A string of pearls.

Most complex interdisciplinary rehabilitative dental cases are associated with tooth eruption due to excessive wear or missing occlusal contacts. As the teeth erupt, the osseous tissues and dentogingival complex extrude with the tooth. Historically, prosthetic treatment demanded placing crowns on short teeth, functional crown lengthening onto root surfaces, or orthognathic surgery to impact teeth. This article will present a more current approach to the treatment of dentoalveolar extrusion.

Determination of medicinal and illicit drugs in post mortem dental hard tissues and comparison with analytical results for body fluids and hair samples.

In burnt or skeletonized bodies dental hard tissue sometimes is the only remaining specimen available. Therefore, it could be used as an alternative matrix in post mortem toxicology. Additionally, analysis of dental tissues could provide a unique retrospective window of detection. For forensic interpretation, routes and rates of incorporation of different drugs as well as physicochemical differences between tooth root, tooth crown and carious material have to be taken into account. In a pilot study, one post mortem tooth each from three drug users was analyzed for medicinal and illicit drugs. …

Stress distribution in endodontically treated and differently restored teeth with periapical lesions: a 3D FEA study.

BACKGROUND: The mechanical performance of post materials used in endodontically treated teeth under occlusal loading is a critical determinant of long-term restorative success but remains insufficiently understood. In this study, stress distributions within teeth with periapical lesions restored using different post materials, as well as their surrounding tissues under occlusal load, were investigated using advanced engineering simulation technique. MATERIALS AND METHODS: The finite element method based engineering analysis technique was employed, and five incisor tooth models representing he

Occlusal Fissures in Equine Cheek Teeth: A Prospective Longitudinal in vivo Study.

Background: It has been suggested that fissures of the occlusal surface of equine cheek teeth may develop into crown fractures. Objectives: … cheek teeth, and time of follow-up proved to be significant risk factors for development of a cheek tooth crown fracture. Type 2 fissures showed the highest odds followed by type 1b fissures. The observed partial crown fractures demonstrated a low clinical impact whereby no tooth showed signs of development of endodontal disease.

Dens invaginatus type II: case report with 2-year radiographic follow-up.

Dens invaginatus is a malformation of teeth probably resulting from an infolding of the dental papilla during tooth development. Clinically, it is frequently characterized by unusual crown morphology. It is common to see peg- or barrel-shaped teeth. The present case describes dens invaginatus in a lateral maxillary incisor (Oehler type II) with a barrel-shaped crown of a 12-year-old female patient; endodontic treatment and adhesive restoration were performed. After two years the tooth showed no signs of pathosis.

Three-dimensional position of impacted maxillary canines: Prevalence, associated pathology and introduction to a new classification system.

Classification of impacted maxillary canines facilitates interdisciplinary communication. Cone beam computed tomography (CBCT) has proven to be superior for the localization of impacted maxillary canines compared with 2D imaging. The purpose of this study was to retrospectively classify a cohort of impacted maxillary canines, using a new developed 3D classification for impacted maxillary canines that is easy to use and does not require complex analysis of the 3D images. A retrospective cohort study was designed, containing CBCT data of 130 patients (male/female: 48/82; median age 16) with a to

Crown-root fracture of lower molar--restorative procedures.

Endodontically treated teeth may undergo crown, crown-root, or root fractures. The treatment of fractured teeth can be complicated, sometimes because of difficulties with the final restoration. This case report describes a case of crown-root fracture of a left lower first molar in which endodontic treatment and rapid orthodontic eruption of the tooth made a good prosthetic restoration possible.

Legislation and informed consent brochures for dental patients receiving amalgam restorations.

In 2008, Norway banned the use of mercury for amalgam restorations. Four states in the United States have developed Informed Consent Brochures for amalgam restorations that must be given to their dental patients. The authors describe a patient who had a large cavity in his left lower molar tooth no.18 that had to be removed by an oral surgeon. When the patient went to the oral surgeon, the surgeon told the patient that he would replace the carious tooth with a gold implant. He was not given an Informed Consent Brochure regarding dental restorative materials. The oral surgeon extracted the cari

Remaining tooth structure associated with various preparation designs for the endodontically treated maxillary second premolar.

Existing literature suggests a relationship between the amount of remaining tooth structure and the fracture resistance of the restored endodontically treated tooth. This study investigated the amount of tooth structure remaining following various tooth preparations used in the restoration of the endodontically treated maxillary second premolar. Illustrations of the maxillary second premolar in buccopalatal, mesiodistal and occlusal sections were drawn to scale. Outlines of various intra- and extracoronal preparations were superim-posed on the illustrations to reveal the amount of tooth tissue

Three-dimensional finite element analysis of the mechanical stress on root from orthodontic tooth movement by sliding mechanics.

In order to study mechanical stress on root from orthodontic tooth movement by sliding mechanics, a 3-dimensional finite element model incorporating all layers of a human mandibular dental arch with orthodontic appliance has been developed to simulate mechanical stress on root from the orthodontic tooth movement. Simulated orthodontic force of 2 N at 0, 30 and 45 degree from the horizontal axis was applied to the crown of the teeth. The finite element analysis showed when orthodontic forces were applied to the tooth, the stress was mainly concentrated at the neck of the tooth decreasing unifor

Tooth preservation of deeply destroyed teeth by forced orthodontic extrusion: A case series.

Maintaining and restoring teeth exhibiting subgingival or even subcrestal defect extensions represent a common problem in daily practice. Such teeth are often deemed "unrestorable" due to a significant hard tissue loss and defect locations violating the biologic width. In order to achieve a sufficient 2-mm-ferrule design and reestablish the biologic width, both surgical crown lengthening and orthodontic extrusion have been suggested. However, surgical crown lengthening has a negative effect on the attachment level of adjacent teeth as well as esthetic disadvantages particularly in the esthetic

Trauma-induced crown dilaceration of permanent maxillary central incisors: A case report with CBCT follow-up.

Traumatic dental injuries to primary teeth may result in developmental disturbances in the permanent successors because of the close anatomical relationship between primary tooth apices and developing permanent tooth germs. Injuries occurring during crown formation may interfere with normal odontogenesis and lead to crown dilaceration. A 7-year-old healthy boy presented with abnormal eruption of the permanent maxillary central incisors. At 2.5 years of age, he had sustained avulsion of both primary maxillary central incisors and crown fractures of the primary lateral incisors, followed by extr

Tooth movements adjacent to single-implant restorations after more than 15 years of follow-up.

PURPOSE: To analyze the prevalence and magnitude of tooth movements adjacent to single-implant crowns in a long-term study, and to discuss these changes in relation to changes in cephalometric measurements of a reference group after 10 years. MATERIALS AND METHODS: Twenty-five of an original group of 39 patients, consecutively restored with single-implant crowns in the anterior maxilla, were included. The mean age was 25.4 years (SD: 10.0) at inclusion. Clinical photographs were taken at implant crown placement and at the final examination an average of 15.9 years (SD: 0.74) later. The refere

Restoring the unrestorable! Developing coronal tooth tissue with a minimally invasive surgical extrusion technique.

Surgical extrusion is a recognised treatment option for teeth that have insufficient coronal tooth structure remaining due to deep caries, resorption or traumatic injury. However, the technique has not been widely adopted, arguably because extraction of a severely compromised tooth may be difficult to achieve in a gentle and predictable way. In this paper, we present our novel approach to surgical extrusion and subsequent management of teeth using a vertical extraction system (Benex), which has become the method of choice in the authors' practice for many teeth that would otherwise be deemed u

Ethnic differences in the root to crown ratios of the permanent dentition.

OBJECTIVE: Root resorption due to orthodontic tooth movement may adversely affect the root-crown (R/C) ratios of permanent teeth, especially in patients with Short Root Anomaly (SRA), a poorly understood disorder affecting root development. Evaluation of SRA R/C ratios to normal dentition would facilitate diagnosis and orthodontic treatment planning. However, reference values are not available for all ethnicities. Our goal was to determine R/C ratios of permanent teeth and their relationship to gender and ethnicity. SETTING/SAMPLE: A retrospective study of 333 patients (109 Caucasians, 112 Af

Multiple Geminated Supernumerary Premolars: A Rare Case Report.

Supernumerary teeth may be defined as any teeth or tooth substance in excess of the usual configuration of 20 deciduous and 32 permanent teeth. Gemination is defined as an attempt by a single tooth bud to divide, with a resultant formation of either a large tooth with a bifid crown or two completely divided teeth throughout the crown and root. Geminated supernumerary premolar is a rarity and the possibility of multiple occurrences is even rarer. An exhaustive review of English literature and a PubMed search conducted using the terms "gemination" and "multiple geminated supernumerary" revealed

One case of complicated crown root fracture of upper anterior teeth managed by multidisciplinary joint approaches.

Complicated crown root fracture is a serious combined fracture of the enamel, dentin, and cementum in dental trauma. The treatment method is complicated. During the procedure, the condition of pulp, periodontal, and tooth body should be thoroughly evaluated, and a multidisciplinary approach combined with sequential treatment is recommended. This case reported the different treatment and repair processes of one case of two affected teeth after complicated crown root fracture of upper anterior teeth, including regrafting of broken crown after flap surgery at the first visit, direct resin repair
