Pattern of lung volumes in patients with sighing breathing.

BACKGROUND: Sighing breathing is observed in subjects suffering from anxiety with no apparent organic disease. METHODS: Lung volumes and expiratory flow rates were measured in 12 patients with a sighing pattern of breathing and in 10 normal subjects matched for age, gender, and anthropometric data. In both groups the measurements were made by spirographic and plethysmographic techniques. In normal subjects functional residual capacity (FRC) and residual volume (RV) were measured during normal breathing and again during simulated sighing breathing to exclude technical artifacts resulting from

Maximum inspiratory muscle endurance capacity during resistive loading in chronic obstructive pulmonary disease.

In 10 patients with stable severe chronic obstructive pulmonary disease (COPD) we evaluated the relationship between the degree of airway obstruction and hyperinflation, and the maximum inspiratory muscle endurance capacity during added inspiratory resistive loading. We measured the ratio of forced expiratory volume in 1 s to forced vital capacity (FEV1/FVC) and airway resistance (Raw) as indices of airway obstruction, and the ratio of functional residual capacity to total lung capacity (FRC/TLC) as an index of hyperinflation. The mean resting transdiaphragmatic pressure to its maximum (Pdi/Pd

Pulmonary diffusing capacity disturbances are related to nailfold capillary changes in patients with Raynaud's phenomenon with and without an underlying connective tissue disease.

PURPOSE: The aim of this study was to evaluate whether pulmonary microvascular damage is part of a more generalized involvement of the microvasculature in the spectrum of scleroderma (Scl)-like syndromes. PATIENTS AND METHODS: We studied four groups of patients, all with Raynaud's phenomenon (RP), distinguished by the extent and nature of their underlying connective tissue disease. Twenty-two patients had primary RP (pRP), another 22 patients had RP and an undifferentiated connective tissue disease (uCTD), 15 patients had Scl, and eight patients had the CREST syndrome (CREST). Pulmonary vascu

Pharyngeal and glottic changes following methacholine challenge in normal subjects.

Recent evidence indicates that some normal subjects exhibit glottic narrowing following experimentally induced bronchospasm. Similar findings have been observed during episodes of bronchospasm in asthmatics. The exact mechanism of this effect is unknown but it is thought to occur as part of a generalized reflex response associated with constriction of intrapulmonary airways. We tested the hypothesis that in addition to the glottic changes, coincident with intrapulmonary airway constriction which occurred after inhalation of methacholine, the pharynx would show similar changes. Pharyngeal and g

The effect of transfusion on pulmonary function in patients with thalassemia major.

Pulmonary involvement has been documented in thalassemia major (TM). We studied 12 patients with TM before and 24 hr after transfusion to evaluate the effect of transfusion on baseline lung function. Personal and family histories of respiratory illnesses were obtained by a questionnaire. Spirometry and carbon monoxide diffusion capacity (KCO) measurements were made. Blood gases (PO2 and SO2) were determined on arterialized samples. Baseline expiratory volumes and flows were within normal range in all patients. Transfusion resulted in a significant reduction of forced expiratory volume in 1 sec

Open biopsy for chronic diffuse infiltrative lung disease: clinical, roentgenographic, and physiological correlations in 502 patients.

Clinical, physiological, roentgenographic, and histological data concerning 502 patients who had open biopsy for chronic "interstitial" lung disease were reviewed. Mortality was 0.3%, the rate of complications was 2.5%, and the diagnostic yield was 92.2%. A modified Chamberlain approach in the second interspace is preferred for easy access to all lobes and mediastinum. Brief tube drainage is mandatory. Atelectasis and hemorrhage in the specimen are prevented by avoiding palpation and clamps, by delineating the wedge during full inflation, and by instant fixation. Customary biopsies of the tip

Recovery of pulmonary diffusing capacity after maximal exercise.

Pulmonary diffusing capacity (DICO), together with spirometric variables, arterial oxygen tension (paO2) and cardiac output were determined before and at intervals after maximal arm cranking, treadmill running and ergometer rowing. Independent of the type of exercise, D1CO increased immediately post-exercise from a median 13.6 (range 7.3-16.3) to 15.1 (9.3-19.6) mmol min-1 kPa-1 (P < 0.01). However, it decreased to 11.6 (6.9-15.5) mmol min-1 kPa-1 (P < 0.01) after 24 h with cardiac output and paO2 at resting values, and D1CO normalized after 20 h. Thoracic electrical impedance at 2.5 and 100 k

Maximal shortening of inspiratory muscles: effect of training.

Normal subjects can increase their vital capacity by appropriate training. We tested whether that change can be achieved by greater maximal shortening of the inspiratory muscles without concomitant increases in peak static inspiratory pressures. Sixteen healthy volunteers participated in the study: eight were randomly assigned to make 20 inhalations to total lung capacity, held for 10 s with the glottis open, each day for 6 wk; the remainder served as nontraining controls. Before and after the 6-wk study period, we made multiple determinations of lung volumes and of curves relating lung volume

Clinical significance of tracheal deviation in patients with idiopathic pleuroparenchymal fibroelastosis.

BACKGROUND: Idiopathic pleuroparenchymal fibroelastosis (IPPFE) is a rare form of interstitial pneumonia, characterized by upper lobe-predominant fibrosis. Tracheal deviation is frequently observed in this condition; however, its clinical relevance remains unclear. This study aimed to clarify the clinical significance of tracheal deviation in patients with IPPFE. METHODS: We retrospectively analyzed 84 patients diagnosed with IPPFE at our institution between 2005 and 2024. Tracheal deviation on chest radiographs was defined as tracheal displacement beyond the midline. The distance and angle o

Derivation and validation of a noninvasive prediction tool to identify pulmonary hypertension in patients with IPF: Evolution of the model FORD.

BACKGROUND: The administration of inhaled prostanoids to patients with pulmonary hypertension (PH) related to idiopathic pulmonary fibrosis (IPF) and other fibrotic lung diseases improves functional outcomes. Selection of patients with IPF at risk for concomitant PH to undergo right heart catheterization (RHC) remains challenging. We sought to develop a clinical prediction tool based on common noninvasive parameters to identify PH in patients with IPF. METHODS: A prediction model based on noninvasive parameters was derived from patients enrolled in the ARTEMIS-IPF randomized, placebo-controll

Measure of lung dielectric proprieties in patients with Idiopathic Pulmonary Fibrosis: correlation with clinical, radiological and pulmonary functional parameters.

BACKGROUND: Dielectric properties of biological tissues are biophysical parameters; in lung they change with amount of air, blood and parenchyma. Remote Dielectric Sensing (ReDS™) technology measures dielectric properties of lung tissues quantifying the content of fluids inside the scan volume. We aimed to evaluate the reliability of ReDS™ measure in Idiopathic Pulmonary Fibrosis (IPF) patients and in healthy volunteers, and to investigate the correlation of ReDS™ score with clinical, radiological and functional parameters. METHODS: We conducted a prospective observational study, including 52

Dynamic Prediction of Pulmonary Hypertension in Systemic Sclerosis Using Landmark Analysis.

OBJECTIVE: Pulmonary hypertension (PH) is a serious complication of systemic sclerosis (SSc). In this study, we explored the prediction of short-term risk for PH using serial pulmonary function tests (PFTs) and other disease features. METHODS: SSc patients in whom disease onset occurred ≥10 years prior to data retrieval and for whom autoantibody specificity and PFT data were available were included in this study. Mixed-effects modeling was used to describe changes in PFTs over time. Landmarking was utilized to include serial assessments and stratified Cox proportional hazards regression analy

Association between clinical, serological, functional and radiological findings and ventilatory distribution heterogeneity in patients with rheumatoid arthritis.

BACKGROUND: In rheumatoid arthritis (RA), the involvement of the pulmonary interstitium can lead to structural changes in the small airways and alveoli, leading to reduced airflow and maldistribution of ventilation. The single-breath nitrogen washout (SBN2W) test is a measure of the ventilatory distribution heterogeneity and evaluates the small airways. This study aimed to find out which clinical, serological, functional and radiological findings are useful to identify RA patients with pathological values of the phase III slope (SIII) measured by the SBN2W test. METHODS: This was a cross-sect

Neck Flexion Strength as a Predictor of Need for Intubation in Guillain-Barre Syndrome.

OBJECTIVES: Respiratory failure in Guillain-Barre syndrome (GBS) is common. Forced vital capacity (FVC) is the gold standard for monitoring respiratory muscle strength in GBS. In some clinical situations, FVC testing could be delayed or unavailable, thus there is a need for accurate, fast, and device-free bedside respiratory evaluation. METHODS: We examined neck flexion strength in 23 GBS patients as a possible predictor of the need for subsequent intubation and as a predictor of FVC change. RESULTS: Intubation was required by 100% of patients with neck flexion strength of Medical Research C
