Syncope is a common medical problem, with a frequency between 15% and 39%. In the general population, the annual number episodes are 18.1-39.7 per 1000 patients, with similar incidence between genders. The first report of the incidence of syncope is 6.2 per 1000 person-years. However, there is a significant increase in the incidence of syncope after 70 years of age with rate annual 19.5 per thousand individuals after 80 years. It presents a recurrence rate of 35% and 29% of physical injury. Among the causes of syncope, the mediated neural reflex, known as neurocardiogenic or vasovagal syncope,…

We report an 81-year-old Japanese patient with takotsubo cardiomyopathy associated with syncope with the possibility that the latter event evoked the former condition. Initial investigations revealed elevation of cardiac enzymes and electrocardiography changes consistent with acute myocardial infarction. The patient subsequently underwent cardiac catheterization that revealed left ventricular apical ballooning and decreased left ventricular ejection fraction without significant coronary artery lesions. A marked elevation in plasma norepinephrine, as a result of baroreflex unloading associated …

Syncope occurring in patients with primary dilated cardiac disease has several causes: ventricular tachycardia (VT), a major severe cause of this diagnosis, occurring however only in one third of cases. The other causes are supraventricular tachycardia, bradycardia and vagal hyperactivity. The management depends on the etiology of syncope in one hand and the severity of the cardiac disease and other comorbidities in the other hand. In 2007, a patient with life expectancy exceeding one year, without irreducible heart failure but with a known and stable altered left ventricular ejection fraction…

OBJECTIVE: To assess the epidemiology of cardiac syncope in children and evaluate the guidelines on its management.
MATERIAL AND METHODS: We analyzed the etiology to syncope and diagnostic workup in consecutive pediatric patients presenting with syncope in our emergency departments or cardiac outpatient clinics between 1997 and 2005, and who were subsequently diagnosed as having cardiac syncope.
RESULTS: A primary cardiac cause was identified in 11 syncopal patients presenting to the emergency room and 14 patients to the cardiac clinic: supraventricular tachyarrhythmia in 9, ventricular tach…

Syncope is a common presentation to the Emergency Department (ED); however, appropriate management and indications for hospitalization remain an ongoing challenge. The objective of this study was to determine if a predefined decision rule could accurately identify patients with syncope likely to have an adverse outcome or critical intervention. A prospective, observational, cohort study was conducted of consecutive ED patients aged 18 years or older presenting with syncope. A clinical decision rule was developed a priori to identify patients at risk if they met any of the following 8 criteria:…

Several disorders resemble syncope. According to the European Society of Cardiology guidelines, certain key questions should be addressed during the initial evaluation of a patient presenting syncope. The physician should be alert to important clinical features that suggest the diagnosis and the evaluation strategy. However, syncope remains a diagnostic challenge. The authors report a case of a 60-year-old man admitted to the Emergency Department (ED) complaining of recurrent syncopal attacks that had begun one month before. He had daily transient, self-limited loss of consciousness lasting fo…

Patient education, identification of possible triggers of syncope and reassurance are a central feature of the management of patients with reflex syncope. Patients should be advised as to the importance of adequate hydration and taught physical countermaneuvers to enhance cardiac venous return. These maneuvers are sufficient for most patients, however, for a small number of patients who continue to have recurrent syncopal events, pharmacological intervention may be considered. Volume expansion can be enhanced with salt and fludrocortisone. Agents from diverse pharmacological classes have been …

We surveyed anaesthetists working in North-West England and in North Wales concerning episodes of vasovagal syncope encountered in their practice. Eighty-eight anaesthetists described 109 such events occurring in either patients or their relatives and the estimated frequency of syncope was 1 in 5000 anaesthetic episodes. The patient syncopal episodes were triggered by venous cannulation in 16 instances and regional or local techniques in 20 instances. Thirty-three of the 53 patients were in the upright position when syncope occurred. Thirty-nine of the 56 relatives were male partners of female…

Neurocardiogenic syncope, alternatively called vasovagal, vasodepressor, or neurally mediated syncope, is a clinical syndrome faced by many clinicians. Its pathophysiology is complicated and not fully understood. Multiple pharmacologic therapies have been evaluated, with no clear ideal agent. Decisions regarding tilt-table testing, selection of pharmacotherapy, and assessment of drug efficacy are not straightforward. This article attempts to assess these issues.

Syncope is the abrupt loss of consciousness and postural tone that reverses without intervention. Typically, syncopal episodes in pediatric patients are brief, lasting seconds (rarely minutes), and are followed by complete recovery without residual neurological sequelae. Syncopal presentations may be dramatic and lead family members and primary care providers to suspect a malignant cardiac condition, prompting referral to a pediatric cardiologist. Significant advances in the understanding of syncope in infants, children, and adolescents have occurred in the past decade. This review emphasizes …

Head-up tilt testing has become a valuable and widely accepted diagnostic tool for evaluation of patients with vasovagal syncope. This test has afforded clinical researchers the opportunity to focus on the hemodynamic, humoral, and neural changes that accompany syncope. We review the animal and clinical studies that provide insight into the possible pathophysiological mechanisms involved in vasovagal syncope. Hemodynamic measurements in patients with vasovagal syncope suggest that a relative decrease in ventricular size and increase in cardiac contractility may be seen in many patients with va…

The definitive diagnosis of a cardiac arrhythmia as the basis for syncope is made by electrocardiographic monitoring during a syncopal episode. In the absence of this evidence, abnormalities demonstrated by an electrophysiologic study may suggest the etiology of syncope. Cardiac electrophysiologic testing in patients with recurrent syncope should probably be limited to patients with underlying cardiac disease. These patients are at a higher risk for sudden death and have a high incidence of electrophysiologic abnormalities. In particular, ventricular tachycardia may be evoked and specific ther…

BACKGROUND: Risk assessment of patients with syncope does not consider a short PR interval despite its association with increased risk of atrial fibrillation and all-cause mortality. This study aimed to explore the association between the PR interval and all-cause mortality and recurrent syncope in patients admitted to the hospital with syncope.
METHODS: We included patients with a diagnosis of syncope and an ECG recorded within 24 hours of hospital admission from the Danish Nationwide Electrocardiogram Cohort and divided patients into short (<120 ms), normal (120-200 ms), or long PR interval…

BACKGROUND: Syncope is a common medical condition. The reflex or neurally mediated syncope (NMS) is the most frequent type. The tilt table test (TTT) helps distinguish syncope from other common causes of complete loss of consciousness, such as epilepsy, define syncope subtypes and guide management. This study aimed to assess the TTT yield in patients with suspected NMS and to compare the nitroglycerin (NTG) and isoproterenol (Isuprel) provocative protocols.
METHODS: This study was a retrospective analysis of the data of 426 consecutive patients who underwent TTT at the Heart Center at King Fa…

AIMS: There are few effective therapies for vasovagal syncope (VVS). Pharmacological norepinephrine transporter (NET) inhibition increases sympathetic tone and decreases tilt-induced syncope in healthy subjects. Atomoxetine is a potent and highly selective NET inhibitor. We tested the hypothesis that atomoxetine prevents tilt-induced syncope.
METHODS AND RESULTS: Vasovagal syncope patients were given two doses of study drug [randomized to atomoxetine 40 mg (n = 27) or matched placebo (n = 29)] 12 h apart, followed by a 60-min drug-free head-up tilt table test. Beat-to-beat heart rate (HR), bl…

Migraine is a cyclic condition with attacks consisting mainly of intense headaches, sensory intolerance, and nausea or vomiting. Loss of consciousness during attacks is often attributed exclusively to a neurally mediated reflex to pain, although it may also be due to migraine's autonomic impairment, with cardiac conduction abnormalities, probably in relation to a degree of reduced sympathetic function. We report the case of a 51-year-old woman presenting episodes of syncope exclusively after vomiting during migraine attacks. A 24-hour Holter monitoring performed during a migraine attack disclo…
