Endoscopic Transfer of Flexor Hallucis Longus Tendon for Chronic Achilles Tendon Rupture: Technical Aspects and Short-Time Experiences.

BACKGROUND: Chronic Achilles tendon ruptures can lead to reduced power of plantar flexion in the ankle with impaired gait ability. The open 1- or 2-incision technique for flexor hallucis longus transfer has proven good functional outcome but has the disadvantage of relatively extensive surgery performed at a vulnerable location. To reduce the risk of soft tissue problems, the flexor hallucis longus transfer can be performed endoscopically. MATERIAL AND METHOD: An endoscopic technique for flexor hallucis longus transfer is presented together with the experiences from the first six patients ope

Intermediate follow-up on the double osteotomy and tendon transfer procedure for stage II posterior tibial tendon insufficiency.

26 patients with 28 pes planovalgus feet secondary to Johnson stage 2 posterior tibial tendon insufficiency were treated with flexor digitorum longus tendon transfer, lateral column lengthening, medial displacement calcaneal osteotomy, and heel cord lengthening. The mean patient age at surgery was 48.5 years. The AOFAS ankle-hindfoot scale was applied postoperatively to assess clinical outcome. Preoperative and postoperative standing radiographs of the foot and ankle were analyzed to determine radiographic correction of the pes planovalgus deformities. The mean follow-up to date is 5 years. Th…

Morphological Analysis of the Latissimus Dorsi Tendon Insertion: Implications for Optimizing Tendon Transfer Surgeries in Rotator Cuff Repair.

Latissimus dorsi (LD) tendon transfer is widely used to treat irreparable rotator cuff tears, particularly in cases with posterosuperior and anterosuperior tear patterns. We conducted a comprehensive anatomical analysis of the attachment of the LD tendon to the humerus, focusing on its morphological patterns and morphometric features, which are critical for optimizing the surgery. Dissection of 32 LD tendons in 16 Korean cadavers revealed three insertion patterns: fully combined (40.6%), partially combined (34.4%), and fully separated (25.0%). The mean width, length, and thickness of the tendo…

Tendon transfers for C8-T1 palsy of the brachial plexus in adults.

Brachial plexus palsy after C8-T1 nerve root injury is rare, but causes a loss of finger flexion and extension, which greatly limits the patient's grip function. It can benefit from nerve transfers if the diagnosis is made early. Otherwise, tendon transfers may be proposed. Transfers of the extensor carpi radialis longus and brachioradialis to the flexor digitorum profundus and the flexor pollicis longus, respectively, restores finger flexion and thumb flexion. Tenodesis of the extensor digitorum communis allows passive extension of the fingers during active wrist flexion. Translocation of the…

An Innovative Anchoring Technique for Anterior Transfer of the Tibialis Posterior Tendon.

Favorable results have been reported for tibialis posterior tendon transfers, which can effectively restore the dorsiflexion of the ankle and normal heel-to-toe gait. However, the commonly used methods for anchoring the transplanted tendon have some drawbacks. Therefore, we developed a new tendon-anchoring method to improve fixation of the transferred tendon and reduce the related complications. The new method entails tying the anchoring suture to the navicular bone instead of the button on the plantar foot to avoid wound complications. It requires no additional skin incisions or special equip…

Are tendon transfers justified in very old hand radial palsies?

There have been very few reports about results of tendon transfers carried out in very old radial palsies affecting the hand. We present two cases of radial palsy operated 52 and 30 years after the original injury, which also had after-effects in pronosupination due to elbow injuries. In the first patient, with high radial-nerve palsy, we made a triple transfer of the pronator teres to the extensor carpi radialis brevis, the flexor carpi ulnaris to the extensor digitorum communis and the palmaris longus to the extensor pollicis longus. In the second case, which had posterior interosseous nerve…

[Transfer of the posterior tibialis tendon to restore an active dorsiflexion of the foot].

After lesions of the peroneal nerve or damage of the tibialis anterior muscle a lack of active dorsiflexion leads to a drop foot deformity. Ober (1933) described a transfer of the posterior tibialis tendon to the dorsum of the foot to restore active extension of the foot. The aim of this retrospective study was to evaluate the results of this method and to compare our results with those in the literature.Between 1992 and 2004 we performed a posterior tibialis tendon transfer in 16 patients with an average age of 40 years. 10 patients suffered from complete peroneal nerve palsy, which was due t…

Simulation of distal tendon transfer of the biceps brachii and the brachialis muscles.

Rupture of the distal tendons of the biceps brachii and the brachialis often consists of a clean avulsion of the end of the tendons from their tuberosities. In most of the reattachment procedures these tendons are reinserted to the same tuberosities. The purpose of this study was to examine the kinetic activity in the upper limb when the insertion locations of the two prime elbow flexors are altered. The right upper limb was modeled as a two-bar linkage moving in the vertical plane of the scapula. Our Hill-type musculo-tendon actuation system was modeled in terms of five muscles moving in thre

Early tendon transfers in the rehabilitation of median, radial, and ulnar palsies.

Early tendon transfers car restore a balanced simplification of motor performance and provide an essential component for re-education of sensation. Recommended transfers include the extensor indicis proprius to restore thumb opposition in low median palsy, the pronator teres to restore wrist extension in radial palsy, and the multi-tailed single flexor digitorum superficialis to restore thumb-index pinch and prevent ring-little finger clawing in low ulnar palsy.

Anatomical Study of the Neurovascular in Flexor Hallucis Longus Tendon Transfers.

The transfer of the flexor hallucis longus tendon or flexor digitorum longus tendon is frequently used for the treatment of posterior tibial tendon insufficiency or chronic Achilles tendinopathy. According to several anatomical studies, harvesting the flexor hallucis longus (FHL) tendon may cause nerve injury. Sixty-eight embalmed feet were dissected and anatomically classified to define the relationship between Henry's knot and the plantar nerves. Two different configurations were identified. In Pattern 1, which was observed in 64 specimens (94.1%), the distance between the medial plantar ner
