Sleep in human narcolepsy revisited with special reference to prior wakefulness duration.

Sleep of 11 narcoleptic subjects was recorded on baseline and after 16 and 24 hours of prior wakefulness (16 and 24 hours sleep deprivation). Eleven sex- and age-matched control subjects were recorded for comparisons. All recordings in narcoleptic subjects were characterized by frequent sleep onset rapid eye movement (REM) episodes, increased amounts of wake time after sleep onset and low sleep efficiencies. Mean total sleep time (TST) was significantly decreased in narcoleptic subjects after sleep deprivation (SD). Recovery sleep after 24 hours SD showed reduced nonREM (NREM) sleep stage 2 pe

Detecting EEG index of arousal in sleeping patients with unresponsive wakefulness syndrome.

Consciousness is assumed to be defined by two components: awareness and arousal. However, fewer electroencephalographic (EEG) indices have been identified for arousal than for awareness. Since patients clinically diagnosed with unresponsive wakefulness syndrome (UWS) show preserved sleep-wake cycles but markedly reduced reproducible behavioral evidence of awareness, their sleep-wake patterns may provide a useful model for examining arousal-related EEG fluctuations. This study recorded nighttime polysomnography in patients with UWS, patients in a minimally conscious state, and healthy controls.

Factors Affecting Sleep and Wakefulness in People with Epilepsy: A Narrative Review.

The importance of sleep has been reported for decades. Epilepsy is a heterogeneous disorder comprising multiple elements that might influence sleep and wakefulness. Notably, animal studies show disruptions of the circadian molecular system in different models of epilepsy, along with altered rest-activity and other circadian rhythms. So far, studies of molecular circadian systems in people with epilepsy are lacking, prompting further research. Seizures-the primary and most debilitating symptom of epilepsy-and interictal activity disrupt regular sleep and sleep-wake rhythms. Alterations in one's

Assessing Obstructive Sleep Apnea Severity During Wakefulness via Tracheal Breathing Sound Analysis.

Obstructive sleep apnea (OSA) is a commonly underdiagnosed condition that not only increases the risk of accidents but also significantly contributes to a wide range of health complications, including heightened perioperative morbidity and mortality risks during surgeries under general anesthesia. Polysomnography (PSG), which is the diagnostic gold standard, is costly, requires skilled technicians, is time-consuming, and is not always accessible. This study presents a fast, objective, and non-invasive method for detecting OSA severity by analyzing tracheal breathing sounds (TBS) recorded durin

An orexin agonist promotes wakefulness and inhibits cataplexy through distinct brain regions.

Narcolepsy type 1, caused by selective loss of the orexin-producing neurons, is characterized by poor maintenance of wakefulness and cataplexy. Clinical trials show that orexin receptor 2 (OX2R) agonists substantially improve narcolepsy symptoms, but the key brain regions through which OX2R signaling produces these benefits are only partially understood. To address this question, we produced recombinant mice expressing the human diphtheria toxin receptor driven by the endogenous orexin promoter (orexinDTR mice). After injection with diphtheria toxin, orexinDTR mice had severe and selective los

[Wakefulness-promoting agents for severe fatigue: to use or not to use?].

About 20% of adults experience excessive daytime sleepiness or severe fatigue. Causes include somatic conditions, psychiatric disorders, and medication or drug use. Treatment depends on the underlying cause. If sleepiness persists despite optimal treatment of the underlying condition, exclusion of other causes, and behavioral interventions, wakefulness-promoting agents may be considered. However, no established pharmacological strategy exists for symptomatic treatment. Modafinil and stimulants like methylphenidate may offer some benefit based on experiences with narcolepsy or idiopathic hypers

Relationship between location of epileptic focus and occurrence during sleep versus wakefulness.

Different sleep stages exert differential effects on interictal discharges, neural synchrony and seizure threshold. We sought to assess the relationship between localization of the epileptogenic focus and seizure distribution in sleep versus wakefulness among patients with refractory epilepsy. We conducted a retrospective chart review-based study. Video-electroencephalography of patients with refractory epilepsy, planned for resective surgery, were reviewed for seizure localisation and occurrence relative to stage of sleep/wakefulness. Demographic/clinical data, including details of surgery, w

Central Neural Circuits Orchestrating Thermogenesis, Sleep-Wakefulness States and General Anesthesia States.

Great progress has been made in specifically identifying the central neural circuits (CNCs) of the core body temperature (Tcore), sleep-wakefulness states (SWs), and general anesthesia states (GAs), mainly utilizing optogenetic or chemogenetic manipulations. We summarize the neuronal populations and neural pathways of these three CNCs, which gives evidence for the orchestration within these three CNCs, and the integrative regulation of these three CNCs by different environmental light signals. We also outline some transient receptor potential (TRP) channels that function in the CNCs-Tcore and

The relationship between fasting-induced torpor, sleep, and wakefulness in laboratory mice.

STUDY OBJECTIVES: Torpor is a regulated and reversible state of metabolic suppression used by many mammalian species to conserve energy. Whereas the relationship between torpor and sleep has been well-studied in seasonal hibernators, less is known about the effects of fasting-induced torpor on states of vigilance and brain activity in laboratory mice. METHODS: Continuous monitoring of electroencephalogram (EEG), electromyogram (EMG), and surface body temperature was undertaken in adult, male C57BL/6 mice over consecutive days of scheduled restricted feeding. RESULTS: All animals showed bouts

[Modulation by the hypocretinergic/orexinergic neurotransmission system in sleep-wakefulness cycle states].

INTRODUCTION: The hypocretins/orexins are neuropeptides synthesized by a small neuronal cell group located in the posterior and lateral hypothalamus. These peptides have been considered modulators of the sleep-wakefulness cycle since their discovery in 1998; the hypocretinergic/orexinergic system is very active during wakefulness. In addition, the absence of either these peptides or their receptors is associated to narcolepsy-cataplexy, a disease in which the sleep-wakefulness cycle is completely disorganized. DEVELOPMENT: Hypocretinergic/orexinergic neurons directly activate the cerebral cor

Roles of orexin/hypocretin in regulation of sleep/wakefulness and energy homeostasis.

The finding of orexin (hypocretin) deficiency in patients with narcolepsy suggests that this hypothalamic neuropeptide plays a crucial role in regulating and maintaining sleep/wakefulness states and energy homeostasis. Orexin might be especially important for stabilization of behavioral states, because the major symptom in narcolepsy is instability of each behavioral state, which results in sleep/wakefulness fragmentation. The efferent and afferent systems of orexin neurons suggest interactions between these cells and arousal/sleep-wakefulness centers in the brainstem as well as important feed

Effects of lateral preoptic area application of orexin-A on sleep-wakefulness.

Deficiency of orexin, a newly discovered hypothalamic peptide, is thought to lead to abnormal sleepiness and cataplexy in both human narcolepsy and animal models of the disease. As the POA contains extensive orexin terminals and is established as a sleep/arousal regulatory site, we evaluated a hypothesis that this site is a target for the arousal-inducing effects of orexin. Orexin-A was microinjected into lateral preoptic area (IPOA) and the effects on sleep-wakefulness and brain temperature were studied. Compared to saline vehicle control, orexin-A induced an increase in wakefulness for 70 mi

Effect of mandibular and tongue protrusion on upper airway size during wakefulness.

Oral appliances for the treatment of obstructive sleep apnea (OSA) produce either mandibular or tongue protrusion, and are thought to enlarge the upper airway (UA). We used videoendoscopy to measure UA cross-sectional area (CSA) and shape in the hypopharynx, oropharynx, and velopharynx during various stages of active mandibular and tongue protrusion during wakefulness in 10 patients with OSA and nine control subjects. Measurements were made in the supine position at end-tidal expiration, and were normalized to the CSA in the normal bite position. Airway shape was expressed as the anteroposteri

Gastroesophageal reflux and apnea in prematurely born infants during wakefulness and sleep.

The hypothesis that acid gastroesophageal reflux may be responsible for the persistence of apnea was tested on 20 prematurely born infants, at a median conceptional age of 38.7 weeks. Gastroesophageal reflux was identified using distal esophageal pH monitoring. Apneas of durations greater than 10 seconds were identified and classified as either central or obstructive and mixed, using recordings of respiration. Wakefulness, active sleep, and quiet sleep were identified using electroencephalography and by assessing eye movements. Of 134 episodes of acid gastroesophageal reflux in the 20 subjects

Abnormal central monoamine metabolism in humans with "true hypersomnia" and "sub-wakefulness".

A case of Kleine-Levin syndrome with true hypersomnia and a case of sub-wakefulness are described. In both patients lumbar cerebrospinal fluid homovanillic acid, 5-hydroxyindoleacetic acid, 3-methoxy-4-hydroxyphenylethylene glycol levels have been assayed during episodes of hypersomnia and normal sleep-waking cycles. Besides an increased 5-hydroxytryptamine turnover, mainly an increased dopamine turnover has been detected in both kinds of hypersomnia, and this finding was more remarkable in the case with sub-wakefulness. The probable role of dopamine in abnormalities in the sleep-waking cycle

GASEOUS METABOLISM IN THE INFANT: THE EFFECT OF SEDATION AND OF WAKEFULNESS.

Accurate oxygen consumption figures are desirable in quantitation of blood flow and shunts in infants with heart disease. Figures derived from normal infants fail to take into account effects of sedation or wakefulness.Oxygen consumption and ventilatory data were obtained in nine infants in natural sleep, in sedated sleep and in wakefulness. In sedated sleep oxygen consumption fell by a mean of 22% below the figure for natural sleep, and arterial carbondioxide tension rose by a mean of 7.04 mm. Hg. Infants who were awake but quiet (less than five limb movements per minute) showed an average in
