Hybrid Prosthesis versus Overdenture: Effect of BioHPP Prosthetic Design Rehabilitating Edentulous Mandible.

AIM: To compare the BioHPP (biocompatible high-performance polymer) as a substructure for the hybrid prosthesis versus the BioHPP bar supporting and retaining implant overdenture by radiographic evaluation to identify bone height alteration around the implants and to evaluate satisfaction based on visual analoge scale questionnaire. MATERIALS AND METHODS: Ill-fitting mandibular dentures were chosen for 14 fully edentulous male patients with adequate dental hygiene, enough interarch space, and free of systemic diseases and parafunctional habits. Patients who received new dentures (CDs) were ra

Spatial Restriction of Neural Activation Using Focused Multipolar Stimulation With a Retinal Prosthesis.

PURPOSE: The resolution provided by present state-of-the-art retinal prostheses is severely limiting for recipients, partly due to the broad spread of activation in the retina in response to monopolar (MP) electrical stimulation. Focused multipolar (FMP) stimulation has been shown to restrict neural activation in the cochlea compared to MP stimulation. We extended the FMP stimulation technique to a two-dimensional electrode array and compared its efficacy to MP and hexapolar (HP) stimulation in the retina. METHODS: Normally-sighted cats (n = 6) were implanted with a suprachoroidal electrode a

Significance of angular mismatch between vertebral endplate and prosthetic endplate in lumbar total disc replacement.

STUDY DESIGN: A retrospective study. OBJECTIVE: To determine whether angular mismatch between the vertebral endplate and prosthetic endplate during lumbar total disc replacement (L-TDR) affects the radiological and clinical outcomes. SUMMARY OF BACKGROUND DATA: A prosthesis anchored to the vertebral body by using a large central keel carries an inherent risk of angular mismatch between the vertebral endplate and prosthetic endplate at a segment with a greater degree of lordosis, such as L5-S1. Theoretically, this angular mismatch can cause several problems, such as segmental hyperlordosis, a

High early failure rate of the Columbus posterior stabilized high-flexion knee prosthesis.

BACKGROUND: Most studies report high survivorship rates for TKAs, however, we observed higher than anticipated rates of dislocation and femoral component loosening after implanting a Columbus posterior stabilized prosthesis. QUESTIONS/PURPOSE: We therefore determined (1) the incidence of dislocation and aseptic loosening that occurred after implantation of posterior stabilized high-flexion prostheses in TKAs, (2) the causative factors of dislocation and aseptic femoral component loosening when comparing two designs of prostheses, and (3) the mechanisms of dislocation. METHODS: We retrospecti

Modeling the biomechanical features affecting the metabolic rate of walking with a powered ankle-foot prosthesis.

For individuals with unilateral transtibial amputation, powered ankle-foot prostheses have the potential to reduce the metabolic rate of walking, which could contribute to improvements in mobility and quality of life; however, physiological improvements have not been consistently demonstrated in experimental studies. To improve our understanding of the biomechanical mechanisms that drive metabolic rate outcomes, we used a machine learning approach to model the relationship between multimodal biomechanical factors and the metabolic rate of walking with a powered ankle-foot prosthesis. Our model

Quality of life and cataracts: a review of patient-centered studies of cataract surgery outcomes.

In the next 10 years growing numbers of adults will be reaching the age at which senile cataracts typically develop. At the same time, pressure is strong to contain medical expenses. In order to formulate wise public policy in this area it is important to describe the effects of this treatment on the patient's quality of life. After a general discussion of research on patient-centered medical outcomes, 18 studies of patient-centered cataract surgery outcomes are reviewed. Then the findings, both direct outcomes (visual acuity, percentage visual impairment, complications, visual functioning, ph

Cost-effectiveness and cost-utility of the ball-and-socket trapeziometacarpal prosthesis compared to trapeziectomy and ligament reconstruction: study protocol for a randomized controlled clinical trial.

BACKGROUND: Trapeziometacarpal (TMC) osteoarthritis (OA) is a common cause of pain and weakness during thumb pinch leading to disability. There is no consensus about the best surgical treatment in unresponsive cases. The treatment is associated with costs and the recovery may take up to 1 year after surgery depending on the procedure. No randomized controlled trials have been conducted comparing ball and socket TMC prosthesis to trapeziectomy with ligament reconstruction. METHODS: A randomized, blinded, parallel-group superiority clinical trial comparing trapeziectomy with abductor pollicis l

From conception to application of a tele-operated assistance robot for middle ear surgery.

The authors' goal was to design and evaluate a robot dedicated to middle ear surgery. Specifications for dimensions, forces, and kinematics were collected, based on the otosclerosis procedure. The robot structure has a compact geometry with 3 linear and 3 rotatory motors. It is remotely piloted via a robot-surgeon interface under operative microscope. Ability to reach anatomical targets, to perform stapedectomy, and to place prosthesis in a model of stapedotomy was evaluated by 6 surgeons. Multiple anatomical targets in the middle ear could be successfully reached without damaging surrounding

The Bryan cervical disc prosthesis as an alternative to arthrodesis in the treatment of cervical spondylosis: 46 consecutive cases.

We present data relating to the Bryan disc arthroplasty for the treatment of cervical spondylosis in 46 patients. Patients with either radiculopathy or myelopathy had a cervical discectomy followed by implantation of a cervical disc prosthesis. Patients were reviewed at six weeks, six months and one year and assessment included three outcome measures, a visual analogue scale (VAS), the short form 36 (SF-36) and the neck disability index (NDI). The results were categorised according to a modification of Odom's criteria. Radiological evaluation, by an independent radiologist, sought evidence of

Generating Natural Neural Response in Visual Neuroprosthetics Using the Artificial Bee Colony Algorithm.

A significant challenge that hampered visual prosthetic systems is its ability to produce a meaningful perception of vision. It is hypothesised that mimicking the natural spiking pattern of the visual cortex would lead to a more natural perception of vision. In this study, an optimisation algorithm, namely the Artificial Bee Colony (ABC) algorithm, is utilised to automatically generate a stimulus waveform to produce a response that best matches a target neural pattern. An in silico model of realistic retinal ganglion cells (RGC) was used to simulate neural responses in a visual prosthesis and …

Double-flanged knotless technique for iris prosthesis and IOL implantation associated with keratoplasty.

INTRODUCTION: Anterior segment reconstruction and penetrating keratoplasty combined with iris prosthesis (IP) and intraocular lens (IOL) scleral fixation require prolonged open-sky surgery and complex suturing. Herein we present a novel double-flanged knotless surgical technique that minimizes the open sky duration and facilitates the fixation of the IP and IOL complex to the sclera. METHODS: After corneal trephination and removal, a temporary keratoprosthesis is fixated. Three 5.0 polypropylene suture segments are introduced in the anterior chamber transconjunctivally and then externalized t

[The assessment of modification of prosthesis fixation method on visceral interposition during laparoscopic abdominal hernia reconstruction].

UNLABELLED: During laparoscopic abdominal hernia repair (LAHR) there is a discrepancy in relative position of mesh and fixation points during the procedure (while abdominal cavity is distended with gas) and afterwards. Therefore, after the surgery the prosthesis becomes corrugated, and tension develops in suture anchoring points. One of the proposed solutions of this problem is to place stabilizing sutures outside the mesh edges and to tie them after emptying the abdominal cavity of CO2. However, due to lack of visualization, viscera entrapment between the mesh and integuments may occur during

Raising the stakes for cortical visual prostheses.

In this issue of the JCI, the dream of restoring useful vision to blind individuals with neurotechnology moves one step closer to realization. Fernández et al. implanted an electrode array with 96 penetrating electrodes in the visual cortex of a blind patient who had been without light perception for 16 years due to optic neuropathy. Remarkably, the patient was able to perceive visual patterns created by passing current through array electrodes. The use of a penetrating electrode array meant that action potentials from single neurons could be recorded to study the neural response to stimulatio

Vibrotactile Haptic and Gesture Feedback in a Smartwatch for Controlling a Multi-Activity Powered Knee-Ankle Prosthesis.

For powered lower-limb prostheses to be translated from research environments to real-world use, they must be able to perform a variety of daily activities, such as walking on level or ramped surfaces, stair climbing, sitting, and standing. The device must quickly and predictably switch between the modes corresponding to these activities. Multiple methods exist to trigger activity mode transitions, but they can overlook user agency, be slow and cumbersome to enact, lack discretion, or have limited predictability. This work presents a smartwatch application that allows the user to wirelessly co
