To determine the efficacy of pneumatic retinopexy, the authors conducted a retrospective study of 56 cases of uncomplicated retinal detachments (RDs) treated with this technique (follow-up, 6-18 months). These were compared with 28 similar cases treated with a Lincoff balloon and 78 similar cases treated with a scleral buckle. The long-term success rates for these three groups were 71, 64, and 96%, with the incidence of new breaks being 20, 18, and 1.3% respectively. In aphakic or pseudophakic patients with an absent or ruptured posterior capsule, the success rate for pneumatic retinopexy was

The ocular pharmacokinetics of 5-fluorouracil (5-FU) after intravitreal administration in the rabbit are described for the first time. Following a 1 mg intravitreal injection of C-14 labeled 5-FU in the phakic nonvitrectomized eye, a peak level of 664 micrograms/ml is achieved which declines to 7.8 micrograms/ml at 24 hours. For the aphakic vitrectomized eye comparable values are 669 micrograms/ml at 0.1 hour and 0.21 micrograms/ml at 24 hours. 5-FU is cleared more than two times more rapidly in aphakic vitrectomized eyes than normal eyes during the first 12 hours following injection. A retrospective study of 36 second eyes with cataract, who have undergone prophylactic retinocryocoagulation prior to cataract extraction, is presented. All 36 patients suffered an aphakic retinal detachment in the first eye in the past. Postoperative complications included iritis, transient cycloplegia and retinal hemorrhages; two patients developed retinal tears following cataract surgery. During the average followup period of 8.4 years, no retinal detachment occurred among this group of patients. This study strongly supports the value of preventive treatment (retinocryocoagulation) of the s

Congenital and developmental cataracts are a major cause of visual impairment in children. They may occur as an isolated ocular finding in an otherwise healthy child or as part of a hereditary syndrome or congenital nongenetic disease. Management of significant unilateral and bilateral congenital cataracts involves early patient identification and evaluation followed by prompt surgery. Appropriate optical rehabilitation and occlusion therapy for amblyopia must begin shortly thereafter. The visual prognosis of a congenital cataract is poorer in unilateral than in bilateral cases and in total th

Collapse of the anterior chamber, choroidal and ciliary body detachments and ocular hypotony developed without explanation 2 weeks after uneventful intracapsular cataract extraction in a 60-year-old woman. Following an unsuccessful attempt to maintain an adequate chamber depth with air, sodium hyaluronate was injected into the chamber after surgical drainage of suprachoroidal fluid, allowing for spontaneous recovery of aqueous production while protecting the anterior segment from the ravages of chamber collapse.

An aphakic patient with a spontaneous intracorneal hemorrhage is described. The patient had been using a soft contact lens on a daily wear basis for 20 months. To my knowledge this paper represents the second report of this complication and the first in which the contact lens can be definitely implicated as the source of the corneal neovascularization and resultant hemorrhage. It is suggested that ophthalmologists observe their aphakic contact lens patients closely and either modify their regimens or discontinue the lenses with the development of corneal neovascularization.
