Osteopathic manipulative treatment of congenital talipes equinovarus: a case report.

BACKGROUND: Treatment recommendations for congenital talipes equinovarus are focused on conservative treatments. OBJECTIVES: The aim of this paper is to present a case report of bilateral congenital talipes equinovarus treated with two short-leg serial casting in combination with osteopathic manipulative treatment. METHODS: A newborn, 12 days old, with severe bilateral congenital talipes equinovarus entered to the Department of Orthopedics at the University of Chieti, Italy. The pediatric orthopedic surgeon applied two single series of short-leg casts, at 12 and 20 days of age. The osteopath…

Comparison of Ponseti versus surgical treatment in congenital talipes equinovarus.

In the present study we compared the rate of relapse after conservative Ponseti treatment method with that of a historical cohort who underwent conventional operative treatment. From June 2002 to December 2004, 70 patients presented with 107 clubfeet and started Ponseti treatment. Of these 70 patients, 9 (15 feet) were excluded because of a teratologic deformity. Thus, 50 patients with 75 clubfeet were studied (41 [82%] boys and 9 [18%] girls). Data were compiled from the clinic assessment forms and patient notes. All cases resulting in recasting or additional operative procedures were regarde

Epidemiology of congenital idiopathic talipes equinovarus in Iowa, 1997-2005.

Congenital idiopathic talipes equinovarus (ITEV), also known as clubfoot, is a well-recognized foot deformity. To date, prevalence estimates and descriptive data reported for ITEV have varied due to differences in study methodology. Using population-based surveillance data collected by the Iowa Registry for Congenital and Inherited Disorders, we examined isolated ITEV births delivered from 1997 through 2005 and compared to live births in Iowa during the same time period. An overall prevalence was calculated for live, singleton full-term births only. Prevalence odds ratios (POR)s and 95% confid…

Modified Turco's postero-medial release for congenital talipes equino-varus.

BACKGROUND: Talipes Equino-Varus (TEV) is one of the most common congenital anomaly. It is managed by various methods, i.e., conservative & surgery. Objective of this study was to determine the efficacy of modified Turco's postero-medical release in children's having congenital TEV. METHODS: This descriptive observational study was conducted from June 2004 to June 2008 in the Orthopaedics Unit of Lady Reading Hospital (LRH) Peshawar. The inclusion criteria were children of age 6 months to 3 years of age having moderate and severe club foot. The exclusion criteria were clubfoot secondary to so…

A South Australian population-based study of congenital talipes equinovarus.

The aims of this study were to provide a population-based prevalence for congenital talipes equinovarus (CTEV), to conduct an epidemiological investigation into the risk factors for CTEV and describe associated features. The study used a retrospective case-control design of CTEV notified to the South Australian Birth Defects Register between 1986 and 1996 inclusive, linking characteristics of mother and baby from the perinatal data collection. The prevalence of isolated CTEV was 1.1/1000 total births (n = 231). Four factors were significantly associated with an increased risk of CTEV: maternal…

Relationship between acquired spastic talipes equinovarus and dystonia musculorum deformans.

Acquired spastic talipes equinovarus associated with dystonia musculorum deformans (DMD) presents orthopaedic surgeons with a difficult treatment problem. We reviewed the natural history, diagnostic evaluation, and treatment of this deformity in 12 patients. Two patients initially had the generalized form of dystonia, and 10 patients had the focal form (in which acquired spastic talipes equinovarus was the only manifestation). With time, five of these 10 developed the generalized form. A positive family history and age < 14 years at onset were prognostic of this progression.

Risk indicators for talipes equinovarus in Washington State, 1987-1989.

To identify risk indicators for talipes equinovarus, we compared 175 case children with talipes equinovarus of unknown etiology identified through the Washington State Birth Defect Registry with 1,470 control children sampled from resident live births. Infant gender, maternal smoking, death of a preterm sibling, and marital status were independently related to talipes equinovarus in an analysis based on information from birth certificate records. The findings differed by infant gender. The prevalence odds ratio for maternal smoking was 2.6 (95% confidence interval: 1.6-4.0) in males and 1.4 (9…

Management of Relapsed, Residual, and Resistant Idiopathic Congenital Talipes Equinovarus.

INTRODUCTION: Congenital talipes equinovarus (CTEV) is a common pediatric deformity, with conservative management being the primary method of management. Even with appropriate management of CTEV, relapses can still occur, and some cases may present with residual deformities or prove resistant to treatment. The cause of these conditions is multifactorial, and there exists a difference of opinion regarding the management of such cases. The study aimed to provide clinically relevant outcome data in this specific and less frequently encountered population. The relatively small sample reflects the …

Bilateral Pseudoaneurysms Following Percutaneous Achilles Tenotomy for Congenital Talipes Equinovarus.

Pseudoaneurysm following percutaneous tendoachilles (TA) tenotomy is a rare complication found in children with congenital talipes equinovarus (CTEV). It is postulated that associated aberrant vascular anatomy in combination with CTEV may be the underlying aetiology. In this case report, we describe a case of a toddler who developed bilateral pseudoaneurysms following percutaneous tendoachilles tenotomy and explore the management and outcome in relation to this. Based on this case report and a review of the literature, the consistent clinical findings of swelling and/or discolouration due to p…

Radiographic assessment of congenital talipes equinovarus: strapping versus forced dorsiflexion.

PURPOSE: To compare 2 radiographic assessment methods for congenital talipes equinovarus: strapping the ankle with tapes versus dorsiflexion with a wooden block. METHODS: Anteroposterior and lateral radiographs were taken with the ankle strapped by tapes or dorsiflexed by a wooden block. Talocalcaneal and tibiocalcaneal angles were measured and analysed. RESULTS: 20 radiographic assessments were performed on 14 patients. From the anteroposterior view, the mean talocalcaneal angle with strapping was 19.4 degrees (range, 0-34.0 degrees; standard deviation [SD], 11.2 degrees). From the lateral …
