclear vertical gaze palsy and dementia) overlap with the clinical triad of Wernicke's encephalopathy (cognitive impairment, gait problems and ocular abnormality). Therefore, superimposed thiamine deficiency in patients with PSP may aggravate the pre-existing symptoms of PSP. Here, we are reporting a 64-year-old woman having supranuclear ocular palsy, gait instability and dementia for the past 2-3 years. The patient fulfilled the diagnostic criteria of PSP

Progressive supranuclear palsy (PSP): Richardson syndrome and other PSP variants.

Phenotypic heterogeneity of progressive supranuclear palsy (PSP) has been increasingly reported in the literature and can be the source of incorrect clinical diagnosis particularly in the early stages of the disease when the classically associated symptoms of early falls and supranuclear gaze palsy may not be apparent. In addition to Richardson syndrome (RS), several atypical clinical phenotypes have been described. Advances in genetic, neuroimaging, and biochemical/molecular technologies contribute to the identification of these clinical subtypes in the context of typical PSP pathological fin
