Should anti-inhibitor coagulant complex and tranexamic acid be used concomitantly?

Inhibitor development in haemophilia patients is challenging especially when undergoing surgical procedures. The development of an inhibitor precludes using factor VIII (FVIII) therapy thereby requiring a bypassing agent (BPA) for surgical bleeding prophylaxis if the FVIII inhibitor titre >5 BU. Concomitant use of anti-inhibitor coagulant complex (AICC) and tranexamic acid has been reported in the literature as a beneficial treatment for this population. Anti-inhibitor coagulant complex is known to cause an increase in thrombin generation and tranexamic acid inhibits fibrinolysis. Hence, the c…

Population pharmacokinetics of tranexamic acid in paediatric patients undergoing craniosynostosis surgery.

BACKGROUND: Tranexamic acid (TXA) effectively reduces blood loss and transfusion requirements during craniofacial surgery. The pharmacokinetics of TXA have not been fully characterized in paediatric patients and dosing regimens remain diverse in practice. A mixed-effects population analysis would characterize patient variability and guide dosing practices. OBJECTIVE: The objective of this study was to conduct a population pharmacokinetic analysis and develop a model to predict an effective TXA dosing regimen for children with craniosynostosis undergoing cranial remodelling procedures. METHOD…

The Impact of Tranexamic Acid Administration Route on Outcomes in Reduction Mammaplasty.

BACKGROUND: Postoperative hematoma requiring unplanned intervention remains a clinically significant complication following reduction mammaplasty. Tranexamic acid (TXA) is increasingly used to reduce its occurrence and perioperative bleeding, but the optimal route of administration in breast reduction surgery remains unclear. OBJECTIVES: This study aimed to evaluate the effects of different TXA administration routes on hematoma incidence and perioperative outcomes in patients undergoing reduction mammaplasty. METHODS: We conducted a retrospective cohort study of consecutive patients who unde…

Safety and Efficacy of Tranexamic Acid in Liposuction: A Review of Current Literature.

BACKGROUND: Tranexamic acid (TXA) is a synthetic lysine analogue that competitively binds to the activation site of plasminogen, inhibiting its conversion to active plasmin, leading to decreased degradation of fibrin clots by plasmin. The use of TXA has been shown to be an effective and safe adjunct for reducing perioperative bleeding in many surgical subspecialties but has only recently started to gain popularity within plastic surgery. This study evaluates the current data regarding the route of administration, dosage, safety, and efficacy of TXA specifically in liposuction procedures. METH…

The Effects of Tranexamic Acid Injections on Surgical Scars: A Retrospective Study.

BACKGROUND: Tranexamic Acid(TXA) has been shown to improve melasma treatment or help homeostatic in the past decade, but no report on improving surgical scars. Given that early intervention may enhance surgical scar outcomes, this split-scar study investigates whether immediate postoperative administration of TXA around the incision site can help prevent scar formation. METHODS: In this prospective trial, 30 consecutive patients who underwent surgery from January 2022 to January 2024 were enrolled. TXA was injected randomly into half of each surgical wound closure immediately after surgery by…

Intradermal tranexamic acid and melasma: a systematic review and level of evidence.

BACKGROUND: Melasma is a common acquired pigmentary disorder characterized by symmetrical hyperpigmentation, mainly affecting sun-exposed areas of the face and neck. While not medically severe, melasma significantly impacts the quality of life due to cosmetic and psychological concerns. This systematic review evaluates the efficacy and safety of intradermal tranexamic acid (ID TXA) at varying dosages for melasma treatment, comparing it with established therapies such as hydroquinone (HQ), laser treatments, and oral TXA. METHODS: A search of the literature was performed using the electronic da…

Insight Into the Cellular Activities of Tranexamic Acid as an Option for Melasma Treatment.

Melasma and other hyperpigmentation disorders represent a significant challenge in dermatology. Tranexamic acid (TXA), traditionally known for its antifibrinolytic activity via inhibition of plasminogen activation, has recently emerged as a promising depigmenting agent. However, its cellular effects on skin cells involved in melanogenesis remain poorly understood. In this study, we investigated TXA's impact on melanocytes, keratinocytes, and inflammatory cells. TXA directly inhibited melanin synthesis in B16 melanoma cells, with an additive effect when combined with α-arbutin. In keratinocytes…

The Use of Tranexamic Acid and Microneedling in the Treatment of Melasma: A Systematic Review.

Melasma is a chronic pigmentary disorder that results in hyperpigmented patches in sun-exposed areas. Tranexamic acid (TXA) and microneedling are potential treatment options for individuals with melasma. The objective of our systematic review was to review 12 randomized controlled trials and clinical trials on the use and efficacy of TXA with microneedling for melasma. The combination of TXA and microneedling was found to be more effective at improving melasma lesions than either treatment alone; TXA alone was equally effective at reducing melasma lesions vs the standard treatment of hydroquin…

The effect of prophylactic tranexamic acid on the incidence of postoperative hemorrhage in greyhounds.

To investigate whether the incidence of postoperative hemorrhage in greyhounds was reduced when a standardized protocol for prophylactic tranexamic acid (TXA) administration to greyhounds undergoing surgery was followed, a retrospective clinical study at a private referral and first opinion hospital group was performed. Patient records of client-owned greyhounds undergoing elective surgery or dental procedures involving extractions were examined retrospectively, and 58 incidents of surgery considered eligible were documented, along with any subsequent reports of hemorrhage and whether the TXA …

Subcutaneous tranexamic acid for bleeding associated with a mycotic aortic aneurysm.

We describe the case of a 64-year-old woman with haemoptysis due to a mycotic thoracic aneurysm, with probable fistulation into the lung and oesophagus. Continuous subcutaneous tranexamic acid was used at the end of life to minimise bleeding associated with this, once the oral route was lost. A 1.5 g of tranexamic acid was administered, diluted with 23 mL water for injection in a 30 mL syringe, as a continuous subcutaneous infusion over 24 hours. Bleeding ceased rapidly following administration. There was no further bleeding in the last days before death and no site reaction noted. This case r…

Tranexamic acid in melasma: A focused review on drug administration routes.

BACKGROUND: Melasma is a disorder of hyperpigmentation and vascularization often found in women between the ages of 20 and 40. The pathogenesis is unknown, but melasma often occurs in sun-exposed areas of the face, forearms, and back. Risk factors include family history, increased estrogen/progesterone, certain medications, and UV exposure. Melasma is typically treated with topical hydroquinone (HQ); however, it is often refractory to treatment. Tranexamic acid (TXA) is a plasmin inhibitor used off-label in the treatment of melasma. TXA can be administered orally, topically, or intralesionally…

Tranexamic Acid and the Risk of Delirium after Off-Pump Surgery.

We investigated whether in patients undergoing off-pump coronary artery bypass grafting surgery a single bolus of 1 g tranexamic acid (TXA) impacts the risk of postoperative delirium using the propensity score matching approach. In 2,757 pairs, the risk of delirium was 4.2% (TXA group) and 5.0% (non-TXA group), with a relative risk in the TXA versus the non-TXA group of 0.83 (95% confidence interval: 0.65-1.07; p = 0.16). There was no significant interaction between TXA administration and renal function on the risk of delirium (p = 0.12). Data indicate that a single bolus of 1 g TXA does not i…

Tranexamic acid use in the setting of ACE inhibitor induced angioedema.

Tranexamic acid (TXA) is an antifibrinolytic agent which reduces bradykinin production through its blockade of the conversion of plasminogen to plasmin and subsequently pre-kallikrein to kallikrein. It has been utilized in angiotensin converting enzyme (ACE) inhibitor-induced angioedema (ACEi-AE) in small case reports and series however overall data is limited. This report describes a patient who historically received TXA for ACEi-AE and represented with ACEi-AE after an accidental exposure and was successfully treated with TXA again. This case suggests TXA may be a safe and effective treatmen…

Tranexamic Acid in Craniosynostosis Surgery: A Systematic Review and Meta-Analysis.

Blood loss is a potential cause of morbidity and mortality in craniosynostosis surgery. Recent reports have suggested that the use of tranexamic acid (TXA), an antifibrinolytic agent, mitigates this blood loss. A comprehensive systematic review and subsequent meta-analysis was undertaken, with the view to clarify the effectiveness of TXA in reducing blood loss and transfusion requirements in craniosynostosis surgery. Medline and PubMed databases were searched using the preferred reporting items for systematic reviews and meta-analyses technique, and 7003 articles were assessed based on predefi…

Cytotoxicity of tranexamic acid to tendon and bone in vitro: Is there a safe dosage?

INTRODUCTION: Tranexamic acid (TXA) has been shown to be effective at reducing peri-operative blood loss and haemarthrosis in arthroplasty and arthroscopic soft tissue reconstructions. Intra-articular application, as an injection or peri-articular wash, is becoming increasingly common. Recent studies have shown TXA has the potential to be cytotoxic to cartilage, but its effects on human tendon and bone remain poorly understood. The aim of this study was to investigate whether TXA has any detrimental effects on tendon-derived cells and osteoblast-like cells and determine whether there is a safe…

A role for nebulized tranexamic acid in veno-venous ECMO patients.

WHAT IS KNOWN AND OBJECTIVE: Although pulmonary haemorrhage as a complication of ECMO has been well documented, optimal management is not fully elucidated. We describe the role of nebulized tranexamic acid as a therapeutic alternative. CASE SUMMARY: We report a case series of three patients with ARDS on ECMO complicated by pulmonary haemorrhage. These patients were treated with 500 mg of nebulized tranexamic acid via the endotracheal tube. Key observations included significant stabilization of haemodynamics, reduced circuit changes and less time off of anticoagulation. WHAT IS NEW AND CONCLU…
