Right ventricular function in valvular heart disease: relation to pulmonary artery pressure.

Right ventricular angiography was performed in 46 patients with acquired valvular heart disease and 8 normal subjects. Right ventricular ejection fraction (RVEF) correlated highly only with right ventricular peak systolic pressure (RVPSP) and mean pulmonary artery pressure, both in patients with and without tricuspid insufficiency. For the group, RVEF = -0.33 RVPSP + 63 (correlation coefficient [r] = -0.76, probability [p] less than 0.001). Of 20 patients with moderate or severe elevation of pulmonary artery pressure, 17 (85%) had an abnormally low ejection fraction (less than 47%), while 19 (

Therapeutic implications of diazepam in patients with elevated left ventricular filling pressure.

Diazepam (0.1 mg. per kilogram) was given intravenously to 12 patients with hemodynamic left ventricular failure at the time of cardiac catheterization. Anxiety was effectively relieved in 10 patients. Systemic and coronary hemodynamic parameters were assessed before and 5 and 15 minutes after diazepam. Heart rate, cardiac index, and left ventricular stroke-work index did not change significantly. Mean aortic pressure decreased in all patients (average of 10 mm. Hg) and left ventricular end-diastolic pressure (LVEDP) decreased from a mean +/- S.E.M. of 24.3 +/- 3 mm. Hg at rest to 16 +/- 2.1 a

Left ventricular hypertrophy, diastolic dysfunction, pulse pressure, and plasma ET-1 in marathon runners with exaggerated blood pressure response.

This study was conducted to study left ventricular hypertrophy (LVH), diastolic dysfunction, pulse pressure (PP), and plasma endothelin (ET)-1 level in amateur marathon runners with an exaggerated blood pressure response (EBPR) to exercise. The study participants included normotensive marathon runners (NM, n = 15), EBPR marathon runners (EBPR, n = 17), normotensive sedentary individuals (CON, n = 13), and hypertensive patients (HTN, n = 14). An integrated M-mode/2-dimensional echocardiographic analysis was performed. Plasma ET-1 levels at resting were measured using a commercial ELISA kit. LV

Independent prognostic information provided by sphygmomanometrically determined pulse pressure and mean arterial pressure in patients with left ventricular dysfunction.

OBJECTIVES: The purpose of this study was to evaluate the relationship of baseline pulse pressure and mean arterial pressure to mortality in patients with left ventricular dysfunction. BACKGROUND: Increased conduit vessel stiffness increases pulse pressure and pulsatile load, potentially contributing to adverse outcomes in patients with left ventricular dysfunction. METHODS: Pulse and mean arterial pressure were analyzed for their effect on mortality, adjusting for other modifiers of risk, using Cox proportional hazards regression analysis of data collected from 6,781 patients randomized int

Characterization of intracranial pressure variations in ventricular and subarachnoid spaces of the rat brain.

Intracranial pressure (ICP) is a dynamic physiological signal shaped by cerebrospinal fluid (CSF) flow, vascular pulsations, and brain tissue compliance, yet most knowledge of CSF (cerebrospinal fluid) hydrodynamics comes from pathological states in humans. To better understand physiological ICP variability, dual recordings were obtained in 15 healthy anesthetized Brown-Norway rats using pressure sensors connected to cannulas in the lateral ventricle (vICP) and subarachnoid space (sICP). Spectral analysis quantified cardiac and respiratory oscillations, whereas slower waves were detected by th

Modelling the effect of ventricular expansion on white matter in normal pressure hydrocephalus.

The mechanism by which idiopathic normal pressure hydrocephalus (iNPH) causes cognitive impairment, gait dysfunction and urinary incontinence is unclear. The key radiological feature is ventriculomegaly, and symptoms can be treated with surgery. We hypothesise that ventricular expansion causes clinical effects by stretching and/or compressing white matter tracts. We define disease-specific white matter abnormalities, explore their relationship to clinical features and test whether they can be explained by a computational model of ventricular expansion in iNPH. In this observational study, diff

Early Outcomes of Right Ventricular Pressure and Volume Overload in an Ovine Model.

Right ventricular (RV) failure is a common complication in multiple congenital heart disease (CHD), significantly increasing morbidity and mortality. Despite its impact, no therapies specifically target the failing RV. The growing population of CHD patients underscores the need to understand the pathophysiology of RV failure through preclinical research. This study aimed to develop an ovine model of RV failure induced by pressure and volume overload. A total of 14 juvenile sheep randomly underwent pulmonary artery banding (n = 6), pulmonary leaflet perforation (n = 4), and pulmonary annulotomy

Noninvasive estimation of ventricular filling pressures in patients with single right ventricles.

BACKGROUND: Increased ventricular end-diastolic pressure (VEDP) is a known risk factor for morbidity and mortality in patients with single-ventricle physiology. Spectral Doppler tissue imaging (DTI) has been shown to modestly correlate with direct measurement of VEDP. Strain rate (SR) values, obtained via myocardial deformation, have not yet been compared with VEDP in this patient population. The goal of this study was to evaluate which of these imaging techniques correlates best with VEDP in patients with single-RV (RV) physiology. METHODS: Patients with single-RV physiology who underwent si

Effect of right ventricular function and pulmonary pressures on heart failure prognosis.

The relationship of right ventricular function and pulmonary systolic pressure in patients with congestive heart failure was evaluated to risk-stratify them. The study included 147 consecutive patients with symptomatic heart failure who underwent clinical and laboratory examination and echocardiography including Doppler tissue echocardiography. They were followed for a mean of 11.2+/-6.4 months. During follow-up, 16 patients died and 45 patients had nonfatal cardiac events. There were 60 readmissions for heart failure. Pulmonary artery systolic pressure and right ventricular systolic function

Development of an inlet pressure sensor for control in a left ventricular assist device.

A Tesla type continuous flow left ventricular assist device (VAD) has been designed by Penn State and Advanced Bionics, Inc. (ABI). When a continuous flow device is used, care must be taken to limit low pressures in the ventricle, which can produce an obstruction to the inlet cannula or trigger arrhythmias. Design of an inexpensive, semiconductor strain gauge inlet pressure sensor to detect suction has been completed. The research and design analysis included finite element modeling of the sensing region. Sensitivity, step-response, temperature dependence, and hysteresis tests have been perfor

Pressure wave with apnoea evaluated by sleep level in patient with ventricular dilation.

Seventeen patients who were suspected of having hydrocephalus, because of ventricular dilation from various causes, were included in this study of the pathophysiologic basis of the appearance of pressure waves (PWs). Pressure waves accompanied by apnoeas originated in arousal responses in the resting state of these patients. Frequent fits of apnoea were included in the entire sleep apnoea syndrome. Most pressure waves characteristically appeared in the state of non-REM sleep. During the appearance of such pressure oscillations, intracranial pressure was elevated transiently. This coincidence i

Doppler assessment of the interventricular pressure drop in patients with ventricular septal defects.

Doppler ultrasound was used to assess the pressure drop between the ventricles in 109 infants and children (61 less than two years old) with a ventricular septal defect who underwent cardiac catheterisation. The pressure in both ventricles was measured at catheterisation in 103 patients either simultaneously through two catheters (41) or with a single catheter withdrawn across the septum or removed from one ventricle to the other (62). When pressure was measured simultaneously with two catheters (41 patients) the peak to peak and instantaneous gradients showed a maximum difference of 20 mm Hg

The effects of positive end-expiratory pressure on right and left ventricular performance.

The cardiovascular effects of positive end-expiratory pressure (PEEP) were studied in mechanically ventilated, vagotomized, Beta-blocked, anesthetized dogs. To compensate for the effect of PEEP on decreasing systemic venous return, acute plasma volume expansion was accomplished returning stroke volume and cardiac output to control values. Left and right ventricular filling pressures (LVFP and RVFP) and aortic pressure were measured relative to pressure (transmural pressure). Ventricular performance was assessed by comparing the transmural ventricular filling pressures at similar stroke volumes
