Atrial and ventricular function after cardioversion of atrial fibrillation.

OBJECTIVE: Previous studies on atrial recovery after cardioversion of atrial fibrillation have not taken into account new knowledge about the pathophysiology of transmitral and transtricuspid flow velocity patterns. It is possible to shed further light on this problem if atrioventricular inflow velocity, venous filling pattern, and atrioventricular annulus motion are recorded and interpreted together. DESIGN: Prospective examinations of mitral and tricuspid transvalvar flow velocities, superior caval and pulmonary venous filling, and mitral and tricuspid annulus motion were recorded using Dop

Initial treatment of ventricular fibrillation: defibrillation or drug therapy.

The belief that defibrillation of unwitnessed ventricular fibrillation frequently results in asystole, combined with perceived low survival rates, led to deviation from "standard" advanced cardiac life support (ACLS) by physicians directing paramedics in the field. In nonstandard ACLS, intubation or drug therapy preceded defibrillation. This study retrospectively compared standard and nonstandard ACLS for ventricular fibrillation. The long-term survival rates were 12.3% (7/57) and 3.6% (6/168) for the two forms of ACLS, respectively (p = 0.03). The incidence of postcountershock asystole was 35…

Hypokalemia after resuscitation from out-of-hospital ventricular fibrillation.

We studied serum electrolyte levels, blood gas values, and diuretic history in 115 patients immediately after resuscitation from out-of-hospital ventricular fibrillation. Comparisons were made with 84 patients admitted for acute myocardial infarction and with 88 ambulatory patients with coronary heart disease. Admission serum potassium values of 3.60 mEq/L or lower were found in 56 (49%) of the resuscitated patients, compared with 16 (19%) and eight (9%) of the comparison groups, respectively. In the resuscitated patients, the mean serum potassium level was 3.70 +/- 0.72 (SD) mEq/L, compared w…

Quinidine-induced ventricular flutter and fibrillation without digitalis therapy.

Three cases are described with documented ventricular flutter and fibrillation during quinidine medication without concomitant digitalis therapy. In all three patients the arrhythmia developed while they were receiving moderate doses of quinidine. Although no changes in QRS width were observed after administration of quinidine, there was marked prolongation of the Q-T interval. The mechanism of development of ventricular flutter and fibrillation in patients taking quinidine may be similar to that in patients with the Q-T prolongation syndrome.

Myocardial scar predicts monomorphic ventricular tachycardia but not polymorphic ventricular tachycardia or ventricular fibrillation in nonischemic dilated cardiomyopathy.

BACKGROUND: The relation between myocardial scar and different types of ventricular arrhythmias in patients with nonischemic dilated cardiom … 5 months, monomorphic ventricular tachycardia (VT) occurred in 18 patients (21%) and polymorphic VT/ventricular fibrillation (VF) in 10 (11%). LGE predicted monomorphic VT (log-rank, P < .001), but not polymorphic VT/VF (log-rank, P = .40). The optimal cutoff value for the extent of LGE to predict monomorphic VT was 7.2 g (area under curve 0.84). Features associated with …

Ventricular Fibrillation in a Patient with Marfan Syndrome without Severe Valvular Disease or Ventricular Dysfunction.

An increased incidence of ventricular fibrillation (VF) has been reported in patients with Marfan syndrome (MFS) compared with the general population. Although reduced left ventricular systolic function and severe valvular disease are recognized as major risk factors for ventricular arrhythmias (VAs) in MFS, we report a case of VF that developed despite the absence of these risk factors.A 22-year-old man with MFS who had previously undergone a valve-sparing aortic root replacement with the David procedure, lost consciousness and was diagnosed with VF. After resuscitation, no new abnormal findi…

The Storm Before the Calm: Ablation of Premature Ventricular Complex Trigger for Incessant Ventricular Fibrillation.

Ventricular tachycardia storm is associated with high mortality rates and is often refractory to treatment. Historically, few options for treatment have existed in cases when antiarrhythmic drugs fail. We report the case of a patient with incessant ventricular fibrillation (VF) in the postinfarction period that was triggered by premature ventricular contractions (PVCs) that persisted despite normal electrolytes, exclusion of ongoing ischemia, infusions of antiarrhythmic drugs, general anesthesia, full circulatory support with extracorporeal membranous oxygenation, and cardiac sympathetic dener…

Mechanisms underlying ventricular tachycardia and its transition to ventricular fibrillation in the structurally normal heart.

Reentrant ventricular tachycardia (VT) is the most common sustained arrhythmia leading to ventricular fibrillation (VF). However, despite more than a century of research, the mechanism(s) of the conversion from reentrant VT to VF have not been elucidated. Based on their different electrocardiographic appearance, reentrant VT and VF have traditionally been thought of as resulting from two widely different mechanisms. Whereas VT is seen as a rapid but well organized process whereby the excitation wave rotates about a single well-defined circuit, fibrillation has been described as turbulent cardi…

Ventricular fibrillation in hypertrophic cardiomyopathy is associated with increased fractionation of paced right ventricular electrograms.

BACKGROUND: Intraventricular conduction in hypertrophic cardiomyopathy (HCM) has been characterized to test the hypothesis that myofibrillar disarray will cause dispersion of activation throughout the ventricular myocardium. METHODS AND RESULTS: Of 37 patients with HCM, four had spontaneous ventricular fibrillation (VF), five had nonsustained ventricular tachycardia (VT), 13 had no risk factors, and 15 had a family history of sudden death. These patients and four controls were studied by pacing one site in the right ventricle and recording electrograms from three other right ventricular sites…

Clinical characteristics and results of electrophysiologic testing in young adults with ventricular tachycardia or ventricular fibrillation.

Thirty-one patients 16 to 40 years of age (mean +/- SD = 30.7 +/- 7 years) had one or more episodes of sustained ventricular tachycardia (VT) or ventricular fibrillation (VF). Underlying cardiac abnormalities consisted most commonly of cardiomyopathy (nine), long QT syndrome (LQTS) (five), and mitral valve prolapse (five); no identifiable heart disease was found in four patients. Programmed ventricular stimulation induced VT in only one of four patients with the LQTS but induced VT in 64% of 22 patients with other abnormalities. Chronic drug treatment was based either on serial electropharmaco…

Surviving the Shocks: A Contemporary of Short-coupled Ventricular Fibrillation.

A young man presented following successful cardiac resuscitation after an out-of-hospital cardiac arrest. During his admission, he had multiple runs of short-coupled ventricular fibrillation with a similar morphology premature ventricular complex (PVC) trigger. He was brought to the electrophysiology laboratory, and, with a high dose of isoprenaline, the PVC was localised to the moderator band. Ablation induced short runs of ventricular tachycardia before elimination of the PVC. He subsequently underwent subcutaneous implantable cardiac defibrillator implantation before his discharge.

Purkinje triggers of ventricular fibrillation in patients with hypertrophic cardiomyopathy.

INTRODUCTION: Ventricular fibrillation (VF) is the main mechanism of sudden cardiac death in patients with hypertrophic cardiomyopathy (HCM). The origin of VF and the success of catheter ablation to eliminate recurrent episodes in this population are poorly understood. METHODS AND RESULTS: From 2010 to 2014, five patients with HCM (age 21 ± 9 years, three female) underwent invasive electrophysiological studies and ablation at our center after resuscitation from recurrent (9 ± 7) episodes of VF. Ventricular premature beats (VPBs), seen to initiate VF in certain cases, were recorded noninvasive…

A case of AL amyloidosis presenting with refractory ventricular fibrillation.

A 66-year-old male with recent diagnosis of heart failure with reduced ejection fraction was referred to our institution for management of cardiogenic/vasodilatory shock. During his evaluation, he suffered a sudden cardiac arrest from refractory ventricular tachycardia/fibrillation (VT/VF) despite normal electrolytes and no evidence of prior ventricular arrhythmias. He was placed on rescue peripheral veno-arterial extracorporeal membrane oxygenation support (VA-ECMO) for 4 days and was decannulated without end-organ damage. Continued workup revealed Mayo stage IV immunoglobulin light chain (AL

Atrial Fibrillation and Ventricular Tachycardia in a Patient with Cardiac Sarcoidosis.

Cardiac sarcoidosis (CS) can cause atrial and ventricular arrhythmias, conduction system disease, and congestive heart failure. The use of advanced imaging modalities including cardiac magnetic resonance and positron emission tomography with 2-deoxy-2-[fluorine-18]fluoro-D-glucose can be helpful in evaluating the extent of disease and response to treatment. The management of CS patients can be challenging, requiring immunosuppression medications, antiarrhythmic drugs, implantable cardiac devices, and cardiac ablation procedures. We report a patient with CS initially presenting with paroxysmal

Ventricular Fibrillation in a General Population - A National Database Study.

BACKGROUND: Ventricular fibrillation (VF) is a life-threatening disease that can be remedied by prompt defibrillation. However, data regarding such risk in a general population remain limited. This general population study was to explore the epidemiological profile of VF. METHODS AND RESULTS: We investigated patients with VF younger than 60 years (average population, 19,725,031) using a national database spanning the period 2000-2010. We identified 3,971 (68.4% male) patients with VF (crude incidence rate: 1.83/100,000). Incidence rates were low in patients younger than 10 years and increased…

Ventricular fibrillation in a patient with Pompe disease: a cautionary tale.

Pompe disease is a rare genetic disorder resulting from a deficiency of the acid α-glucosidase enzyme. Although arrhythmias occur in these patients undergoing general anesthesia, they have not received sufficient emphasis in pediatric cardiology. We report a case of an infant with Pompe disease who experienced ventricular fibrillation during induction of anesthesia.
