Cognitive function and mood during acute cold stress after extended military training and recovery.

INTRODUCTION: Acute cold stress is often accompanied by exposure to other adverse factors, such as sleep loss, under-nutrition, and psychological stress that singly and together may affect cognitive function. METHODS: The effect of moderate cold stress on cognitive function was investigated in 15 male volunteers exposed to cold air (10 degrees C) for 4 h after they had completed an intense, 61-d regimen (U.S. Army Ranger training). The single cohort of volunteers was tested on three separate occasions: (1) immediately after completing Ranger training; (2) 2 d later when they had partially rec

Suppression of visually induced vertigo: effects of attentional position and modality.

INTRODUCTION: Visually induced vertigo (VIV) is a serious problem because humans rely on vision more than any other sensory system to receive information from their surrounding environment. We investigated whether attention to simultaneous visual or auditory tasks would reduce VIV. METHODS: Subjects were four women and one man who were highly susceptible to VIV and four women who had low susceptibility to VIV. An animation display was prepared to test the effect of overlapping visual task stimuli when the position of the stimuli on the screen was changed. The effect of modality was tested by

Seating type and cognitive performance after 3 hours travel by high-speed boat in sea states 2-3.

INTRODUCTION: Transit in high-speed marine craft subjects occupants to a rough ride as the boat impacts the waves. This induces high levels of physical stress, which may inhibit cognitive performance during military operations and life-saving activities. Land-based research suggests that suspension seats reduce vibration and, therefore, stress. We hypothesized that subjects using suspension seats would demonstrate better cognitive performance, lower perceptions of exertion, fatigue, and sleepiness, and lower salivary concentrations of cortisol than those using fixed seats. METHODS: Subjects,

AED use in a passenger during a long-haul flight: repeated defibrillation with a successful outcome.

INTRODUCTION: Sudden cardiac arrest is one of the leading causes of death, and early defibrillation of ventricular fibrillation (VF) is the single most important intervention for improving survival. The automated external defibrillator (AED) and the concept of public access defibrillation provide a solution to shorten defibrillation delays. Commercial aircraft create a unique environment for the use of the AED since an emergency medical service system (EMS) response is not available. We review published studies on this subject and describe the case of a passenger who developed VF during an int

Hypoxic syncope in a competitive breath-hold diver with elevation of the brain damage marker S100B.

INTRODUCTION: Competitive breath-hold divers can accomplish previously unbelievable performances; e.g., the current world record for apnea during rest ("static apnea") is 11 min 35 s. However, whether such performances are associated with a risk for hypoxic brain damage has not been established. CASE REPORT: A breath-hold diver's competitive performance resulted in a loss of consciousness, after which he was subjected to a medical examination by the event physician. Blood samples were collected for analysis of the brain damage marker S100B in serum. The S100B in serum was 0.100 microg x L(-1)

Motion sickness with fully roll-compensated lateral oscillation: effect of oscillation frequency.

BACKGROUND: During lateral acceleration, the addition of an appropriate roll motion can improve comfort, but some combinations of lateral and roll motion increase motion sickness. OBJECTIVES: To determine how motion sickness caused by lateral oscillation fully compensated by roll oscillation (so subjects feel no lateral acceleration) depends on the frequency of oscillation and compare sickness with that caused by uncompensated lateral oscillation. METHOD: A total of 160 subjects (8 groups of 20) were exposed for 30 min to fully roll-compensated sinusoidal lateral oscillation at one of 8 freq

Simulator sickness in a helicopter flight training school.

INTRODUCTION: Simulator sickness (SS) is a common problem during flight training and can affect both instructor pilots (IP) and student pilots (SP). This study was conducted in response to complaints about a high incidence of SS associated with use of new simulators for rotary-wing aircraft. METHODS: The problem was evaluated using the Simulator Sickness Questionnaire (SSQ) to collect data on 73 IP and 129 SP who used the new simulators. Based on analysis of these data, operator comments, and a search of the literature, we recommended limiting simulator flights to 2 h, removing unusual or unn

B-type natriuretic peptide after open-water and hyperbaric chamber exposure to 10 msw.

INTRODUCTION: Hyperbaric environment exposure in humans has cardiovascular effects mainly characterized by an increase in afterload and a decrease in cardiac output. In a previous study we did not find B-type natriuretic peptide (BNP) changes in healthy volunteers exposed to hyperbaric oxygen while other authors documented a significant increase in N-terminal pro-BNP after scuba diving. On the basis of these data we hypothesized that dry hyperbaric exposure and scuba diving could have different effects on BNP secretion. METHODS: Nine healthy volunteers performed a 1-h open-sea air dive at 10

Zolpidem in fatigue management for surge operations of remotely piloted aircraft.

INTRODUCTION: Zolpidem is a hypnotic medication approved to manage sleep for crewmembers involved in combat operations in the USAF and other services. Previous studies demonstrated the effectiveness of zolpidem under laboratory conditions, but confirmation from the field is needed. METHODS: We evaluated zolpidem in managing sleep-work cycles for crews of remotely piloted aircraft (RPA) during surge combat operations. There were 3 10-mg tablets of zolpidem dispensed to each of 43 crewmembers tasked to support RPA surge combat operations requiring rapid unscheduled shift changes. All personnel

Different flying histories in flying phobics: association with psychopathology and treatment outcome.

INTRODUCTION: Passengers experiencing fear of flying can threaten the safety of a flight, its passengers, and crew. In the present study we investigated the effect of different flying histories on the nature and treatment of fear of flying and attempted to determine the following: 1) the prevalence of different flying histories in a sample of self-referred flying phobics; 2) the demographic and psychopathologic characteristics of flying phobics differing with respect to flying history; and 3) the predictive value of different flying histories for treatment outcome. METHODS: Of 2001 self-refer

Motion sickness: effect of the magnitude of roll and pitch oscillation.

BACKGROUND: Rotational oscillation in roll and pitch can cause motion sickness, but it is not known how sickness depends on the magnitude of rotational oscillation or whether there is a difference between the two axes of motion. HYPOTHESIS: It was hypothesized that motion sickness would increase similarly with increasing magnitudes of roll and pitch oscillation. METHOD: There were 120 subjects (6 groups of 20 subjects) who were exposed to 30 min of 0.2-Hz sinusoidal roll or pitch oscillation at 1 of 3 magnitudes: 1) +/- 1.830; 2) +/- 3.66 degrees; or 3) +/- 7.32 degrees. Orthostatic response in rats after hindlimb unloading: effect of transcranial electrical stimulation.

INTRODUCTION: Orthostatic hypotension is a commonly observed phenomenon after exposure to microgravity and in various forms of autonomic failure. It has been suggested that insufficient activation of supraspinal structures responsible for descending sympathetic drive could play a significant role in this disorder. We examined the effect of transcranial electrical stimulation (TES) of autonomic nuclei within the brain on the orthostatic hypotension induced by exposure to simulated microgravity using a hindlimb unloading model. METHODS: There were 20 male Wistar rats that were suspended by thei

Episodic hypoxemia in an airline passenger with chronic respiratory failure on supplemental oxygen.

Assessing the requirements for in-flight oxygen in passengers with pulmonary limitations can be a challenging task for clinicians. Aeromedical guidelines are available to help identify passengers that may require oxygen in flight. However, little is known about the actual in-flight response to passengers on oxygen. We measured the oxygen response (pulse oximetry) of a 67-yr-old female patient with chronic respiratory failure during a trans-Tasman flight (duration 170 min). This patient was assessed at the respiratory clinic before her journey and resting PaO2 (57 mmHg) indicated the requiremen

Vestibular symptoms and otoneurological findings in retired offshore divers.

INTRODUCTION: Inner ear barotraumas and decompression sickness (DCS) may cause acute vestibular symptoms in divers. The result may be irreversible damage to the vestibular end organs or their central connections. We examined a group of offshore divers in order to find out how many divers experience vestibular symptoms later in life and how this was related to occupational history and objective findings. METHODS: A questionnaire was sent to 230 offshore divers (mean age 52 yr) and 166 age-matched non-diving controls. Most of the divers had retired from diving. A subgroup (n=96) of the divers w
