BACKGROUND: Survival outcomes in patients managed nonoperatively ("watch-and-wait"; W&W) have been compared to those achieving pathological complete response after total mesorectal excision (TME). However, limited data exists on how these outcomes align across the spectrum of treatment responses in TME patients.
METHOD: Adult patients at Mayo Clinic (2017-2023) with clinical TNM stage II/III microsatellite-stable primary rectal adenocarcinoma treated with total neoadjuvant therapy were categorized into two cohorts: W&W and TME. The TME cohort was stratified by American Joint Committee on Canc

BACKGROUND/AIMS: Gastrointestinal follicular lymphoma (GI-FL) patients with low tumor burden are often managed with a "watch and wait" (W&W) strategy, but regular endoscopic examinations impose a significant burden. We investigated whether the follicular dendritic cell (FDC) staining pattern of GI-FL lesions could help identify a lower-risk subgroup of patients during W&W follow-up.
METHODS: We studied 76 Japanese patients with GI-FL (September 2003-July 2022), classified by CD21 staining into "duodenal pattern" (n=57) and "nodal pattern" (n=19) groups. All patients underwent regular clinical

INTRODUCTION: Mind-body interventions (MBIs) are therapeutic practices that target the interactions between cognitive, emotional, and physiological systems to influence health outcomes. Previously, we demonstrated that MBI prolonged lymphocyte doubling time and treatment-free survival (TFS) in treatment-naïve chronic lymphocytic leukemia (CLL) patients during the watch-and-wait phase. In this follow-up study, we investigated the long-term effects of MBI on TFS after the intervention ceased.
METHODS: Sixty participants from the initial study (34 who received intervention vs. 26 controls) were

Follicular lymphoma (FL) patients with low tumor burden at diagnosis frequently undergo the watch-and-wait (W&W) strategy. The study aimed to facilitate risk assessment in predicting the time to lymphoma treatment (TLT) for W&W patients through an integrated analysis of clinical factors and genetic mutations. A retrospective study was conducted on 214 FL patients managed with W&W between 2016 and 2023. Among them, 184 patients underwent targeted sequencing. The median follow-up was 30.4 months (IQR 21.4-41.9, range 6.4-95.8). A clinico-genetic model m3-PRIMA-PI was developed using the multivar

INTRODUCTION: The watch-and-wait (WW) strategy is an alternative to anterior resection in patients with rectal cancer (RC) that have had a complete clinical response to neoadjuvant treatment. Few reports describe the quality of life and functional anorectal disorders (FADs) in that population.
AIM: To analyze and compare the FADs and quality of life in patients with locally advanced adenocarcinoma of the rectum treated with neoadjuvant therapy, divided into two different strategy groups: group 1 (G1), WW; and group 2 (G2), anterior resection.
MATERIALS AND METHODS: Thirty patients (G1: n = 2

BACKGROUND: Overdiagnosis and overtreatment are rarely discussed in the context of NTDs despite their relevance for patients under the care of health services with limited resources where the risks of therapy induced complications are often disproportionate to the benefit. The advantages of cyst staging-based management of patients with cystic echinococcosis (CE) are not yet fully explored. Questions are: Do inactive cysts (CE 4 and CE 5) need treatment and is there a difference between cysts which reach CE4 and CE5 naturally or by benzimidazole therapy?

Human cystic echinococcosis is a chronic, complex and neglected infection. Its clinical management has evolved over decades without adequate evaluation of efficacy. Recent expert opinion recommends that uncomplicated inactive cysts of the liver should be left untreated and solely monitored over time ("watch-and-wait" approach). However, clinical data supporting this approach are still scant and published mostly as conference proceedings. In this study, we report our experience with long-term sonographic and serological follow-up of inactive cysts of the liver. From March 1994 to October 2013,

BACKGROUND: The Watch-and-Wait (W&W) approach for rectal cancer aims to improve quality of life by avoiding total mesorectal excision in patients with a clinical complete response after neoadjuvant treatment. Over the past two decades, W&W has gained increasing acceptance and is now incorporated into several national guidelines. However, successful and safe implementation outside highly specialised centres encounters barriers at multiple levels.
OBJECTIVE: To describe the nationwide implementation of the W&W strategy for rectal cancer in the Netherlands and to identify key barriers at policy,

INTRODUCTION AND OBJECTIVE: Organ preservation strategies for rectal cancer following neoadjuvant treatment require intensive endoscopic monitoring to detect early luminal recurrence. This is termed 'Watch and Wait' (WAW). Standard protocols commonly mandate flexible sigmoidoscopy every 3-4 months for 2 years. Digital proctoscopy with the LumenEye device has been shown to be a safe alternative that can be performed in an outpatient setting without the need for sedation and with less staffing requirements. The study objective is to evaluate the economic implications of using digital proctoscopy

The rectum constitutes the distal segment of the large intestine, and rectal cancer typically arises through a multistep process in which normal epithelial cells undergo dysplastic transformation, eventually progressing to invasive carcinoma. This progression is often asymptomatic and may take years, making early detection challenging. Consequently, many cases are diagnosed at advanced stages. Management strategies are highly dependent on the stage at diagnosis. For T3N1M0 rectal cancers, current guidelines recommend neoadjuvant chemoradiotherapy followed by surgical resection. However, when t

BACKGROUND: Watch-and-wait management (WW) of rectal cancer is accepted for select patients with a complete clinical response (cCR) to neoadjuvant therapy (NAT). The appropriateness of WW for cancers beyond the reach of digital rectal examination (DRE) and/or with a delayed cCR is unclear.
METHODS: In this retrospective multicenter study, tumor palpability and clinical response were determined after NAT. Local regrowth-free survival (LRFS), disease-free survival (DFS), rectal organ preservation (OP), and salvage surgery outcomes were assessed.
RESULTS: Among 477 patients, 427 (90%) had palpa

INTRODUCTION: Complete surgical resection for locally advanced rectal cancer is the standard treatment after a clinical complete response following chemoradiotherapy. However, some novel clinical approaches could achieve better functional results, such as Robotic Resection, or avoiding surgical procedure and incrementing surveillance intensity, called Watch-and-Wait policy. We use computational techniques to compare these clinical approaches using quality adjusted life years (QALYs).
METHODS: A Markov decision analytic model was used in order to perform a cost-utility analysis, comparing stan
