Swallowing in motor neurone disease.

Ninety-two patients with motor neurone disease have been assessed clinically and radiologically for evidence of swallowing problems. At the time of examination, moderate or severe swallowing difficulty was present in 89% of those whose disease had presented as bulbar palsy, in 45% of those in whom the disease began many months before as progressive muscular atrophy and in 29% of those with amyotrophic lateral sclerosis. Patients with more severe swallowing symptoms appeared more likely to have abnormal findings on videofluoroscopy overall. However, not all patients with an abnormal radiologica

Swallowing problems in neuromuscular disorders.

Feeding problems in patients with neuromuscular diseases are frequently underestimated and poorly analyzed. To gain a better understanding of the most common complaints, we surveyed 451 patients and received 409 responses representing seven disorders. Difficulties in the pre-oral phase of swallowing were encountered primarily in Duchenne muscular dystrophy (DMD), limb-girdle muscular dystrophy (LGMD), facio-scapulo-humoral muscular dystrophy (FSHMD), and spinal muscular atrophy (SMA). A limitation in the ability to open the mouth was also noted in SMA. Some features are characteristic of certa

Swallowing disorders and nutritional support.

Evaluation of nutritional needs in patients with swallowing disorders should include a global assessment. This includes a nutritional assessment, a determination of the metabolic state, and a separation of the causes of nutrient deficits due to the patient's underlying disease(s) from diminished nutrient intake related to the dysphagia. Techniques for intense nutritional support are surveyed, and the complications of each are discussed.

[Supraventricular tachycardias induced by swallowing].

The authors report the case of a 34 year old woman admitted to hospital for attacks of atrial tachycardia inducing very poorly tolerated junctional tachycardia at 260 beats/min. The attacks were always induced by swallowing and could be reproduced at will. Electrophysiological studies could only be undertaken after the administration of large doses of amiodarone. An exclusively retrograde rapidly conducting paranodal accessory pathway was demonstrated with triggering of runs of reentrant tachycardia. No underlying gastro-oesophageal or cardiac disease apart from thromboembolism was found. Swal

Swallowing of central bearing plate.

While he was being fitted for complete dentures, the patient swallowed the central bearing plate that had become detached. The plate was lodged in the hypopharyngeal region of the pharynx and, with the patient under intravenous sedation, laryngoscope and biopsy forceps were used to retrieve it. Precautions that must be taken to prevent such occurrences include the proper attachment of the device components and testing for security, as well as upright positioning of the patient. The components should be explained to patients and they should be given instructions to eject immediately any part of

Assessment of the Diagnostic Yield of Multiple Rapid Swallows in Symptomatic Patients with Normal High-Resolution Esophageal Manometry.

INTRODUCTION: Multiple rapid swallows (MRS) is a provocative maneuver used to assess contractile reserve (CR) in ineffective esophageal motility, but its clinical role in symptomatic patients with normal high-resolution esophageal manometry (HREM) remains unclear. Thus, we aimed to assess the diagnostic ability of MRS for showing any diagnostic yield in these individuals. METHODS: A retrospective analysis was performed on 190 patients with normal standard HREM (single liquid swallow) examinations. Patients were categorized as reflux symptom patients (RSP) or obstructive symptom patients (OSP)…

Use of Transfer Learning for the Automated Segmentation and Detection of Swallows via Digital Cervical Auscultation in Children.

Digital cervical auscultation (CA) has high diagnostic test accuracy in the detection of aspiration in children. However, the clinical appli … her automated machine learning using a transfer learning approach can accurately detect and segment swallows from digital CA recordings in children. Swallow sounds were collected from 16 typically developing children, median age 18 months (range 4-35 months, 50% males); and 19 videofluoroscopic swallow studies of children with pediatric feeding disorders, …

Evaluating Anterior and Posterior Oral Tongue Contributions to Pressure Generation: A Comparison Between Regular and Effortful Saliva Swallows.

PURPOSE: This study determined the contributions of the anterior and posterior tongue regions in tongue pressure generation during regular saliva swallows (SSs) and effortful swallows (ESs) completed under two different instructions. The association between tongue pressure and perceived effort to swallow was also examined. METHOD: Forty healthy adults without swallowing disorders participated in this study, divided into two age groups: 20 younger (Mage = 21.95 years, SD = 4.43) and 20 older (Mage = 70.10 years, SD = 4.30). Simultaneous data acquisition involved submental surface electromyogra…

When Swallowing Triggers VT.

We describe a case of swallowing-induced ventricular tachycardia in a young, otherwise healthy male with a structurally normal heart which was refractory to multiple anti-arrhythmic medication and was successfully treated with ablation of a cardiac ganglion in the crux of the heart. We use this case to describe an unusual disease presentation and highlight current evidence related to evaluation and management of ventricular arrhythmias and add to the limited body of evidence related to swallowing induced tachycardia.

Dysphagia, Swallowing Disorders, and Cough.

This article explores the relationship between cough and dysphagia. Both are nonspecific symptoms that arise from a variety of heterogenous and frequently multifactorial etiologies. We specifically examine the physiology of cough in relation to swallowing and summarize pertinent existing literature in disorders and diseases of aspiration, cervical diverticula, esophageal motility, eosinophilic esophagitis, and age-related changes in swallowing physiology. Treatment options for specific swallowing disorders that can result in cough are discussed, although robust data for cough outcomes are limi

Syncope: A tough pill to swallow.

Syncope is a common clinical syndrome that is typified by loss of consciousness and has several etiologies ranging from benign to potentially life-threatening. To differentiate syncope from other conditions, a detailed history is crucial. Several key historical components that aid in establishing the etiology of syncope include frequency, identifiable triggers, and prodromal symptoms. The differential diagnosis of syncope includes mechanical fall, seizure, narcolepsy, intoxication, and psychiatric conditions. Meticulous review of a patient's past medical conditions and medication list must be

The importance of taste on swallowing function.

The world's population is aging. Pneumonia is the leading cause of death among the older adults, with aspiration pneumonia being particularly common. Aspiration pneumonia is caused by a decline in swallowing function. Causes can include age-related sarcopenia of swallowing muscles, cognitive decline, cerebrovascular and other diseases or even changes in individual taste preference. Currently, the main treatment approach for dysphagia is resistance training of swallowing-related muscles. This approach has not been effective and establishment of novel methods are required. In this review, we int

A Device that Models Human Swallowing.

The pharynx is critical for correct swallowing, facilitating the transport of both air and food transport in a highly coordinated manner, and aberrant co-ordination causes swallowing disorders (dysphagia). In this work, an in vitro model of swallowing was designed to investigate the role of rheology in swallowing and for use as a pre-clinical tool for simulation of different routes to dysphagia. The model is based on the geometry of the human pharynx. Manometry is used for pressure measurements and ultrasonic analysis is performed to analyze the flow profiles and determine shear rate in the bo

Accidental Swallowing of a Molar Band.

Accidental ingestion or aspiration of foreign bodies is considered as a medical emergency in dentistry. Despite their rare occurrence, accidental ingestions are associated with various complications and morbidity, thereby necessitating prevention of their incidence along with early and effective management. Herein, we report a case of accidental swallowing of an orthodontic molar band in a patient with unilateral cleft lip and palate and its management.

[The physiologic and the pathologic swallowing process].

It is estimated that 5 million people in Germany currently suffer from swallowing disorders. Due to increasing life expectancy and the good outcome of very premature infants, this number is expected to increase. Swallowing normally occurs in four phases, and for each phase there are diagnostic tools to assess whether the problem is anatomic-organic or functional. A detailed case history is followed by clinical examination of the cranial nerves, the orofacial region, the swallowing reflex, and pharyngeal and esophageal transit. Attention is also paid to age-related changes in the swallowing pro
