BACKGROUND AND OBJECTIVES: ABO-incompatible red blood cell (RBC) transfusions are a major risk in transfusion medicine. Identification of factors leading to this hazard is important to improve transfusion safety.
MATERIAL AND METHODS: All consecutive erroneous ABO-incompatible transfusions occurring from January 1997 to December 2004 at the Charité University Hospital in Berlin, Germany were analysed.
RESULTS: A total of 343,432 RBC units were transfused, and eight patients erroneously received 13 ABO-incompatible RBC concentrates. The most frequent error was incorrect bedside testing (n=7).…

Most discussions of blood transfusion cover risks and side effects, while the daily benefits of donor systems and transfusion concepts for m … sed by human error. Minimizing transfusion risks is not only the task of the doctors specialized in transfusion medicine, but can only be achieved by close cooperation between all the persons associated with each step, from motivating unpaid blood donors, evaluation of donors, phlebotomy, viral testing, preparation of components, storage, and transportation conditions …

Neonates and prematures are among the most transfused categories of patients. Adverse reactions due to transfusions, such as transfusion-transmitted infections, can affect the rest of their lives. In this systematic review, we revised the literature concerning transfusion-transmitted infection in neonates. We reported case-reports and case-series previously published and we integrated these data with our experience at local neonatal intensive care unit. Moreover, we illustrated strategies for mitigating transfusion-transmitted infections, including donor selection and testing, pathogen inactiv

INTRODUCTION: Dramatic improvements in blood transfusion have occurred during the last two decades. Transfusion medicine services and practices in Africa remain underexplored.
METHODS: A survey of blood bank/transfusion medicine (BBTM) practices, available blood products, blood product source(s), pre-transfusion testing, and blood donor infectious disease testing methodologies across Africa was performed using the American Society for Clinical Pathology (ASCP) listserv. Survey recipients included hospital-based laboratories/blood banks, national transfusion medicine services, and free-standin…

Transfusion medicine resembles all of medicine in that expert opinion predominates because hard data on clinical outcomes from randomized controlled trials and high quality observational data are simply unavailable. Indeed, some of the first trials evaluating important outcomes are barely two decades old. Patient blood management (PBM) depends on high quality data for assisting clinicians in making clinical decisions. In this review, we focus on several red blood cell (RBC) transfusion practices that new data suggest need reconsideration. The practices that may need revision include transfusio…

Transfusion and apheresis science : official journal of the World Apheresis Association : official journal of the European Society for Haemapheresis2026eng
Transfusion medicine is a high-stakes discipline where laboratory professionals play critical roles to ensure safe and appropriate blood product use. Five papers, from Argentina, Australia, Japan, Thailand, and the United States, report how blood bank and transfusion laboratory professionals (hemotherapy technicians, laboratory scientists, medical technologists) are educated, credentialed, and integrated into clinical practice. Common themes include: (1) the need for standardized, accessible foundational education; (2) the evolution of technical roles toward professionalization and leadership;…

Despite evidence-based guidelines to inform platelet transfusion practice, unnecessary platelet transfusion persists. We performed a multicentre retrospective analysis of adults admitted to general medicine, subspecialty medicine and critical care from 1 January 2017 to 30 June 2022. Platelet transfusion guideline compliance was defined as platelet transfusion below 100 × 109/L for neurosurgical, cardiac surgery or extracorporeal membrane oxygenation indications, below 50 × 109/L for invasive procedures, bleeding or therapeutic anticoagulation, and below 10 × 109/L if the patient did not have

BACKGROUND: It is increasingly difficult to maintain a hospital-based blood donor center. Cost pressures, shifting healthcare priorities, and a dwindling donor base have shifted blood collection toward larger economies of scale achieved by larger blood suppliers. Many transfusion medicine fellowships are within healthcare systems without a dedicated donor center. Yet, the management of blood donors and a blood product collection program is essential knowledge for transfusion medicine professionals. We describe an approach to teaching blood donor center operations and management in a fellowship…

In the vein-to-vein flow of blood from donor to patient, the role of the transfusion medicine specialist has become increasingly centered at the bedside. Three clinically centered issues in blood safety and in blood conservation are presented in this chapter. In Section I, Dr. Patricia Hewitt presents the epidemiologic and clinical evidence regarding new variant Creutzfeldt-Jakob disease (nvCJD) in the UK and its relevance to transfusion medicine. Lessons learned from the responses by the National Blood Service to this crisis are discussed, particularly in the context of recent evidence of a c…

Transfusion and apheresis science : official journal of the World Apheresis Association : official journal of the European Society for Haemapheresis2026eng
Apheresis, the selective removal or collection of blood components, has become an essential pillar of modern transfusion medicine, serving both donor and therapeutic purposes. While high-income countries have achieved wide adoption of automated apheresis for component collection and disease management, its use in Africa, including Ethiopia, remains limited. In Ethiopia, apheresis services are confined to a few facilities within the Ethiopian Blood and Tissue Bank Service (EBTBS) and select private institutions. Key challenges include inadequate infrastructure, unreliable supply chains, a short…

Karl Landsteiner (1868-1943) was an Austrian-American biologist, physician, and immunologist whose groundbreaking discoveries revolutionized the fields of transfusion medicine, immunology, and virology. His most famous work was the identification of the ABO blood group system in 1901, which explained the causes of transfusion reactions and laid the foundation for safe blood transfusions. This discovery earned him the Nobel Prize in Physiology or Medicine in 1930. Landsteiner continued his research, identifying the MN and P blood group system in 1927 and the Rh blood group system in 1940, which…

BACKGROUND: Immune hemolytic anemia poses a significant challenge in transfusion medicine, as identification of underlying alloantibodies can be masked by warm and/or cold autoantibodies. This increases the risk of transfusing incompatible blood, which can precipitate or exacerbate hemolysis. Identifying alloantibodies in the presence of autoantibodies remains difficult with standard serologic and genotypic methods, often delaying accurate diagnosis and appropriate transfusion strategies.
CASE REPORT: We describe a 63-year-old woman with autoimmune hemolytic anemia who suffered near-fatal hem…

The characterization of platelet concentrates (PCs) in transfusion medicine has been performed with different analytical methods and platelet lesions (from biochemistry to cell biology) have been documented. In routine quality assessment and validation of manufacturing processes of PCs for transfusion purposes, only basic parameters are monitored and the platelet functions are not included. However, PCs undergo several manipulations during the processing and the basic parameters do not provide sensitive analyses to properly picture out the impact of the blood component preparation and storage …

Since it was first cloned in 1985, the therapeutic potential of recombinant human erythropoietin in the neonatal hyporegenerative anaemias has been studied-the anaemia of prematurity and haemolytic disease of the newborn. Between 60% and 100% of preterm infants are transfused before three weeks of age, a large proportion receiving more than one transfusion. Blood transfusions are currently also the mainstay of treatment for the hyporegenerative anaemia encountered in neonates with Rhesus disease. Sometimes the situation is complicated by the religious beliefs of the parents. Blood transfusions

Purpose To provide evidence-based guidance on the use of platelet transfusion in people with cancer. This guideline updates and replaces the previous ASCO platelet transfusion guideline published initially in 2001. Methods ASCO convened an Expert Panel and conducted a systematic review of the medical literature published from September 1, 2014, through October 26, 2016. This review builds on two 2015 systematic reviews that were conducted by the AABB and the International Collaboration for Transfusion Medicine Guidelines. For clinical questions that were not addressed by the AABB and the Inter…

Genomics is affecting all areas of medicine. In transfusion medicine, DNA-based genotyping is being used as an alternative to serological antibody-based methods to determine blood groups for matching donor to recipient. Most antigenic polymorphisms are due to single nucleotide polymorphism changes in the respective genes, and DNA arrays that target these changes have been validated by comparison with antibody-based typing. Importantly, the ability to test for antigens for which there are no serologic reagents is a major medical advance to identify antibodies and find compatible donor units, an…

Careful assessment of risks and benefits has to precede each decision on allogeneic red blood cell (RBC) transfusion. Currently, a number of key issues in transfusion medicine are highly controversial, most importantly the influence of different transfusion thresholds on clinical outcome. The aim of this article is to review current evidence on blood transfusions, to highlight 'hot topics' with respect to efficacy, outcome and risks, and to provide the reader with transfusion guidelines. In addition, a brief synopsis of transfusion alternatives will be given. Based on up-to-date information of…

Transfusion is an inevitable event in the life of many individuals. Transfusion medicine personnel attempt to provide blood products that will result in a safe and harmless transfusion. However, this is not always possible since no laboratory test gives totally accurate and reliable results all the time and testing in routine transfusion services is devoted primarily to the identification of red blood cell problems. Thus, when patients are transfused, several possible adverse effects may occur in the transfused patient even though quality testing indicates no potential problem. These adverse e…
