Exit site management: neonatal european vascular access team - ESPR recommendation 7.

INTRODUCTION: Neonatal skin immaturity, increased transepidermal water loss, prolonged catheter dwell times, and repeated dressing manipulation expose neonates to a high risk of catheter-related infection, skin injury, dressing failure, and device dislodgement. Standardized exit site management is therefore a fundamental component of neonatal vascular access care. This article is part of a special series of ten recommendations for the management of neonatal vascular access, developed through a structured multidisciplinary consensus process integrating literature appraisal and expert deliberati

Flushing ports of totally implantable venous access devices, and impact of the Huber point needle bevel orientation: experimental tests and numerical computation.

The use of totally implantable venous access devices developed as a medical device allowing mid- and long-term, frequent, repeated, or continuous injection of therapeutic products, by vascular, cavitary, or perineural access. The effective flushing of these devices is a central element to assure long-lasting use. Our experimental work demonstrates that directing the Huber point needle opening in the diametrically opposite direction of the implantable port exit channel increases the flushing efficiency. These results are consolidated by numerical computations, which support recommendations not

Implantable venous access device.

Procedures for long-term angio access have for many years made up a major component of vascular surgical practice. A totally implantable vascular device for chronic venous access developed in the early '80s compares favorably with other methods of angio access. We implanted 76 of these devices in 73 patients on the oncology service of Fairview General Hospital between May 1984 and May 1986. The infusion ports were in place for a total of 14,509 patient days with a 3.9% rate of catheter related sepsis and 6.5% rate of catheter occlusion. Three occlusions resolved with streptokinase. Pocket infe

Ex vivo modeling of extraluminal migration of pathogenic microbes in the presence of a hemostatic agent.

BACKGROUND: Vascular access devices, such as central venous catheters are essential for managing the treatment needs of complex patients. However, these devices are susceptible to complications including infection and postinsertion bleeding. Therefore, this study aimed to evaluate the antimicrobial potential of the potassium ferrate hemostatic disc on common vascular access device related pathogens using an ex vivo model. METHODS: This study utilized an ex vivo skin model to evaluate the antimicrobial potential of the hydrophilic polymer and compressed potassium ferrate hemostatic disc for in…

A propensity analysis of the risk of vascular complications after cardiac catheterization procedures with the use of vascular closure devices.

BACKGROUND: Complications of vascular access are one of the most common adverse events after coronary angiography and percutaneous coronary intervention (PCI) and are reported to occur in 1% to 9% of cases. There are conflicting reports of the association of vascular complications with the use of vascular closure devices (VCDs). The purpose of this study was to assess femoral arterial access-related vascular outcomes after invasive cardiology procedures with the routine use of VCDs. METHODS: A total of 12,937 consecutive patients were studied for inhospital outcomes through a prospective regi

Follow-up of 452 totally implantable vascular devices in cystic fibrosis patients.

The use and complications of totally implantable vascular access devices (TIVADs) were examined during multiple courses of antibiotics in cystic fibrosis (CF) patients. This retrospective study involved 36 CF centres. Risk factors for removal and septicaemia were sought by survival analysis of censored data. Multivariate Cox models were constructed with removal or septicaemia as the event and the characteristics of TIVADs as explanatory variables. TIVADs (n = 452) were implanted in 315 patients. The mean functional time per device was 32 +/- 25 months. Long-term complications occurred with 188…

Totally percutaneous transfemoral transcatheter aortic valve replacement via synthetic vascular grafts: a single-center case series.

OBJECTIVE: Transfemoral (TF) vascular access is the preferred route for transcatheter aortic valve replacement (TAVR) in patients with severe aortic stenosis; however, patients with prior peripheral vascular surgery are often treated via alternative, more invasive access sites. The authors aimed to assess the feasibility of valve delivery via synthetic vascular grafts in patients with aortobifemoral bypass surgery. METHODS: The authors report 3 consecutive patients with peripheral vascular disease and a history of aortobifemoral bypass graft surgery who underwent TF-TAVR via direct puncture o

Elevated Trimethylamine-N-oxide (TMAO) Is Associated with Vascular Access Dysfunction in Maintenance Hemodialysis Patients.

INTRODUCTION: Elevated levels of trimethylamine-N-oxide (TMAO), a metabolite produced by gut microbiota, have been associated with cardiovascular diseases and complications in various populations. However, its role in vascular access dysfunction in hemodialysis patients remains underexplored. This study investigates the potential relationship between TMAO levels and vascular access dysfunction in maintenance hemodialysis patients. METHODS: This study included 80 hemodialysis patients. The baseline serum TMAO levels were measured, and clinical characteristics and dialysis-related data were col

Duodenal Cannulation in Pigs (Sus scrofa) as a Drug Delivery Method.

Currently available animal models for delivery of drug capsules and pharmacokinetic testing are limited by either intersubject variability in gastric emptying time or the need to sedate animals when using targeted delivery methods of drug capsules. With the increasing development of large-molecule biologics, better in vivo models for testing the pharmacokinetics of capsule-delivered drugs are urgently needed. To this end, we made engineering modifications to an existing bovine surgical cannula device, successfully implanted this modified cannula into pigs, and delivered drug capsules directly

Drug-coated percutaneous balloon catheters.

Percutaneous transluminal angioplasty revolutionized coronary and peripheral revascularization. However, it is always accompanied by major drawbacks such as elastic recoil and neointimal hyperplasia. Percutaneous transluminal angioplasty along with bare metal stents reduced elastic recoil, but in-stent restenosis (ISR) and peripheral artery stenting remain problems. Drug-eluting stents addressed the issue of ISR, but late stent thrombosis and delayed endothelialization, along with longer dual antiplatelet therapy, are of concern. Non-stent-based delivery such as drug-coated balloons (DCBs) tha

Evaluation of a structured training program for arterial femoral sheath removal after percutaneous arterial catheter procedures by assistant personnel.

After cardiac catheterization procedures, arterial closure can be achieved by manual compression (MC), using external mechanical compression devices, or by applying vascular closure devices (VCDs) with comparable vascular access site-related complication rates. The aim of the present study was to assess vascular access site-related complications during the implementation of structured sheath removal and MC by paramedics after catheterization procedures. After an observational phase of 3 months to assess the baseline complication rate, a structured 4-level training program was implemented to tr

Effect of external infusion connection devices replacement frequency on catheter related bloodstream infection.

Timely and effective rescue of critically ill children no longer solely relies on advanced medical technology; vascular access plays a pivotal role. Best practice recommendations for nursing in vascular access are critical for ICU patients. However, clear guidelines for the maintenance of external infusion connection devices remain lacking. To address this gap, we conducted a prospective observational cohort study to examine the relationship between the number or replacement frequency of external infusion connection devices and catheter-related bloodstream infection (CRBSI). From September 202
