Referred pain from myofascial trigger points in head and neck-shoulder muscles reproduces head pain features in children with chronic tension type headache.

Our aim was to describe the referred pain pattern and areas from trigger points (TrPs) in head, neck, and shoulder muscles in children with chronic tension type headache (CTTH). Fifty children (14 boys, 36 girls, mean age: 8 ± 2) with CTTH and 50 age- and sex- matched children participated. Bilateral temporalis, masseter, superior oblique, upper trapezius, sternocleidomastoid, suboccipital, and levator scapula muscles were examined for TrPs by an assessor blinded to the children's condition. TrPs were identified with palpation and considered active when local and referred pains reproduce heada

Oxygen dissociation curve inflection point during incremental exercise: a trigger for the Bohr effect.

We previously hypothesized that the inflection point of the oxygen dissociation curve (ODC) is linked to the gas exchange threshold (GET) during cardiopulmonary exercise testing. This hypothesis was supported by femoral venous blood gas data sampled during constant exercise below and above the GET, which showed that the ODC shifts rightward at the GET. What had gone unnoticed since these original observations in 1994 was that this rightward shift begins slightly earlier, precisely when the oxygen saturation crosses the ODC inflection point. To investigate this phenomenon, we analyzed the 1994

Trigger point dry needling increases masseter muscle oxygenation in patients with temporomandibular disorder.

BACKGROUND: Temporomandibular disorder (TMD) is an umbrella term encompassing various clinical complaints involving the temporomandibular joints, masticatory muscles, and/or associated orofacial structures. Myogenous TMDs are the most frequent cause of chronic orofacial pain. Musculoskeletal pain is commonly associated with myofascial trigger points (MTPs), for which dry needling (DN) is a routine treatment. OBJECTIVE: To investigate muscle oxygenation and pain immediately after DN application on an MTP in the masseter muscle of patients with myogenous TMDs. METHODOLOGY: Masseter muscle oxyg

Effectiveness of Massage Including Proximal Trigger Point Release for Plantar Fasciitis: a Case Report.

BACKGROUND: Plantar fasciitis (PF) is a common degenerative condition of the plantar fascia. Symptoms include tenderness on the plantar surface of the foot, pain on walking after inactivity, and difficulty with daily activities. Rest, non-steroidal anti-inflammatories, and manual therapies are frequently used treatments for PF. Trigger point release (TrPR) for PF has been found as a viable treatment option. OBJECTIVE: To determine the effects of massage, including proximal TrPR, for pain and functional limitations in a patient with PF. METHOD: A student massage therapist from MacEwan Univers

Spinal cord injury by direct damage during trigger point injection: a case report.

Trigger point injection (TPI) is commonly administered for myofascial pain syndrome management, but occasionally leads to complications, including bleeding, muscle hematoma, vasovagal syncope, skin infections, and pneumothorax. This report presents a case of TPI-induced iatrogenic spinal cord injury (SCI). A 59-year-old woman received TPI for myofascial pain on both thoracolumbar paraspinal muscles. She experienced an electric shock sensation throughout the lower extremities upon receiving blind TPI in the left thoracolumbar paraspinal muscle, and later complained of weakness (manual muscle te

Effects of trigger point acupuncture treatment on temporomandibular disorders: a preliminary randomized clinical trial.

We compared the effects of trigger point acupuncture with that of sham acupuncture treatments on pain and oral function in patients with temporomandibular disorders (TMDs). This 10-week study included 16 volunteers from an acupuncture school with complaints of chronic temporomandibular joint myofascial pain for at least 6 months. The participants were randomized to one of two groups, each receiving five acupuncture treatment sessions. The trigger point acupuncture group received treatment at trigger points for the same muscle, while the other acupuncture group received sham treatment on the tr

Role of greater occipital nerve blocks and trigger point injections for patients with dizziness and headache.

BACKGROUND: The trigeminocervical system is integral in cervicogenic headache. Cervicogenic headache frequently coexists with complaints of dizziness, tinnitus, nausea, imbalance, hearing complaints, and ear/eye pain. Controversy exists as to whether this constellation of symptoms may be cervically mediated. OBJECTIVES: To determine whether a wider spectrum of cervically mediated symptoms exist, and to investigate a potential role of greater occipital nerve blocks (GON) and trigger point injections (TPI) in these patients. METHODS: Retrospective review of GON/TPI performed in a tertiary oton

Trigger point evaluation in migraine patients: an indication of peripheral sensitization linked to migraine predisposition?

Although migraine is a neurovascular disorder, both scalp tenderness and referred pain have been observed in migraine patients. The present study was carried out to investigate the presence of trigger points eliciting referred pain in 98 migraine patients and in 32 healthy subjects. Trigger points were found in 92 (93.9%) migraineurs and in nine (29%) controls (P < 0.0001). The number of individual migraine trigger points varied from zero to 14 (modal number: 4), and was found to be related to both the frequency of migraine attacks (P < 0.0001), and the duration of the disease (P = 0.017). Quantification of changes in myofascial trigger point sensitivity with the pressure algometer following passive stretch.

In order to determine the relationship between trigger point sensitivity and the referred symptoms of myofascial pain, VAS ratings of referred pain intensity and pressure algometer measures of myofascial trigger point sensitivity were taken pre and post treatment of the muscle containing the trigger point with passive stretch. The results in 20 subjects, experiencing unilateral or bilateral myofascial head and neck pain, showed that myofascial trigger point sensitivity decreases in response to passive stretch as assessed by the pressure algometer, and that trigger point sensitivity and intensi

Localized Cutaneous Reaction to Chlorhexidine After Lumbar Trigger Point Injections: A Case Report.

We describe the case of a 38-year-old patient who developed a delayed localized cutaneous hypersensitivity reaction after she underwent lumbar trigger point injections (TPIs). This patient had undergone numerous TPIs in the past without adverse reactions. Notably, she had one prior similar episode; in both episodes, a larger volume of chlorhexidine was used during skin preparation. These findings suggest that the combined effects of a longer dry time and inadvertent subcutaneous chlorhexidine exposure were the likely drivers of these reactions. This case presents a unique manifestation of chlo

Bladder pain syndrome/interstitial cystitis response to nerve blocks and trigger point injections.

OBJECTIVES: Bladder pain syndrome (BPS)/interstitial cystitis (IC) is a debilitating condition characterised by bladder/pelvic pain and pressure as well as persistent or recurrent urinary symptoms in the absence of an identifiable cause. It is hypothesised that in addition to organ specific visceral hypersensitivity, contributions of the hypertonic pelvic floor, peripheral sensitisation, and central sensitisation exacerbate this condition. The aim of this paper is to investigate outcomes of treating underlying neuromuscular dysfunction and neuro-plastic mechanisms in BPS/IC patients. METHODS:

Electroacupuncture at Traditional Acupoints or Myofascial Trigger Points for Chronic Nonspecific Low Back Pain: High or Alternated Frequency? A Double-Blinded Randomized Controlled Trial.

BACKGROUND: Chronic nonspecific low back pain (cNLBP) can be effectively treated by electroacupuncture (EA) at traditional acupoints (TAPs) and myofascial trigger points (MTrPs). However, the optimal type and frequency of stimulation (alternated frequency [AF] and high frequency [HF]) remain unclear. This study aimed to explore this. METHODS: A double-blinded randomized controlled trial was conducted with four treatment groups: EA using AF at MTrPs (MTP-AF group), HF at MTrPs (MTP-HF group), AF at TAPs (TAP-AF group), and HF at TAPs (TAP-HF), each with 40 middle-aged cNLBP patients. The AF wa
