Ten aging gender dysphoria patients (eight men and two women, average age 52 years) were the subjects of this study. All had presented to the Gender Identity Clinic at Case Western Reserve University requesting sex-reassignment surgery. A psychiatric-psychological profile of the patients, their sexual functioning, problems with aging, diagnostic issues, and follow-up are discussed. Utilizing the NEO Personality Inventory (NEO-PI) and the Derogatis Sexual Functioning Inventory (DSFI), 24 transvestitic fetishists (TVs) were compared with a similar clinic-evaluated group of 26 other paraphilics (OPs). Social, sexual, economic, familial, and psychological characteristics of 764 applicants for surgical gender reassignment, 479 males and 285 females, who completed the application questionnaire and were subsequently interviewed by the Gender Dysphoria Program in Palo Alto, California, are examined. While violence against sexual minorities is a well-known phenomenon, sexual homicides committed by men with cross-dressing or transsexual identity are not, because they are relatively few. We report a 23-year follow-up of the case of a transvestite, possibly transsexual, man who killed his mother by strangulation at the age of 20, and later in two separate cases strangled a female victim towards whom he felt sexual desire. He reported being sexually aroused by being strangled himself. The case raises questions concerning legislation which does not allow life-long control of criminals.

Following a brief review of the psychoanalytic literature on perversions, a case of fetishism is presented. The patient was a man who could only have an erection while wearing an apron. Nevertheless, it did not help him to reach an orgasm. He could only achieve this while having certain anxiety dreams that produced nocturnal pollutions. It is suggested that whatever the pregenital determinants of this perversion are, fetishism makes special reference to problems related to the phallic position (a concept suggested as an alternative to the notion of a developmental genetic phallic phase).
